When Captain Siborne died in 1849, it is unlikely that he was aware of the enduring historical legacy that he was to leave behind. His History of the War in France and Belgium in 1815 has become the most well known English history of the famous campaign and despite being written over 150 years ago is still in print, still eminently readable and remarkably accurate. The book was the result of his life’s work and passionate dedication to the “Waterloo Model” which depicts a stage of the battle in tremendous detail. The accuracy of the book is accounted for by four tremendously important points; Firstly, Siborne was engaged by the British military establishment to produce a model of the battle of Waterloo, which he did with scrupulous accuracy including painstaking research on the battle ground and environs including surveys of the ground. Secondly, Siborne was a noted topographical engineer who wrote a number of treatises and one of the standard works of the time enabling his appreciation of the battle to be precise and avoid fault of many histories written merely from maps (some produced years afterward)of the area. Thirdly, he undertook what was a the time a ground-breaking “questionnaire” of the surviving officers of the British, King’s German Legion, Hanoverian units involved, to piece together the events of the day. These letters were published in part by Siborne’s son much later. Fourthly he expanded his search for eye-witness testimony to both the Prussian and French army staffs, and although rebuffed by the French, who were understandably tender about the loss of the battle and their Emperor with it, his enquiries were fruitful amongst the Prussian command who supplied a priceless counterbalance to the sometimes jingoistic British accounts. Siborne and his works were ahead of their time, and his search for an accurate representation of the battle won him few friends at Horse Guards. Funding was difficult to obtain from the British establishment and Siborne’s attempts at self-funding the model which was his life’s work were unsuccessful, Siborne died a broken man. He left behind the “Waterloo Model” and a larger scale model which are housed at the Royal Army Museum in London and this excellent book. We chose the third edition as it includes the impassioned defence of his work against the plagiarism of Rev R Gleig’s “Story of Waterloo” and a number of notable changes from the first and second editions prompted by further eye-witness testimony gathered by Siborne. In 2012 we received a grant from the Veterans Health Administration Office of Specialty Care entitled, “Patient-Centered Model for the Management of Chronic Obstructive Pulmonary Disease.” The grant’s goals were to enhance the recognition and diagnosis of COPD and implement a Patient-Centered Model for the Management of COPD. As the work on that proposal progressed, we realized that providers did not have an up-to-date, comprehensive, easily read, “how to” manual for the management of COPD despite all the advances in COPD care that have occurred over the past 5 years. Consensus documents such as the VA-DOD Guidelines were abbreviated summaries that were rarely used. From those discussions, the concept for this volume, a COPD Primer, developed. The goal was to develop a practical book that concisely presented COPD to providers with sufficient background and explanation of the physiologic and scientific rationale for various management strategies without becoming an esoteric academic work. We hope that this COPD Primer has achieved that goal and will be a useful, practical text for practitioners and medical trainees alike. The COPD Primer begins with an examination of what COPD is; it is really a syndrome, a constellation of historical features and clinical, physiologic, and radiographic findings. However, those elements come together in many different ways to create multiple different COPD phenotypes that are only now being recognized and used to define specific management strategies. COPD research has progressed beyond the simple classification of “blue bloaters” and “pink puffers.” Next, the epidemiology and economic consequences of COPD are reviewed. Bill Eschenbacher presents an approach to the patient with respiratory symptoms with detailed discussions of pulmonary function testing and how airflow limitation/obstruction is identified by spirometry and the use of lung imaging to identify individuals with COPD. This COPD Primer incorporates the knowledge that we have learned over the past several years during the development and implementation of a patient-centered model for the management of COPD. It was written with the explicit goal of assisting both the practicing provider and medical trainee in the care of patients with COPD.
Remarks by JVS. Volumes 1 and 2 of Feldspar Minerals were published in 1974, but Volume 3 was not completed because I was forced to devote 3 years to the resolution of unforeseen problems in the construction of an ion probe. By 1977, the incomplete draft for Volume 3 had become obsolete because of the enormous advances in knowledge of feldspars, particularly those in lunar rocks and meteorites, and in both deep-seated and ancient terrestrial rocks. Furthermore, it soon became obvious that a completely new version of Feldspar Minerals was needed because of the important new results on the physical and chemical properties. I had kept up with the interesting but tedious chore of weekly reading of the incoming literature and maintenance of the files. By 1980, the intense day-to day pressure had gone from my research programs on lunar rocks and on the development of the ion microprobe as a quantitative geochemical instrument, and I began preparation of a second edition of Feldspar Minerals.
Rush to Policy explores the appropriate role of technical analysis in policy formulation. The authors ask when and how the use of sophisticated analytic techniques in decision-making benefits the nation. They argues that these techniques are too often used in situations where they may not be needed or understood by the decision maker, where they may not be to answer the questions raised but are nonetheless required by law. House and Shull provide an excellent empirical base for describing the impact of politics on policies, policy analysis, and policy analysts. They examine cost-benefit analysis, risk analysis, and decision analysis and assess their ability to substitute for the current decision-making process in the public sector. They examine the political basis of public sector decision-making, how individuals and organizations make decisions, and the ways decisions are made in the federal sector. Also, they discuss the mandate to use these methods in the policy formulation process. The book is written by two practicing federal policy analysts who, in a decade of service as policy researchers, developed sophisticated quantitative analytic and decision-making techniques. They then spent several years trying to use them in the real world. Success and failures are described in illuminating detail, providing insight not commonly found in such critiques. The authors delineate the interaction of politics and technical issues. Their book describes policy analysis as it is, not how it ought to be. Peter W. House is the director of policy research and analysis at the National Science Foundation. He is the author of ten books on multidisciplinary science and technology policy research and analyses in government, private, and university sectors, including The Art of Public Policy Analysis and with Roger D. Shull, Regulatory Reform: Politics and the Environment and Regulations and Science: Management of Research on Demand. Roger D. Shull is a senior analyst at the Division of Policy Research and Analysis, National Science Foundation.
