Physician, surgeon, natural historian, educator, Protestant evangelical. Andrew Fernando Holmes's name is synonymous with the McGill medical faculty and with the discovery of a congenital heart malformation known as the "Holmes heart." He also played a critical role in the creation of a scientific culture in early-nineteenth-century Montreal. Born in captivity at Cadiz, Spain, Holmes immigrated to Lower Canada in the first decade of the nineteenth century. He arrived in a province that was experiencing profound social, economic, and cultural change as the result of a long process of integration into the British Atlantic world. A transatlantic perspective, therefore, undergirds this biography, from an exploration of how Holmes's family members were participants in an Atlantic world of trade and consumption, to explaining how his educational experiences at Edinburgh and Paris informed his approach to the practice of medicine, medical education, and medical politics. This fascinating biography also examines Holmes's deepest religious convictions, positioning them at the centre of his work and life.

The author of Ephesians shows an interest in the Isaianic new exodus (INE) in facilitating his focus on the present triumph of the people of God. The recipients required both insight and confidence, especially in regard to the certainty of their salvation, its mysterious nature, Jewish-Gentile relations, and their former manner of life. In the face of these challenges the author maintains that victory is secured through God's choice of them and the working of his redemptive program--which inevitably leads to triumphal blessings. As with the exodus-era Hebrews, the readers participate in paschal redemption, now embodied in Christ, along with his resurrection and ascension as the triumphant new exodus warrior over principalities and powers. In addition, and as predicted by OT writers, Gentile proselytes share the blessings of the new exodus, but now on equal footing and access as Jewish Christians--creating a new eschatological temple. Triumphant Christ distributes gifts, facilitates Spirit-endowed living, and enables Christian warfare that mirrors Yahweh and his servant. The recipients of the epistle appear to be called to view INE triumph as above, below, now and not yet, while not discarding current applications on earth.

Biblical scholars have often contrasted the exegesis of the early church fathers from the eastern region and “school” of Syrian Antioch against that of the school of Alexandria. The Antiochenes have often been described as strictly historical-literal exegetes in contrast to the allegorical exegesis of the Alexandrians. Patristic scholars now challenge those stereotypes, some even arguing that few differences existed between the two groups. This work agrees that both schools were concerned with a literal and spiritual reading. But, it also tries to show, through analysis of Theodore and Theodoret’s exegesis and use of the term theoria, that how they integrated the literal-theological readings often remained quite distinct from the Alexandrians. For the Antiochenes, the term theoria did not mean allegory, but instead stood for a range of perceptions—prophetic, christological, and contemporary. It is in these insights that we find the deep wisdom to help modern readers interpret Scripture theologically.

In the late 1980s, a promising new treatment for breast cancer emerged: high-dose chemotherapy with autologous bone marrow transplantation or HDC/ABMT. It details how the factors that drove clinical use--patient demand, physician enthusiasm, media reporting, litigation, economic exploitation, and legislative and administrative mandates--converged to propel the procedure forward despite a lack of proven clinical effectiveness. It also analyzes the limited effect of technology assessments before randomized clinical trials evaluated decisively the procedure and the ramifications of this system on healthcare today. Sections of the book consider the initial conditions surrounding the emergence of the new breast cancer treatment, the drivers of clinical use, and the struggle for evidence-based medicine. A concluding section considers the significance of the story for our healthcare system.

We see tobacco spread rapidly from its aboriginal sources in the New World 500 years ago, as it becomes increasingly viewed by some as sinful and some as alluring, and by government as a windfall source of tax revenue. With the arrival of the cigarette in the late-nineteenth century, smoking changes from a luxury and occasional pastime to an everyday—to some, indispensable—habit, aided markedly by the exuberance of the tobacco huskers. This free-enterprise success saga grows shadowed, from the middle of this century, as science begins to understand the cigarette's toxicity. Ironically the more detailed and persuasive the findings by medical investigators, the more cigarette makers prosper by seeming to modify their product with filters and reduced dosages of tar and nicotine. We see the tobacco manufacturers come under intensifying assault as a rogue industry for knowingly and callously plying their hazardous wares while insisting that the health charges against them (a) remain unproven, and (b) are universally understood, so smokers indulge at their own risk. Among the eye-opening disclosures here: outrageous pseudo-scientific claims made for cigarettes throughout the '30s and '40s, and the story of how the tobacco industry and the National Cancer Institute spent millions to develop a "safer" cigarette that was never brought to market.
