After considering the aging population in developed countries, it has become clear to physicians and public policy administrators that prevention of cancer must play a more important role in national anti-cancer policy than it has in the past. The recent introduction of an HPV vaccine, coupled with discoveries concerning the relationship of H. pylori and cancer has brought the role of infectious agents in cancer into sharp focus in the medical community. While interest in the subject has grown, no single source existed to bring clinicians up-to-date on developments in disease mechanisms, population-based risk assessment and policy considerations in the field of cancer prevention. In this current and comprehensive text the authors review the basic science and clinical implications of individual infectious agents, while going beyond a mere update of the literature to offer insights on the current emerging prevention possibilities. This prevention perspective is what makes this particular text so valuable to researchers, epidemiologists, health care policy makers and oncologists. The discussion is organized to highlight the vital role of primary cancer prevention, and suggest directions for future research, practice and policy. Since HPV continues to be at the center of interest in the arena of infectious agents and cancer, the authors frame the majority of their discussion on this now-famous virus. The sheer volume of literature related to this virus and its many related cancers, and the burgeoning research on the development and implementation of a prophylactic vaccine necessitates a much fuller review of this infectious agent. Therefore, the book is roughly divided into two equal parts: one part devoted to HPV and another part devoted to five other prominent infectious agents in cancer.

Religious activity flourished in the eighteenth-century navy; this book examines the reasons why and its manifestations. The Evangelical Admiral Gambier, notorious for distributing tracts to his fleet in a theatre of war, is commonly seen as a misfit in a fighting service that had scant time for fervent piety. In fact, the navy of the Revolutionaryand Napoleonic Wars showed a level of religious observance not seen since the days of Queen Anne. Evangelical laymen provided one dynamic for this change: concentrating first on public worship, they moved to active proselytism insearch of converts amongst sailors, and in a third phase developed a loose network of prayer groups in scores of ships, uniting officers and seamen in voluntary gatherings that transcended rank. This book explores the effect this new piety had on discipline and human governance, on literacy, on the development of chaplains' ministry and on the mindset of the officer corps. Between June 1940 and August 1943, RAF Bomber Command undertook a little-known strategic bombing campaign in Europe. The target was Mussolini's Italy. This air campaign was a key part of the strategic policy of Britain from 1940 to 1943, which aimed at securing Italy's early surrender. However, it posed unique challenges, not least of which was Italy's natural defences of distance and the Alps. The bombing campaign against Italy can be divided into a number of phases, with each one having its own specific goals such as affecting Italian war production or hindering the Italian Navy's war in the Mediterranean. However, each also furthered the ultimate aim of forcing Italy's final capitulation, demonstrating that the tactic of area-bombing was not just about the destruction of an enemy's cities, as it could also fulfil wider strategic and political objectives. Indeed, the intensity and frequency of attack was greatly controlled, and the heavy bombing of Italy was only ever sanctioned by Britain's civilian war leaders to achieve both military and political goals. The issue of target-selection was also subject to a similar political restriction; cities and ports like Milan, Turin, Genoa and La Spezia were sanctioned under an official Directive, but other places, such as Verona, Venice, Florence and, above all, Rome, remained off-limits. Sam Elworthys career was remarkable by any standards. Born in New Zealand in 1911 and educated in England, he was called to the Bar. After learning to learning to fly he joined the RAAF. During the Second World War he won the DFC, DSO and AFC and, after commanding 82 Squadron, worked closely with Bomber Harris and General Eisenhower. He became an air commodore aged 33.His meteoric rise continued post-war. Switching to Fighter Command he saw service in India, Pakistan, and the UK before becoming Commandant of the RAF Staff College. By 1960 he was tri-service C-in-C Middle East and his actions prevented the invasion of Kuwait by Iraq.As Chief of Air Staff and Chief of defense Staff in the 1960s he fought the Services corner at a difficult political and economic time. He secured the long term future of the RAF, whose very existence was threatened. A hugely respected figure, he became a life peer, Knight of the Garter and Constable of Windsor Castle. He died in 1993 in his native New Zealand.This long overdue biography attempts successfully to do justice to a man of great stature, integrity and achievement.

On the eastern coast of Brazil, facing westward across a wide magnificent bay, lies Salvador, a major city in the Americas at the end of the eighteenth century. Those who distributed and sold food, from the poorest street vendors to the most prosperous traders—black and white, male and female, slave and free, Brazilian, Portuguese, and African—were connected in tangled ways to each other and to practically everyone else in the city, and are the subjects of this book. Food traders formed the city's most dynamic social component during the late eighteenth and early nineteenth centuries, constantly negotiating their social place. The boatmen who brought food to the city from across the bay decisively influenced the outcome of the war for Brazilian independence from Portugal by supplying the insurgents and not the colonial army. Richard Graham here shows for the first time that, far from being a city sharply and principally divided into two groups—the rich and powerful or the hapless poor or enslaved—Salvador had a population that included a great many who lived in between and moved up and down. The day-to-day behavior of those engaged in food marketing leads to questions about the government's role in regulating the economy and thus to notions of justice and equity, questions that directly affected both food traders and the wider consuming public. Their voices significantly shaped the debate still going on between those who support economic liberalization and those who resist it.

An in-depth overview of the role of the Royal Air Force's leaders during World War II. The RAF did not come of age until the Second World War. The role of its forerunners in the Great War, the Royal Flying Corps and the Royal Naval Air Service, although important, was peripheral to that of the ground forces. The founding father of the RAF, Lord Trenchard, was determined that it should become a fully-fledged third service, equal in status to the Royal Navy and British Army, and this he succeeded in doing between the wars, firstly by setting up the RAF College at Cranwell, and Staff College at Andover, and secondly by providing a cost-effective policeman of the more rebellious parts of the British Empire. By 1939 the RAF had grown substantially, but, of the three best aircraft of the coming War, only the Supermarine Spitfire was in service, as neither the Avro Lancaster nor the De Havilland Mosquito would be available until early 1942. Aircraft, however, were not enough. It was the leaders of the RAF, the subjects of this book, who would take the battle to the enemy and who, after six long years, would prevail.
