One of the main causes of failure in the treatment of cancer is the development of drug resistance by the cancer cells. The design of cancer chemotherapy has become increasingly sophisticated, yet there is no cancer treatment that is 100 percent effective against disseminated cancer. Resistance to treatment with anticancer drugs results from a variety of factors including individual variations in patients and somatic cell genetic differences in tumours, even those from the same tissue of origin. Frequently resistance is intrinsic to the cancer, but as therapy becomes more and more effective, acquired resistance has also become common.The most common reason for acquisition of resistance to a broad range of anticancer drugs is expression of one or more energy-dependent transporters that detect and eject anti-cancer drugs from cells, but other mechanisms of resistance including insensitivity to drug-induced apoptosis and induction of drug-detoxifying mechanisms probably play an important role in acquired anticancer drug resistance. Studies on mechanisms of cancer drug resistance have yielded important information on how to circumvent this resistance to improve cancer chemotherapy and have implications for pharmacokinetics of many commonly used drugs.

What is the church? What is its mission in the world? Modern Protestantism's inability to provide a clear answer to these seemingly simple questions has resulted in vast confusion amongst pastors about the nature of their calling and has left congregations languishing without a clear reason for existence. Many of the voices and allegiances competing for the churches' attention have rushed in to fill the void, with the result that the church in modernity has frequently found itself captive to the prevailing culture. Yet from within the belly of highly culturally accommodated churches, both the German pastor-theologian Dietrich Bonhoeffer and the American theological ethicist Stanley Hauerwas were able to articulate compelling visions of churches freed from their cultural captivity in order to truly and freely serve God and neighbor. Against the complex and confusing backdrops of Nazi Germany and late twentieth- and early twenty-first-century America respectively, Bonhoeffer and Hauerwas sought to recover the ethical and political character of the Christian faith through recalling the church back to the christological center of its faith. Together they provide a rich set of complementary, and at times mutually correcting, resources for the contemporary church as it seeks to faithfully bear witness to Christ amidst the ruins of Christendom.

When realizing a programming language as VM, implementing behavior as part of the VM, as primitive, usually results in reduced execution times. But supporting and developing primitive functions requires more effort than maintaining and using code in the hosted language since debugging is harder, and the turn-around times for VM parts are higher. Furthermore, source artifacts of primitive functions are seldom reused in new implementations of the same language. And if they are reused, the existing API usually is emulated, reducing the performance gains. Because of recent results in tracing dynamic compilation, the trade-off between performance and ease of implementation, reuse, and changeability might now be decided adversely. In this work, we investigate the trade-offs when creating primitives, and in particular how large a difference remains between primitive and hosted function run times in VMs with tracing just-in-time compiler. To that end, we implemented the algorithmic primitive BitBlt three times for RSqueak/VM. RSqueak/VM is a Smalltalk VM utilizing the PyPy RPython toolchain. We compare primitive implementations in C, RPython, and Smalltalk, showing that due to the tracing just-in-time compiler, the performance gap has lessened by one magnitude to one magnitude.

This textbook was finalized during the worldwide COVID-19 pandemic. For years prior to the pandemic, clinical trials have shown improvement and cures of pulmonary disorders with the intravenous administration of mesenchymal stem cells. These cells are administered into the venous system, pass through the heart, and then lodge in the capillary network of the lungs where they decrease scar tissue and stimulate the regeneration of new lung tissue. The known benefits of cell therapy for pulmonary disease has been the foundation for the use of allogeneic stem cells to effectively treat (and in some cases cure) COVID-19-related lung disorders. Sadly, many of the highly qualified health care professionals caring for these critically ill patients are unfamiliar with the concept of stem cells and regenerative medicine. This unfamiliarity has resulted in patients throughout the world not receiving the benefit of these potentially life-saving treatments. The goal of this textbook is to provide a basic scientific and clinical multispecialty reference source for stem cells and regenerative medicine to be used as an extension of the American College of Regenerative Medicine.™ Chapters focus on basic science as well as the extravascular applications of regenerative medicine for all hard and soft tissues of the body including musculoskeletal and orthopedics, dental and maxillofacial surgery, and dermatology and plastic surgery.
