Errors occur commonly in healthcare and can cause significant harm to patients. Most errors arise from a combination of individual, system, and communication failures. Neurologists may be involved in harmful errors in any practice setting and should familiarize themselves with tools to prevent, report, and examine errors. Although physicians, patients, and ethicists endorse candid disclosure of harmful medical errors to patients, many physicians express uncertainty about how to approach these conversations. A growing body of research indicates physicians often fail to meet patient expectations for timely and open disclosure. Patients desire information about the error, an apology, and a plan for preventing recurrence of the error. To meet these expectations, physicians should participate in event investigations and plan thoroughly for each disclosure conversation, preferably with a disclosure coach. Physicians should also anticipate and attend to the ongoing medical and emotional needs of the patient. A cultural change towards greater transparency following medical errors is in motion. Substantial progress is still required, but neurologists can further this movement by promoting policies and environments conducive to open reporting, respectful disclosure to patients, and support for the healthcare workers involved.

The 637 documents in this volume span 1 February to 31 August 1819. As a founding member of the University of Virginia Board of Visitors, Jefferson helps to obtain builders for the infant institution, responds to those seeking professorships, and orchestrates the establishment of a classical preparatory school in Charlottesville. In a letter to Vine Utley, Jefferson details his daily regimen of a largely vegetarian diet, bathing his feet in cold water each morning, and horseback riding. Continuing to indulge his wide-ranging intellectual interests, Jefferson receives publications on the proper pronunciation of Greek and discusses the subject himself in a letter to John Adams. Jefferson also experiences worrying and painful events, including hailstorm damage at his Poplar Forest estate, a fire in the North Pavilion at Monticello, the illness of his slave Burwell Colbert, and a fracas in which Jefferson's grandson-in-law Charles Bankhead stabs Jefferson's grandson Thomas Jefferson Randolph on court day in Charlottesville. Worst of all, Jefferson's financial problems greatly increase when the bankruptcy of his friend Wilson Cary Nicholas leaves Jefferson responsible for $20,000 in notes he had endorsed for Nicholas.

Sex offender registers are not really registers at all but a set of ‘legal requirements’ that fall automatically on a person convicted or cautioned for a designated sexual offence; the term ‘register’ is a form of shorthand for these requirements, designed to be a contribution to greater public protection and community safety. This book provides the first serious and detailed narrative of the conception and implementation of the sex offender registers. It looks in detail at the practice of implementing registers and considers questions about their effectiveness in monitoring sex offenders and the implications of someone being on a sex offender register.
