Frequency spectrum is a limited and valuable resource for wireless communications. A good example can be observed among network operators in Europe for the prices to pay for UMTS-frequency bands. Therefore, the first goal when designing future wireless communication systems (e.g. 4G - fourth generation) has to be the increase in spectral efficiency. The development in digital communications in the past years has enabled efficient modulation and coding techniques for robust and spectral efficient data, speech, audio and video transmission. These are the multi-carrier modulation (e.g. OFDM) and the spread spectrum technique (e.g. DS-CDMA), where OFDM was chosen for broadcast applications (DVB, DAB) as well as for broadband wireless indoor standards (ETSI HIPERLAN-II, IEEE-802.11) and the DS-CDMA was selected in mobile communications (IS-95, third generation mobile radio systems world wide, UMTS/IMT 2000). Since 1993 various combinations of multi-carrier (MC) modulation and the spread spectrum (SS) technique have been introduced and the field of MC-SS communications has become an independent and important research topic with increasing activities. New application fields have been proposed such as high rate cellular mobile, high rate wireless indoor and LMDS. It has been shown that MC-SS offers the high spectral efficiency, robustness and flexibility that is required for the next generation systems. Meanwhile, different alternative hybrid schemes such as OFDM/OFDMA, MC-TDMA, etc. have been deeply analysed and adopted in different international standards (ETSI-BRAN, IEEE-802 & MMAC). Paul's reading of the Old Testament continues to witness to the significance of reading the Old Testament in a Christian way. This study argues that a theological approach to understanding Paul's appeal to and reading of the Old Testament, especially Isaiah, offers important insights into the ways in which Christians should read the Old Testament and a two-testament canon today. By way of example, this study explores the ways in which Isaiah 40-66's canonical form presents the gospel in miniature with its movement from Israel to Servant to servants. It is subsequently argued that Paul follows this literary movement in his own theological reflection in 2 Corinthians 5:14-6:10. Jesus takes on the unique role and identity of the Servant of Isaiah 40-55, and Paul takes on the role of the servants of the Servant in Isaiah 53-66. From this exegetical exploration conclusions are drawn in the final chapter that seek to apply a term from the history of interpretation to Paul's reading, that is, the plain sense of Scripture. What does an appeal to plain sense broker? And does Paul's reading of the Old Testament look anything like a plain sense reading? Gignilliat concludes that Paul is reading the Old Testament in such a way that the literal sense and its figural potential and capacity are not divorced but are actually organically linked in what can be termed a plain sense reading.
Complex disorders of the carbohydrate metabolism and associated complications cause many abnormalities detectable by radiography in the bones and joints. Mild clinical symptoms associated with very severe radiological changes were first rec ognized in relation to the gastroenterologic complications of diabetes. This phenomenon is more frequent in the skeletal system. For example, mild and painless swelling of the foot joints may often mask extremely severe bone destruction. Several other bone changes associated with diabetes are only detectable by radiography. Thus, the radiologist plays an important role in confirming these diabetic complications, furthermore he is involved in the therapeutic management of the patient. Although many details on this subject have been published, however no summarizing monograph has yet appeared. Manuals discussing diabetes include only short reviews on complications of the osseous system. The fact that the incidence of diabetes is very high, at present 1 %-2 % of the population is affected and their number is gradually increasing - dis plays the timeliness of this subject. Fifty years of experience with insulin therapy indicates that several important problems still remain to be solved. Insulin and modern oral antidia betic drugs proved extremely efficient in the management of hyperglycemia and ketosis, but the incidence of other complications has not decreased. Moreover, as the number of diabetics and their life expectancy increase, late complications become likewise more fre quent. Diabetic osteoarthropathy is one of these complications.

Chronic renal disease has received increasing attention and concern since the passage in 1972 of PL 92-603, which provided coverage for end-stage renal disease (ESRD) treatment by the federal government. The human and economic costs of the ESRD program serve to emphasize the need to prevent or to arrest those diseases resulting in chronic renal failure, since none of the available treatments is without complications and/or side effects. The ESRD program, the only federal one that provides coverage for a catastrophic illness for almost the entire population (those qualifying under Social Security), cost almost $2 billion in 1983. The escalating costs of the ESRD program are attributed to the increasing number of patients requiring treatment and have focused concerns of the United States Government, both Congress and the administration, on ESRD. The National Institutes of Health (NIH), especially the Kidney, Urology, and Hematology Division of the National Institutes of Arthritis, Diabetes, and Digestive and Kidney Diseases (NIADDK), supports a sizable research program that bears on chronic renal disease and in association with this has sponsored many conferences and workshops on research on and causes and complications of chronic renal failure. This book is an outgrowth of the issues addressed by participants at a number of NIH conferences held in the 1980s.

During the 1970s ultrasonography developed along different paths in continental Eu rope and the English-speaking world. Whereas static imaging dominated the United Kingdom and North America, real-time examination performed by physicians was the norm in the Federal Republic of Germany and France. Francis Weill was uniquely able to bridge the gap between the two schools since he employed both techniques extensively. He made many important contributions to ab dominal ultrasonography during that decade, including the first descriptions of peri pancreatic vascular anatomy, the sonographic diagnostic features of obstructive jaun dice, and the localization of intraperitoneal fluid collections. During the past 20 years he has been working on an evolving textbook of gastroin testinal ultrasonography. This edition in English is the latest fruit of that effort. There are many textbooks of ultrasonography but very few are the work of a single author with a systematic, consistent, and thoroughly organized approach. Moreover, a work that has been crafted and refined by 20 years of clinical experience is certainly unique in this field. My relationship to this book began with the first English edition which I helped to translate from French.
