Socially excluded youth with mental health problems and co-occurring difficulties (e.g. conduct disorder, family breakdown, homelessness, substance use, exploitation, educational failure) attract the involvement of multiple agencies. Poorly coordinated interventions often multiply in the face of such problems, so that a young person or family is approached by multiple workers from different agencies working towards different goals and using different treatment models; these are often overwhelming and may actually be experienced as aversive by the young person or their family. Failure to provide effective help is costly throughout life This is the first book to describe Adaptive Mentalization-Based Integrative Treatment (AMBIT). This is an approach to working with people - particularly young people and young adults - whose lives are often chaotic and risky, and whose problems are not limited to one domain. In addition to mental health problems, they may have problems with care arrangements, education or employment, exploitation, substance misuse, offending behaviours, and gang affiliations; if these problems are all occurring simultaneously, any progress in one area is easily undermined by harms still occurring in another. Peter H. Wolff, a world-renowned authority on infant behavior, helped lay the foundation for the field in the 1960s with his innovative studies of behavioral studies, motor coordination, smiling, and crying in infancy. Some twenty years later, as infancy studies have become increasingly specialized and fragmented, he calls for new theoretical perspectives and methods of investigation. Applying ethological methods used in field studies of animal behavior, Wolff first observes how babies behave in the "natural" ecology of their homes to catalog their species-typical behavioral repertory and then manipulates their behavior through informal experiments designed to examine functional significance. Wolff argues that a coherent psychobiological theory of early human development must begin with knowledge about the infant's behavioral repertory under free field conditions. Many current theories of human development begin instead with assumptions about the organization of behavior derived from studies of psychological function in the adult; moreover, they appeal to instincts, maturational programs, or genomes to explain the apparent lawfulness in the development of these behavioral categories. Such a priori explanations, Wolff contends, beg the whole question of development. As an alternative to theoretical metaphors that portray the infant as a closed system and suggests that development is controlled by prescient programs that anticipate the mature steady state, Wolff proposes a metaphor of the infant as an open, self-organizing system with partial, mutative mechanisms of development. Applying this metaphor, he addresses the essentially unsolved problem of how novel behavioral forms are induced during ontogenesis. Wolff presents a study of twenty-two infants who were observed for thirty hours each week in their homes during the first months after birth. He builds a week-by-week description of changes in behavioral states of wakefulness and examines how reversible state changes influence developmental transformations in social-affective behavior and sensori-motor intelligence. The observations and informal experiments emphasize expressions of emotion and the infant's changing relations to persons and things. Pointing out that movements are our only clue to what infants "feel" or "think," Wolff gives special emphasis to the systematic variations in spontaneous and environmentally evoked patterns of motor coordination as a function of behavioral state transitions. First published in 1974, Attributes of Memory rejected the prevalent stress on the structure of memory. It suggests that the view of memory as a sequence of stores through which information passes is mistaken. Instead, the author emphasizes the coding process of memory by which the nominal stimulus, the stimulus as presented, is transformed into the functional stimulus, the stimulus as coded. Dr Herriot proposes that there are many different forms of coding, and that efficiency of recall or recognition performance is a function of the nature of coding employed. He suggests that the subject’s linguistic system is the most frequently employed linguistic device; that is, that the underlying attributes and rules of language are used automatically when material is verbal. Since the basic function of language is to communicate meaning, those forms of coding which are meaningful in nature are most effective in memory. The book cites a great deal of experimental evidence, including many studies of the time. As well as stating a point of view, it should be useful to undergraduate and postgraduate students as a review of the early literature, read in its historical context.

Neuroimaging is illuminating diverse brain regions that contribute to mentalizing capacity, including a "mentalizing region" in the medial prefrontal cortex that is consistently activated in mentalizing tasks; concomitantly, research on autism and psychopathy attests to the neurobiological basis of psychopathologies in which stable impairments of mentalizing are most conspicuous. In development and in psychotherapy, mentalizing begets mentalizing, as exemplified by a mentalizing stance that fosters inquisitiveness and curiosity about mental states in oneself and others; basic principles and clinical examples, including the use of transference, demonstrate the spirit and technique of mentalizing, capped off by a patient's first-hand account of mentalization-based treatment for borderline personality disorder. Attachment trauma is the wellspring of disrupted mentalizing capacity, and a focus on mentalizing provides an integrative framework for psychodynamic and cognitive-behavioral treatment of trauma as well as for parenting, family, and social-systems interventions directed toward interrupting the perpetuation of trauma in relationships. Psychoeducational interventions, including patient education and structured exercises, are employed to cultivate a therapeutic alliance around mentalizing; the book includes a straightforward explanation clinicians can use with patients, "What is Mentalizing and Why Do It?" In the chapter on mentalizing interventions, the authors propose to clinicians, "You are already doing it." If the effectiveness of treatment depends on therapists mentalizing and helping their patients do so more consistently and skillfully, clinicians of all persuasions can benefit from the extensive knowledge now available to hone further their attention to this vital therapeutic process.

Originally published in 1984, one of the few facts that emerged clearly in the beleaguered field of psychology and mental health at the time was the extent of poor social skills in psychiatric patients, the mentally handicapped and problem adolescents. As a result, during the 1970s, social skills training – espoused as a form of behaviour therapy – seemed to offer great promise, based on the notion that social skills, like any other skills, are learnt and can be taught if lacking. However, in evaluating social skills training, many investigators found that skills did not endure and generalise. This book attempts a major re-assessment of social skills training. It examines the underlying paradigms, which are shown to be fundamentally behaviourist. Such paradigms, it is argued, severely constrain the aims and method of current types of training. Thus the book develops what is termed an ‘agency’ approach, based on man as a social agent who actively constructs his own experiences and generates his own goal-directed behaviour on the basis of those constructs. This new model is developed in both theoretical and practical ways in the main body of the book and should, even today, be of great interest to all those involved with social skills training.

In front of you is the finished product of your work, the text of your contributions to the 2003 Dayton International Symposium on Cell Volume and Signal Transduction. As we all recall, this symposium brought together the Doyens of Cellular and Molecular Physiology as well as aspiring young investigators and students in this field. It became a memorable event in an illustrious series of International Symposia on Cell Volume and Signaling. This series, started by Professors Vladimir Strbák, Florian Lang and Monte Greer in Smolenice, Slovakia in 1997 and continued by Professors Rolf Kinne, Florian Lang and Frank Wehner in Berlin in 2000, is projected for 2005 in Copenhagen to be hosted by our colleague, Professor Else Hoffmann and her team. We dearly miss Monte Greer to whom this symposium was dedicated and addressed so eloquently by Vladimir Strbák in his Dedication to Monte. Monte and I became friends in Smolenice and had begun to discuss the 2003 meeting only a few days before his tragic accident in 2002. There are others who were not with us, and we missed them, too. We would not have been able to succeed in this event without the unflagging support of our higher administration at Wright State University, the NIDDKD of the National Institute of Health, and the Fuji Medical System (see Acknowledgments).
