Specialization has driven medical practice over the last century. Expanding knowledge has increased the physician workforce but has decreased the proportion of general family doctors, leaving rural areas with critical physician shortages. In 1978 almost half of family physicians delivered babies. By 2005 this number decreased to 23 percent. Although many recognize the broad medical and surgical skills required in remote Africa, few recognize the need for similar skills among doctors in rural America. These physicians blend family and emergency medicine with specific skills from obstetrics, gynecology, and general surgery. Many family doctors provide extensive women’s health services, including cancer screening, prenatal care, and cesarean delivery. This work is written for these physicians. In rural areas, most women cannot leave their communities to have children. Extensive travel for prenatal care and delivery has detrimental economic and social consequences. Although rural counties provide quality care, they cannot afford an array of technicians, nurses, and physicians trained in obstetrics, gynecology, surgery, neonatology, and critical care. Drawing from my mentors’ and family’s experiences in Texas and Tennessee, I realized that there was no formal ultrasound curriculum for family doctors providing rural obstetrical care. The majority of texts have been written for ultrasound technicians performing examinations and subspecialists interpreting the acquired images. Historically, family physicians have played a prominent role in obstetrical patient safety and care, as seen through the continued success of the Advanced Life Support in Obstetrics (ALSO) curriculum. Although the ALSO course provides valuable training, I argue that a formal program for physician performed obstetrical ultrasound is equally, if not more, important. Ultrasound is the basis for obstetrical risk stratification. Without basic skills, physicians may inadvertently put mothers and children at risk. For example, a physician in west Texas should not trend beta hCG levels over two days when evaluating a suspected ectopic pregnancy. Any suspected threat to pregnancy should be investigated immediately at the point of care to avoid unnecessary delays that may lead to maternal or fetal death. This text serves as a rational but not exhaustive introduction to obstetrical ultrasound for the physician providing women’s health services in remote or resource-poor settings. This text distills volumes of information found in advanced obstetrical textbooks as well as recent American and European literature. The referenced articles come from a variety of journals and represent many specialties. Ultimately, we desire to promote patient safety and improve care and welcome any suggestions or ideas that help achieve this goal. This book treats Balthasar's engagement with Protestantism, primarily in the persons of Martin Luther and Karl Barth, a topic which has not yet been given the attention it deserves. Furthermore, instead of focusing on particular theological issues, such as soteriology or ecclesiology, the book examines the implications of this engagement for Fundamental Theology. At the very root of Luther's confrontation with the Catholic Church of the Late Middle Ages, lies his antipathy for Aristotle and for "natural theology." In other words, the Protestant difference has as much to do with its suspicion of the treatment of faith and reason in Catholic thought as it does of the Catholic treatment of faith and works. This is a suspicion that is only exacerbated in Barth's identification of the "analogy of being" with the Antichrist. Balthasar takes these criticisms very seriously, and, in addressing them, not only has much of relevance to say to the Catholic-Protestant differences, but also has much to say to the Yale-Chicago differences. In short, this study treats primarily Balthasar's dialogue with Luther and Barth, with the hope that this dialogue will shed light on the impasse that seems to have arisen between the so-called "correlation" and "revelocentric" schools of contemporary theology. If, indeed, Christ is the "concrete universal," then we shouldn't have to decide between the two. Part of this proposal, then, is to emphasize the fact that Balthasar refuses to separate Fundamental and Dogmatic theology.

From 1913 through 1918, Long Beach, California, was home to the largest independent film company in the world, the largely forgotten Balboa Studio. Founder Herbert M. Horkheimer bought the studio from Edison Company in 1913, and by 1915 Balboa's expenses exceeded $2,500 a day and its output hit 15,500 feet of film per week. Bert Bracken, Fatty Arbuckle, Henry King, Baby Marie Osborne, Thomas Ince, and William Desmond Taylor began their careers with the studio. In 1918, Horkheimer stunned the industry by declaring bankruptcy, shutting down Balboa, and walking away from moviemaking. The closing of the studio effectively ended Long Beach's runs as a major film location and left many wondering about the true reasons behind Horkheimer's decision. Most of Balboa's films have been lost, and little has until now been written about the studio. This book first explores the history of filmmaking in Long Beach and then fully details the story of Balboa. The extensive filmography includes length, copyright date when available, cast and credits, and a plot summary.

However, greater civic association and a general sense of community, embodied in the concept of social capital, are said to have tremendous beneficial effects and profoundly influence American society. This 2007 study juxtaposes and critically assesses two bodies of research that have reached different conclusions on these issues. Is America's legacy of racial inequality an 'evil twin' of the benefits of social capital? By analysing the social outcomes for racial minorities, in addition to other dimensions of American politics, the author shows that the impact of racial diversity consistently outweighs that of social capital.

These ideas started making themselves at home in my brain 30 or 40 years ago. But the ideas' arrivals have been much more detailed and frequent since I discovered the science website vixra.org at the start of 2012. I'm not a professional of any kind - just your average citizen, with a burning desire to understand how everything works (my main interest is cosmology).* I can't say I've been originating the ideas presented here. My feeling as I typed these thoughts has always been that they already exist (though, since physics' Unification appears to connect every point and time in the Cosmos, not on early 21st-century Earth in every case). And I'm just a student, learning about them. * It doesn't matter that I'm not a professional scientist. Keep the following words said in 2015 by scientist Professor Neil Turok in mind - "We need a very different view of basic physics. This is the time for radical, new ideas." He believes this is a great time in human history for the revolution to occur.

The abolishment of flogging in 1850 started the U.S. Navy on a quest for a prison system that culminated with the opening of Portsmouth Naval Prison in 1908. During World War I, that prison became the center of the Navy’s attempt to reform what many considered outdated means of punishment. Driven by Progressive Era ideals and led by Thomas Mott Osborne, cell doors remained opened, inmates governed themselves, and thousands of rehabilitated prisoners were returned to the fleet. Championed by Secretary of the Navy Josephus Daniels and Assistant Secretary of the Navy Franklin Roosevelt, Osborne’s reforms proceeded positively until Vice Adm. William. Sims and others became convinced that too many troublemakers were being returned to the fleet. In response, FDR led an on-site investigation of conditions at Portsmouth prison, which included charges of gross mismanagement and rampant homosexual activity. Although exonerated, Osborne resigned and initiatives were quickly reversed as the Navy returned to a harsher system.

This first volume of the Official History of the UK Civil Service covers its evolution from the Northcote-Trevelyan Report of 1854 to the first years of Mrs Thatcher’s government in 1981. Despite current concerns with good governance and policy delivery, little serious attention has been paid to the institution vital to both: the Civil Service. This Official History is designed to remedy this by placing present problems in historical context and by providing a helpful structure in which others, and particularly former officials, may contribute to the debate. Starting with the seminal 1854 Northcote-Trevelyan Report, it covers the ‘lost opportunity’ of the 1940s when the Service failed to adapt the needs of ‘big government’ as advocated by Beveridge and Keynes. It then examines, in greater detail, the belated attempts at modernisation in the 1960s, the Service’s vilification in the 1970s and the final destruction of the ‘old order’ during the first years of Mrs Thatcher’s government. Particular light is shed on the origins of such current concerns as the role of special advisers the need for a Prime Minister’s Department the evolution of Parliamentary Select Committees to resolve the potential tension between bureaucracy and Parliamentary democracy.
