Americans are fond of reflecting upon the Founding Fathers, the noble group of men who came together to force out the tyranny of the British and bring democracy to the land. Unfortunately, as Terry Bouton shows in this highly provocative first book, the Revolutionary elite often seemed as determined to squash democracy after the war as they were to support it before. Centering on Pennsylvania, the symbolic and logistical center of the Revolution, Bouton shows how this radical shift in ideology spelled tragedy for hundreds of common people. Leading up to the Revolution, Pennsylvanians were united in their opinion that "the people" (i.e. white men) should be given access to the political system, and that some degree of wealth equality (i.e. among white men) was required to ensure that political freedom prevailed. As the war ended, Pennsylvania's elites began brushing aside these ideas, using their political power to pass laws to enrich their own estates and hinder political organization by their opponents. By the 1780s, they had reenacted many of the same laws that they had gone to war to abolish, returning Pennsylvania to a state of economic depression and political hegemony. This unhappy situation led directly to the Whiskey and Fries rebellions, popular uprisings both put down by federal armies. Bouton's work reveals a unique perspective, showing intimately how the war and the events that followed affected poor farmers and working people. Bouton introduces us to unsung heroes from this time--farmers, weavers, and tailors who put their lives on hold to fight to save democracy from the forces of "united avarice." We also get a starkly new look at some familiar characters from the Revolution, including Benjamin Franklin, Alexander Hamilton, and George Washington, who Bouton strives to make readers see as real, flawed people, blinded by their own sense of entitlement. Taming Democracy represents a turning point in how we view the outcomes of the Revolutionary War and the motivations of the powerful men who led it. Policy makers--Republican and Democrat, liberal and conservative--call for federal intervention to fund emerging high-growth industries, believing they are starved for capital. Congressional hearings, newspapers, industry newsletters, and government reports all assert that capital gaps exist for these firms. But the widely held belief that emerging high-growth firms like those in high technology--so vital to the growth of the U.S. economy--face severe capital gaps, preventing them from starting up or growing to their full potential, is false. This book systematically brings together, for the first time, disparate sources of information from a wide variety of disciplines and synthesizes them into a compelling case against federal intervention. Scientific studies, conventional wisdom among entrepreneurs and investors, and economic reasoning all fail to support the existence of widespread capital gaps for start-up high-growth firms. Nor does this evidence show capital in short supply in some regions, in industrial sectors including high technology, or for women and minorities. Nor do existing federal programs providing capital to emerging high-growth businesses reveal capital gaps. Rather, they either unnecessarily duplicate private investment or represent poor investment decisions. This study shows that calls for increased federal intervention, using public monies to plug capital gaps, are unjustified.

U.S. health care is a $2.5 trillion system that accounts for more than 17 percent of the nation’s GDP. It is also highly susceptible to fraud. Estimates vary, but some observers believe that as much as 10 percent of all medical billing involves some type of fraud. In 2009, New York’s Medicaid fraud office recovered $283 million and obtained 148 criminal convictions. In July 2010, the U.S. Justice Department charged nearly 100 patients, doctors, and health care executives in five states of bilking the Medicare system out of more than $251 million through false claims for services that were medically unnecessary or never provided. These cases only hint at the scope of the problem. In Phantom Billing, Fake Prescriptions, and the High Cost of Medicine, Terry L. Leap takes on medical fraud and its economic, psychological, and social costs. Illustrated throughout with dozens of specific and often fascinating cases, this book covers a wide variety of crimes: kickbacks, illicit referrals, overcharging and double billing, upcoding, unbundling, rent-a-patient and pill-mill schemes, insurance scams, short-pilling, off-label marketing of pharmaceuticals, and rebate fraud, as well as criminal acts that enable this fraud (mail and wire fraud, conspiracy, and money laundering). After assessing the effectiveness of the federal laws designed to fight health care fraud and abuse—the antikickback statute, the Stark Law, the False Claims Act, HIPAA, and the food and drug laws—Leap suggests a number of ways that health care providers, consumers, insurers, and federal and state officials can bring health care fraud and abuse under control, thereby reducing the overall cost of medical care in America.
