A vascular system consists of a supplying arterial and a draining venous part which are connected by a terminal vascular network. The arterial segment can be characterized according to the structural features of the vessel wall. However, it is sometimes diffi cult to distinguish the capillary from the postcapillary vessels on the basis of structural features alone. On the other hand, physiologic qualities such as permeability can hard ly be associated with an equivalent histologic pattern of the vessel wall (lllig 1961; Rhodin 1967, 1968; Hauck 1971; Westergaard 1974). A defmition of a vascular seg ment based on biologic significance should combine morphological and functional qualities of the vessel walls. During the ontogeny of the mammalian organism a variety of vascular patterns (e. g., distribution of arteries and veins, arrangement of the capillaries) has been formed typical of each organ (Wolff et al. 1975; Baez 1977). The capillaries connect the feed ing arterioles and the collecting venules in two different ways according to the branch ing pattern of the terminal vessels (Hauck 1975, Wolff et al., 1975). The arterioles and venules are directly connected by capillary segments. Consequently a terminal vessel called arteriovenous (a-v) capillary results, or a closely meshed capillary network is de veloped which connects arterioles and venules by a variable number of small capillary branches arranged parallel to the preexisting a-v capillary.
Until 1947, professional ball players were paid only from opening day to season's end. Even during the season, a lot of their expenses came out of their own pockets. Even the best-paid players had trouble making ends meet. One answer to their money woes was barnstorming--tours out of season. Cities lacking their own major league teams were happy to host big-league players for such events, as well as for special exhibition games whose proceeds sometimes went to local charities. Here is a history of barnstorming and exhibition games from 1901 (when both of the two current major leagues began operating) through 1962 (when a team led by Willie Mays was unsuccessful in its attempt at a tour, signaling an end to true barnstorming). Nasal continuous positive airway pressure ventilation is the gold standard in the treatment of obstructive sleep apnea. Long-term compliance rates are about 60%. Therefore, several alternative treatment options are of special interest. Beside conservative therapies, various surgical concepts exist. The field of surgery for sleep disordered breathing has rapidly grown with new instrumentation and surgical techniques in the last 10 years. Surgeons in these fields have to attend scientific meetings, participate in workshops, and read the literature to stay up to date. In our sleep laboratories we conduct 30 polysomnographies each night. Each year, we perform almost 1,000 surgical procedures for sleep disordered breathing apart from numerous other conservative and apparative treatment modalities. Referring to our expe- ence and the present literature, we tried to give new information on surgical techniques in this second edition. The chapters are grouped in different anatomical fields of interest. We wanted to give general advice and specific new hints for the surgery of sleep disordered breathing so that the reader learns basic techniques followed by more advanced surgery. In addition to the illustrated surgical descriptions, the chapters contain informations about indications and contraindications of each surgical procedure and the postoperative care. Special interest has been dedicated to evidence-based medicine. So in each chapter, there is a table of references summarizing the effectiveness of the procedure and EBM grade.

Certain individuals find a singular moment in life used to portray them and/or define them, if not basically brand them. In the world of sports, particularly baseball, this practice is raised to a level that is questionable at best, laughable in most instances and blatantly unfair as a rule. The sports media, along with a growing portion of the general public that refuses to form their own opinions, goes to extremes to constantly relive an individuals weak moments. Curiously, they often then close their ridiculing diatribes by mentioning that labeling a particular person is really unfair. In Never These Men, Thomas Porky McDonald, whose previous works stressed the notion that a game like baseball is there for the joy it gives to those who truly love it and understand it, takes a peek at a small collection of the most famous (or is it infamous?) of these media-fueled characters. The idea that someone who cannot possibly do something (play professional sports) might then play judge and jury on those that can (athletes) seems absurd to McDonald, who clearly feels that the ever-growing rash of media outlets, in concert with an unthinking generation of spectators, has only bloated the array of unfounded criticisms and hypocritical rhetoric within our midst. From Fred Merkle, an early media creation, through to Bill Buckner, a truly fine and underrated ballplayer, Never These Men fundamentally asks the reader to imagine how it feels to be branded for a singular moment in ones life. McDonald, foremost a poet, liberally spreads a few relevant original verses throughout this volume, which is fundamentally a call for fair play. The idea of balanced and proper reporting is considered as well, as in the cases of Ralph Branca, Mitch Williams and Ralph Terry. Though all three were solid Major Leaguers who gave up famous home runs, only the former two are constantly cited, a point of contention here. In the area of authority figures, why Charles Comiskey and Judge Landis are barely scrutinized for their truly abhorrent behavior, while a lifetime baseball man like Gene Mauch is merely brushed aside by far too many is a question that clearly haunts this writer. Never These Men asserts that working in a world with little or no accountability, while demanding total accountability from those whose skill and expertise literally creates your professional existence, is an absurdity that needs to be addressed.
