This is the third report of the Shipman Inquiry, set up to investigate the circumstances surrounding the murders of over 200 patients by their GP, Dr. Harold Shipman. It examines the present arrangements for death registration, cremation certification and coroners' investigations in England and Wales; and sets out recommendations for changes to protect patients from the concealment of homicide in the future, as well as to establish a sound system for promoting medical knowledge and aiding NHS resource planning. 48 recommendations are made including: the need for radical reform of the coronial system, with a new Coroner Service to be established as a executive non-departmental public body (ENDPB), governed by a Board responsible for policy formulation and strategic direction. The aim of the Service should be to provide an independent system of death investigation and certification accessible to the public, with procedures designed to provide a through and open investigation of suspicious deaths and to detect cases of homicide, medical error and neglect. Posts of medical and judicial coroners should be established as independent office-holders under the Crown, and trained investigators given an enhanced role in place of the coroner's officer. The Service should be organised through a regional and district structure, based on the ten administrative regions in England and Wales. The death certification scheme should require the completion of two forms, with a statutory duty imposed upon the consultant doctor in cases of a death occurring in hospital or on the GP in case of a death occurring other than in hospital. There should also be a statutory duty on any 'qualified' or 'responsible' person to report any concerns relating to a death believed to amount to evidence of crime, malpractice or neglect.
Ted Shippy Shipman was one of The Few who flew with 41 Squadron in the Battle of Britain. He left his father's farm in 1930 and enlisted in the RAF as a driver ACII. He flew for thirteen years of his thirty years service, achieved the highest grade of flying instructor and retired as a Wing Commander. This book is based on the copious notes that Shippy wrote in the 1970s and bring a firsthand insight into the life of an RAF Spitfire pilot during the early war years and then his remaining wartime and postwar service until 1959. His career as a senior instructor included No 8 Service Flying Training School, Montrose and the Central Flying School at Upavon. He then went on to teach at the Flying Instructors School at Hullavington in 1942 and the Rhodesian Air Training Group between 1943 and 1945. After the war he did tours in Germany and Cyprus. He was Commanding Officer at RAF Sopley, Hampshire and RAF Boulmer in Northumberland until his retirement in 1959. During retirement he actively supported the Air Training Corps, Battle of Britain Fighter Association and the RAF Benevolent Fund.

Originally published in 1983, this non-statistical guide aims to help teachers organise their work so as to improve their assessment of their pupils and also to analyse their own management efforts. A model is developed showing how teachers can progressively structure their work to increase the objectivity of their assessment. The use of published tests and the production of tests by teachers is covered, but the focus throughout is on the part played by assessment in the organisation of learning. The steps recommended are arranged so that the first yield the largest return. But none requires any statistical expertise or a large investment of time. The second part of the book shows how schools can be evaluated by their staff – school organisation, staff development and the curriculum are given special attention in this step-by-step guide to effective and rewarding school assessment.
People with diabetes mellitus experience health-literacy and self-management problems from the first days of diagnosis. As the tedium of detail and the continuous responsibility for well-being are realized, the self-efficacy of the individual is challenged (Polonsky, 1999). In addition, an exhaustion of physical, mental, and emotional strength may eventually impair motivation to self-manage this chronic illness. Stress often results from the continuing challenges required for successful management of the disease (Sperry, 2008). Management decisions are stressful as the diabetic strives to balance medication, diet, and exercise in order to reduce co-morbidities, complications, and end-stage progression. Because of the stresses involved, many diabetics suffer psychologically for years before experiencing the physical complications of the disease. More people die yearly from diabetes than from breast cancer and AIDS together (ADA, 2010). Diabetes management products, and their illusion of hope for living with diabetes, are not matched with the challenge of integrating health-literacy and self-management skills (Polonsky, 1999).
