The initial design criteria in the choice of indwelling materials for medical and dental purposes may be pragmatic, and based on the necessary mechanical properties required to fashion a functional device. Orthopedic implants require strong materials for weight-bearing, and articulating surfaces such as joints require durability and resistance to wear. Stents and shunts require flexibility and patency, and sutures require a high tensile strength yet also must be flexible enough for intricate manipulation. As the devices became more sophisticated and developments in materials science provided more options for manufacture, implants are being used more frequently and with longer anticipated lifetimes. Concurrently, the design process increasingly incorporated biocompatibility and comfort into the design criteria. However, with longer lifetimes, the more frequent use of invasive surgical procedures involving indwelling devices and biologically-friendly materials, there has been a rise in the number of incidences of device-related infection. Urinary catheters have been estimated to account for 30% of all nosocomial infections [1]. Between 66 and 88% of these occur after urinary catheterization [2]. It is also reported that almost 100% of catheterized patients develop an infection in an openly draining indwelling catheter which has been in place for four days or more [2]. For some procedures, such as orthopedic joint arthroplasties, the diagnosed surgical site infection rates are relatively low (between 1% and 2%; [3]); however, the increasing number of patients undergoing joint-replacement surgery translates to large numbers of patients afflicted with the consequences of complicating infections per year. Furthermore, infection of artificial joints can be devastating, since oral or IV antibiotic therapy frequently fails to resolve the infection, with the only remaining course of action being surgical debridement or partial or total revision. These two examples, the first with very high numbers of patients but of lesser severity in terms of impact to the individual, and the second, low numbers but severe patient impact, reflect the incentive to pursue a third design criteria—that of infection resistance—into materials and devices [4]. In the following sections we will discuss the role of bacterial biofilms in infection, and the growing literature highlighting biofilms as an important cause of device-related infection.

Encephalitis lethargica (‘sleeping sickness’) was a mysterious disorder that swept the world in the decade following the First World War, before disappearing without its cause having been identified. Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease – attacking both motor and mental functions – appeared just as the separation of neurology and psychiatry had reached a critical point. Encephalitis lethargica sufferers presented an unprecedented combination of neurologic and psychiatric symptoms – including previously puzzling phenomena primarily associated with schizophrenia and hysteria, as well as behavioral changes and attention deficit disorders in children – that not only underscored the unity of mind and movement in the CNS, but also illuminated the critical role played by subcortical structures in consciousness and other higher mental functions that had formerly been associated with the soul and more recently presumed to be localized to the human cerebral cortex. However, human activity within river valleys also commonly bridges areas of both wetland and dryland; ecological zones which are often approached using quite different archaeological methods and which present considerable differences in levels of archaeological visibility and preservation. The site at Mill Lane offered an uncommon opportunity to explore the interface between these two types of environment. Here we present the results of the study of a wetland/dryland interface on the edge of palaeochannels of the River Cam in Cambridgeshire. Through the integrated archaeological and palaeoenvironmental analysis of a site on the western edge of Sawston, a detailed picture of life on the edge of the floodplain from the late glacial to the post-medieval periods has been developed. At the heart of this is the relationship between people and their changing environment, which reveals a shifting pattern of ritual, occupation and more transitory activity as the riparian landscape in a wooded setting became a wetland within a more openly grazed environment. The presence of potential built structures dating to the early Neolithic, the early Bronze Age and the early Anglo-Saxon periods provides some sense of continuity, although the nature of these structures and the environmental context within which they were constructed was very different. The site at Mill Lane, Sawston, represents millennia of human activity within a dynamic and changing landscape setting. The present volume contains papers and poems presented at Saarland University's international conference "A World of Local Voices: Poetry in English Today" (October 22-23, 1999), and the "Day of International Poetry" (October 24, 1999), both organised by the university's Department of North American Literature and Culture. This book, Bacteriophages in health and disease, is an effort to provide an introduction to the breadth of roles that phages play or can play in our everyday lives. To capture this variety of phage roles in human conditions, both natural and applied, the book is divided into three parts. A brief introduction to various concepts and terminology associated with phages is provided in chapter 1. Part I (chapters 2-6) considers the role of phages in the natural state. That is, where phages are, how they contribute directly to disease, the underlying mechanism by which phages do this, and how they can contribute indirectly to disease, that is, to pathogen evolution. Part II (chapters 7-11) considers various phage-based technologies other than the use of whole phages to combat bacterial infections (i.e. besides phage therapy).
