Over the past two decades, the assessment of Attention-Deficit/Hyperactivity Disorder (AD/HD) has evolved into a sophisticated balance of science and clinical judgement essential for arriving at reliable and valid diagnostic de- sions. Because of the precarious mix of clinical and empirical skill needed to evaluate children with this disorder, diagnostic practice in this area has been found wanting by many critics. In fact, a 1998 National Institutes of Health consensus panel concluded that “existing diagnostic treatment practices ... point to the need for improved awareness by the health service sector conce- ing an appropriate assessment, treatment, and follow-up. A more consistent set of diagnostic procedures and practice guidelines is of utmost importance” (p. 21). Drs. Arthur D. Anastopoulos and Terri L. Shelton have designed a book that addresses this need. A number of themes are highlighted throughout the text. Perhaps the most important is that the assessment guidelines set forth in this book represent a balance between science and practice. The authors account for the realities of clinical practice in an age of managed care while challenging clinicians to heed the lessons of empirical research. Although the use of empirically based asse- ment procedures may at times fly in the face of cost constraints (e. g. , systematic evaluation of medication effects), the authors present a strong argument for them. Further, they call upon their vast clinical experience to provide concrete suggestions for translating research findings into effective evaluations.

Carmel

Carmel started as a small trading post and farming community in 1836 but has long been regarded as a gateway to Indiana's capital city. The nickname "North Gate of Indianapolis" was adopted by Carmel's centennial committee, reflecting the town's appreciation of the big-city association. Carmelites could enjoy the charm of small-town living along with the amenities of a large city the distance of a short train ride. For decades, Carmel remained nearly unchanged from its one-stoplight status. The 1950s marked the start of major changes. Affordable automobiles and better roads helped create the demise of the railroad to Carmel but enhanced the suburb's appeal to families. With the ease of transportation to Indianapolis and a reputation for excellent schools, Carmel began to witness a steady migration of new residents. By 1975, the town had experienced the beginning of a housing boom and increased its size at least tenfold by 2006. As a result, Carmel has a new persona, a city independent of its big sister to the south with its own healthy business environment and cultural attractions.

Caring for the Family Caregiver is an extensive practical tool kit for health care providers across the healthcare continuum. Regardless if it is a mother caring for a child with a developmental disability, a wife caring for a husband with a long term chronic illness, or a daughter sitting at the bedside of her father who is enrolled in hospice, family caregivers are the silent "other patient" in the health care drama. Healthcare providers who do not attend to the needs of the caregiver not only inflict interactional suffering, but dilute their treatment by not engaging the caregiver as a partner. In fact, they may unintentionally do harm as the caregiver flounders and thus patient treatment fails. As noted by one dying cancer patient in an educational YouTube video of his cancer journey, "there are two patients not one." If we are to eliminate the interactional suffering experienced by family caregivers, we must train both the caregiver and the health care team for the important interaction and roles that are required for the successful care of the patient. Caregivers lack information, skills, and emotional support for the tireless task they are volunteering for. They need to be taught how to advocate for themselves and their patients and how to best communicate with the health care team. Likewise, health care providers have the skills and knowledge to provide outstanding patient centered care; however, they are not taught the importance of the family caregiver, nor do they always understand that experience or how to help"--
