The cerebello-pontine angle has always posed a challenge to the neurosurgeon, the otoneurosurgeon, and the neuroradiologist. Angle masses which are very small and difficult to detect frequently produce symptoms, but may remain silent while growing to exceptional size. The neuroradiologist must have firm knowl edge of the clinical manifestations of the diverse angle lesions in order to tailor his studies to the patients' needs. The majority of angle lesions are benign; thus successful surgery has the potential for complete cure. Angle lesions typically arise in conjunction with vital neurovascular structures, and often displace these away from their expected positions. Large lesions may attenuate the vestibulocochlear and facial nerves and thin them over their dome. Since the nerves often remain functional, the surgeon then faces the need to separate the tumor from the contiguous nerve, with preservation of neurological function. Depending on the exact location and extension of the lesion, resection may best be attempted via otologic or neurosurgical approaches. The neuroradiologist must determine - precisely -the presence, site, size, and extension( s) of the lesion and the displacement of vital neurovascular structures as a guide to selecting the line of surgical attack. Since the arteries, veins, and nerves that traverse the angle are fine structures, the neuroradiologist must perform studies of the highest quality to do his job effectively.

Benedictine scholars around 1700, most prominently proponents of historical criticism, have long been regarded as the spearhead of ecclesiastical learning on the brink of Enlightenment, first in France, then in Germany and other parts of Europe. Based on unpublished sources, this book is the first to contextualize this narrative in its highly complex pre-modern setting, and thus at some distance from modernist ascriptions ex posteriori. Challenged by Protestant and Catholic anti-monasticism, Benedictine scholars strove to maintain control of their intellectual tradition. They failed thoroughly, however: in the Holy Roman Empire, their success depended on an anti-Roman and nationalized reading of their research. For them, becoming part of an Enlightenment narrative meant becoming part of a cultural project of “Germany”.

Special emphasis has been put on making the correct diagnosis, and detailed recommendations have been given for specific treatments. The second edition has resulted in an almost completely new book due to the dramatic expansion in the 1990s of our understanding of vestibular function and dis orders. A few rele vant examples include the novel concept of canalolithiasis, as opposed to cupulolithiasis, both of which are established causes of typical posterior and horizontal canal benign paroxysmal positioning vertigo; familial episodic ataxia land II have been identified as inherited chan nelopathies; otolithic syndromes were recognized as a variety separate from semicircular canal syndromes; several new central vestibular syndromes have been described, localized, and attributed to vestibular pathways and centres; a new classification based on the three major planes of action of the vestibulo-ocular reflex is available for central vestibular syn dromes; and the mystery of the location and function of the multisensory vestibular cortex is slowly being unravelled. This book differs from other clinical textbooks in that it is not divided into two parts: anatomy and physiology, on the one hand, and disorders, on the other.

There is a distinction between a contrite sense of sin and a feeling of guilt. The former is a true and healthy thing, the latter tends to be false and pathological. The man who suffers from a sense of guilt does not want to feel guilty, but at the same time he does not want to be innocent. He wants to do what he thinks he must not do, without the pain of worrying about the consequences. The history of our time has been made by dictators whose characters, often transparently easy to read, have been full of repressed guilt. They have managed to enlist the support of masses of men moved by the same repressed drives as themselves. Modern dictatorships display everywhere a deliberate and calculated hatred for human nature as such. The technique of degradation used in concentration camps and in staged trials are all too familiar in our time. They have one purpose: to defile the human person.

While women in developing countries continue to die in large numbers in child birth, Population and Reproductive Health specialists and advocates around the world are struggling to keep the policy agenda focused on the rights and needs of poor women. The 1994 Cairo Conference and Program of Action changed how we do business, and opened many doors, but the agenda is not complete and has stalled in a number of ways. At the country level, governments and donors are making difficult choices about how and where to allocate scarce human and financial resources. Funding approaches have moved away from the implementation of narrowly directed health programs to a broader approach of health system development and reform. At the same time, countries are also centering their development agenda on the broad goal of poverty reduction. This volume addresses a large knowledge and capacity gap in the Reproductive Health community and provides tools for key actors to empower faster positive change. It is a synopsis of the materials developed for WBI's learning program on Achieving the Millennium Development Goals: Poverty Reduction, Reproductive Health and Health Sector Reform.
