A myth from the colonial period was that Americans could defend themselves by keeping a rifle in the closet and when needed, grab it, and march off to battle in times of crisis. Unfortunately, providing national defense is more complicated that that; indeed it was more complicated even during the Revolutionary war. General George Washington’s struggles to form a standing army supported by workable logistics and supply processes and to get funding for both from the Revolutionary Congress are well documented. Financing national defense requires planning and resourcing in advance. Reacting at the instant of crisis is too late. Building an educated, highly trained and capable Armed Forces and the acquisition of defense weapons and weapons systems has long lead times and involves making decisions the consequences of which are likely to last for decades. These decisions include how to recruit and retain military and civilian personnel as well as designing, buying and fielding a vast array of ground weapons, ships, aircraft and other weaponry. A decision to buy a major defense weapons system for example sets in motion a chain of other decisions that will affect the U.S., its allies and enemies around the world. Implementation of such decisions is financed through the U.S. federal government and Department of Defense budget processes in a planned yet highly and pluralistic and disaggregated system for determining how to advocate, acquire and allocate scarce resources in a manner that culminates in congressional and presidential approval. In this book we examine the concepts and practices of defense financing, provide a detailed description and analysis of resource policy decision making, financial management and budget execution processes, and analyze the most significant features of the national defense and U.S. federal government resource decision and management system. The book assesses the numerous factors, including those that characterize the complex budget review and appropriation decision making dynamics of Congress, that make U.S. defense finance and budgeting different from any other system in the world. In addition, in a concluding chapter the book compares U.S. defense policy and budgeting to other nations in different regions of the globe, drawing conclusions about the effects of U.S. defense policy and defense financing abroad in regions including Europe, Russia, the Middle-East and Asia.

Over several decades, many U.S. states abandoned the practice of selecting their judges by direct popular election and adopted the Missouri Plan of judicial selection. In From Ballot to Bench, Philip L. Dubois subjects the various criticisms raised against judicial elections to a more searching scrutiny than previously has been attempted. Dubois carefully reviews the three central counts on which judicial elections have been faulted: for lowering the quality of the bench, for impairing judicial independence, and for failing to secure judicial accountability. After concluding that the potential for judicial elections to hold judges popularly accountable is what might commend them over alternative selection methods, Dubois concentrates on the analysis of empirical evidence to evaluate judicial elections as mechanisms of accountability. The study examines all the statewide partisan and nonpartisan elections for state supreme court justices in non-southern states from 1948 to 1974. Included is a detailed examination of voter participation, electoral competition, the behavior of judicial electorates, and the patterns of gubernatorial vacancy appointments. An analysis of decision making on eight state supreme courts also tests the relationship between different selection systems and judicial behavior. Dubois finds that partisan elections maximize voter participation, meaningfully structure voter choices, minimize accession to the bench by appointment, and allow popular control over gubernatorial appointments. Additional evidence on the extent of partisan voting by judges selected under different methods leads Dubois to conclude that partisan elections are superior to both nonpartisan elections and nonelective selection methods as instruments of accountability. The importance of the questions addressed, the breadth of the data collected, and the unorthodox conclusions offered make this a significant book for political scientists, judges, lawyers, and public officials.

Pain assessment has remained largely unchanged for decades and is currently based on self-reporting. Although there are different versions, these self-reports all have significant drawbacks. For example, they are based solely on the individual’s assessment and are therefore influenced by personal experience and highly subjective, leading to uncertainty in ratings and difficulty in comparability. Thus, medicine could benefit from an automated, continuous and objective measure of pain. One solution is to use automated pain recognition in the form of machine learning. The aim is to train learning algorithms on sensory data so that they can later provide a pain rating. This thesis summarises several approaches to improve the current state of pain recognition systems based on physiological sensor data. First, a novel pain database is introduced that evaluates the use of subjective and objective pain labels in addition to wearable sensor data for the given task. Furthermore, different feature engineering and feature learning approaches are compared using a fair framework to identify the best methods. Finally, different techniques to increase the interpretability of the models are presented. The results show that classical hand-crafted features can compete with and outperform deep neural networks. Furthermore, the underlying features are easily retrieved from electrodermal activity for automated pain recognition, where pain is often associated with an increase in skin conductance.

If national health insurance becomes a reality, what options should be considered for the coverage of prescription drugs? The authors—whose Pills, Profits, and Politics has had a dramatic effect on physicians, pharmacists, patients, and the drug industry as well as on federal and state legislators—insist that the major objective must be the best possible health care. But holding down costs to patients and taxpayers must also be a goal. To complicate matters further, the advantage of each likely option—including price controls, the use of formularies, drug utilization review, patient cost-sharing, and the use of low-cost, generic-name products—is offset by a disadvantage, even a danger. If drug prices are slashed too much, the industry will lose many of its incentives to develop better drugs for the future. Particular attention is focused on the so-called drug lag—the lengthy delays in licensing of new drugs, even after they have been used with apparently good results in other countries. Pills and the Public Purse also addresses the seldom-appreciated fact that investing tax dollars in needed drugs may save taxpayers in the long run by minimizing unnecessary physician visits and hospitalization. Pills and the Public Purse challenges Congress and such agencies as the Food and Drug Administration and the Health Care Financing Administration to enact policies that put the interests of the public before those of government, industry, physicians, and pharmacists.
