Public discussion of global healthcare issues is dominated by those who believe that top-down, government-driven interventions are the solution to the myriad health problems suffered by people in less developed countries. This thinking is responsible for a plethora of harmful policies, ranging from a drive towards socialized healthcare systems, to calls for the centralization and semi-nationalization of pharmaceutical research and development, to impractical but grandiose UN-sponsored schemes for tackling HIV/AIDS and malaria.In spite of the abysmal track record of top-down approaches, non-governmental organizations and UN agencies continue to promote them, to the detriment of the private sector, economic development, and human health. The resulting politicization of diseases such as HIV/AIDS has led to a diversion of resources away from more easily treatable diseases that affect more people. Meanwhile, cost-effective and simple interventions such as vaccination are being subordinated to other more politically correct diseases.This centralizing mindset has also resulted in many governments in less developed countries attempting to plan and control universal healthcare systems, which has encouraged rationing, inequitable access, and entrenched corruption. It has also seriously undermined the effectiveness of overseas development aid. Moreover, the politicization of diseases such as HIV/AIDS has led to a diversion of resources away from more easily treatable diseases that affect more people. As a result, cost-effective and simple interventions are neglected by donors.There has to date been little public discussion of the role of markets and their underlying institutions--property rights and the rule of law--in improving human health. Economic growth and globalization has led to unprecedented improvements in human health. The challenge is to enable the poorest countries to take part more fully in this process. Natural infections with Ascaris lumbricoides and A. suum of pigs and humans, respectively, have a worldwide distribution. Ascaris infections induce strong Th2 responses in the host that appear to be central to the development of protective immunity. Th2 responses may be modulated actively through a complex host–parasite interaction that allows the parasite to survive but also limits severe Th2-mediated pathology in the host. The increased production of IL-10 by regulatory cell populations may have a key role in the modulation of the immune response during chronic ascariasis although most evidence in support is derived from experimental infections with A. suum and A. suum antigen extracts. Antigen fractions of A. suum have potent anti-inflammatory effects in experimental models of arthritis and asthma, but other antigen fractions are strong inducers of inflammation. The clinical effects of such immune modulation in humans is uncertain although there is some evidence that chronic infections may be associated with impaired immune responses to vaccines indicating potentially important effects on both mucosal and systemic immunity. Human ascariasis has been associated with an increased risk of asthma and other allergic reactions, observations that preclude the use of experimental infections for the treatment of inflammation in favor of the study of the anti-inflammatory properties of specific molecules derived from Ascaris.

Youth unemployment is one of the most critical problems generated by any recession. Widespread changes in the structure of the youth labour market, together with the increasing affluence amongst the employed, meant that the experience of unemployment for young people in the 1970s and 1980s was quite different from that of previous decades. Originally published in 1988, this book examines the psychological consequence of prolonged periods of joblessness among a national cohort of 16-19 year olds. It places the problem in a historical context and then examines evidence for the effect of unemployment on the work ethic, motivation to work, the search for jobs, psychological health, political views, lifestyles and early careers. Particular attention is paid to gender and ethnic group differences. Original research was integrated with existing literature with the aim of bringing together a wide variety of studies and theoretical positions previously dispersed throughout the literature.
