Now published in two volumes to accommodate new chapters on the many advances in understanding and treatment options, this set of volumes represents the definitive reference on inflammatory bowel disease, a spectrum of diseases that is receiving increasing attention as our understanding of the etiological factors increases and diagnostic tools are refined. Basic research accelerated at the beginning of this decade and is now yielding new, more targeted treatments than were available just a few years ago. Volume 1 is on IBD and Ulcerative Colitis, and Volume 2 is on IBD and Crohn's Disease. All areas that were covered in the 2nd edition have been expanded and updated. New sections include the rapidly expanding knowledge of genetics and the role of the intestinal flora and environmental factors in etiology and pathogenesis. Among the 168 chapters, there are 20 on biologic therapies and 30 on surgical management. The consultant/authors clearly state their approach to important issues, such as the duration of immunomodulator and of anti-TNF-a use and the options for managing isolated low-grade dysplasia. A strong focus on the individual patient is woven throughout both volumes, including the benefits and risks of potentially life-altering therapies and surgeries. Entire sections detail concerns about the well-being of each person. This book provides information for health professionals who help both pediatric and adult patients navigate through the lifelong shadow of a chronic, probably genetically determined ordeal.

In the past decade, primal-dual algorithms have emerged as the most important and useful algorithms from the interior-point class. This book presents the major primal-dual algorithms for linear programming in straightforward terms. A thorough description of the theoretical properties of these methods is given, as are a discussion of practical and computational aspects and a summary of current software. This is an excellent, timely, and well-written work. The major primal-dual algorithms covered in this book are path-following algorithms (short- and long-step, predictor-corrector), potential-reduction algorithms, and infeasible-interior-point algorithms. A unified treatment of superlinear convergence, finite termination, and detection of infeasible problems is presented. Issues relevant to practical implementation are also discussed, including sparse linear algebra and a complete specification of Mehrotra's predictor-corrector algorithm. Also treated are extensions of primal-dual algorithms to more general problems such as monotone complementarity, semidefinite programming, and general convex programming problems.

This enhanced edition contains match footage highlights from every tour from 1955 to 2009, additional photographs and text, as well as a statistical section and an abridged history of the Lions. This is the history of the British & Irish Lions... in their own words. For 125 years the British & Irish Lions have stood out as a symbol of the ethics, values and romance at the heart of rugby union. To represent the Lions is the pinnacle for every international player in Britain and Ireland, and the dream of tens of thousands of avid fans who follow them. A Lions tour, undertaken every four years to the southern hemisphere, is more than a series of rugby matches played out on foreign fields; it is an epic crusade where the chosen few face a succession of mental and physical challenges on their way to the Test arena, where they do battle with the superpowers of the world game. Behind the Lions sees four esteemed rugby writers from each of the Home Nations delve to the very heart of what it means to be a Lion, using diaries and letters from those who pioneered the concept, to interviews with a vast array of players who have followed in their footsteps. In so doing they have uncovered the passion, pride and honour experienced when taking up the unique challenge of a Lions tour. This is a tale of heart-break and ecstasy, humour and poignancy that is at once inspirational, moving and utterly compelling. And it is the only story worth hearing: the players' own.
Shortly before Gauguin made his first Tahitian journey in 1891, he spent nearly two years in the remote Breton fishing village of Le Pouldu. Seeking creative isolation in a "primitive" setting, he pursued his art accompanied by several followers. One of them was the Dutch painter Meyer de Haan, who was able to pay the living expenses in Le Pouldu and was also knowledgeable in literary and philosophical matters that fascinated Gauguin. Their association resulted in some of Gauguin's most remarkable works, including the Wadsworth Atheneum's symbolist portrait of de Haan inscribed "Nirvana." This and the rich variety of paintings and sculpture by Gauguin produced in 1889-90 are the focus of this beautiful book. Gauguin and de Haan settled into an inn at Le Pouldu run by an attractive unwed mother named Marie Henry, who began a liaison with de Haan despite the fact that he was a sickly hunchback. The intensity of relations between Gauguin and de Haan is reflected in many of the works, including frescoes, which they installed in the inn. Gauguin's time in Le Pouldu was crucial to the advancement of his art, and the vivid Breton subjects and personality of Meyer de Haan remained in his imagination to reappear even during his later Tahitian period. In this book several distinguished experts draw on previously unavailable sources to examine in depth the history of this period, Gauguin's relationship with de Haan, their interest in religion and exotic cultures, and the meaning of the many innovative symbolist works they produced.
From Omaha to Oakland, from Coffeyville to Contra Costa County, two immigrant families act out the American dream. This true story of two Midwestern families begins in England and Prussia, and hopscotches across the U.S. to chronicle the lives of the Foltzes and Pickerings. Settled in Omaha and small-town Kansas respectively, they seek advancement – through jobs, romance, higher education, lower humidity. And advancement, for most, leads west. Their narrative is the story of America writ small, in sharply rendered profi les: of Clarence Foltz, Omaha physician and thrower of dinner-table bones; Ruth, his wife, diffi dent but weary of hot summers and capable of hurling dishes when the subject is his philandering; their four daughters, who form a string quartet and build their own couture. Th e youngest becomes a stewardess in the pioneer days of commercial aviation. The Pickerings have their own quirks. Th omas, scion of a prosperous English farming family, chucks it all and joins the 1851 gold rush in Australia. His brother John, a newly minted lawyer, goes to Kansas to be a farmer. Divorce scatters Thomas’s family; a son invests (unwisely) in California orange groves. The two clans connect in Depression-era Seattle when George Pickering meets Ethel Foltz. Th ey marry, and World War II prods an ingathering of the family to California, where Clarence and Ruth now reside, escaping Omaha’s weather. Some serve in the Pacifi c Th eater. In postwar Oakland Ethel indulges her love of music, studies voice and makes prominent friends. As the years begin to claim family members, some branches will die out. If there is a moral, it is the importance and comfort of memory.

The Western Front has become, once again, and after 100 years, an important and increasingly popular tourist destination. The Centenary is already encouraging large numbers of visitors to engage with this highly poignant landscape of war and to commemorate the sacrifice and loss of a previous generation. Interest is also being sharpened in the Ôplaces of warÕ as battle-sites, trench-systems, bunkers and mine craters gain a clearer identity as war heritage. For the first time this book brings together the three strands of heritage, landscape and tourism to provide a fresh understanding of the multi-layered nature of the Western Front. The book approaches the area as a rich dynamic landscape which can be viewed in a startling variety of ways: historically, materially, culturally, and perceptually. To illustrate these two dominant interpretations of the regionÕs landscape Ð commemorative and heritage Ð are highlighted and their relationship to tourism explored. Tourism is a lens through which these layers can be peeled away, and each understood and interacted with according to the individualÕs own knowledge, motivation, and degree of emotional engagement. Tourism is not regarded here as a passive phenomenon, but as an active agent that can determine, dictate and inscribe this evocative landscape. The Western Front: Heritage, Landscape and Tourism is a timely addition to our increasing interest in the First World War and the places where it was fought. It will be indispensable to those who seek a deeper understanding of the conflict from previously undervalued perspectives.
