Health care reform is always about more than morality and values, but if it lacks a moral foundation, it will not stand the test of time. Dr. Olson provides a moral foundation expressed in eight standards derived from his study of ethics, systems theory, and health policy research. From these normative standards he derives a vision of universal health care as both a public policy of protecting and promoting the health of an entire population, and a system for organizing, fi nancing, and delivering high quality care, which is affordable and accessible to everyone based on their need. Universal health care is fi nanced fairly to ensure sustainability through shared responsibility with personal choice of health care providers and/or health care plans. This unifying vision for health care reform is pluralistic with respect to the design of systems to implement it. The author provides several moral arguments from divergent ethical theories, which converge to support his vision of universal health care and its specifi c elements. In the process, he illustrates how to reason to moral conclusions based on clear thinking about both values and relevant facts. He also shows why and how ethical discourse is both relevant and necessary to the formulation, implementation, and evaluation of health care policy. Discussion of the controversy over medically necessary care and an ethical analysis of the Affordable Care Act are unique features of this book.

This 1991 study deals with a specific set of institutions in nineteenth-century Athens. Relying on matrimonial contracts, travellers' accounts, memoirs and popular literature, the authors show how distinctive forms of marriage, kinship and property transmission evolved in Athens in the nineteenth century. These forms then became a feature of wider Greek society which continued into the twentieth century. Greece was the first post-colonial modern nation state in Europe whose national identity was created largely by peasants who had migrated to the city. As Athenian society became less agrarian, a new mercantile group superseded and incorporated previous elites and went on to dominate and control the new resources of the nation state. Such groups developed their own, more mobile, systems of property transmission, mostly in response to external pressures of a political and economic character. This is a persuasive piece of detective work which has advanced our knowledge of modern Greece. It is a model for scholarship on the development of family and other 'intimate' ideologies where nation states encroach upon local consciousness.

This book presents how metasurfaces are exploited to develop new low-cost single sensor based multispectral cameras. Multispectral cameras extend the concept of conventional colour cameras to capture images with multiple color bands and with narrow spectral passbands. Images from a multispectral camera can extract significant amount of additional information that the human eye or a normal camera fails to capture and thus have important applications in precision agriculture, forestry, medicine, object identifications, and classifications. Conventional multispectral cameras are made up of multiple image sensors each externally fitted with a narrow passband wavelength filters, optics and multiple electronics. The need for multiple sensors for each band results in a number of problems such as being bulky, power hungry and suffering from image co-registration problems which in turn limits their wide usage. The above problems can be eliminated if a multispectral camera is developed using one single image sensor.

This resource considers the Baroque cello's revival as part of the period instrument movement from the viewpoints of more than forty cellists from three generations and four luthiers who have worked on period cellos. What emerges is a nuanced and detailed picture of the cello in the past and present and the varied instruments now played under the label 'Baroque cello.' Period instruments played with appropriate techniques have become a major presence in classical music. For the cello, which changed substantially between the sixteenth and eighteenth centuries, it is challenging to describe specific traits for certain time periods. Through improvements in strings and the efforts of luthiers such as Stradivari, the cello became smaller and easier to play. Many controversies remain concerning the Baroque cello's form, including aspects of the bass bar, neck, fingerboard, and bridge. Although an uneasy consensus on technical matters has emerged for Baroque cellists today, one still encounters significant questions on important issues. Doubts compound when period performers enter the Classic and Romantic eras. By chronicling the searches of top cellists in England, Europe, and North America, the author reveals the great variety of forms that exist among what cellists call the 'Baroque cello.' This is the first study in which the revival of a single period instrument has been considered in such qualified detail. This book also offers many details concerning the history of the period performance movement in reference to famous ensembles and musicians. This volume will be welcomed by musicologists, luthiers, and anyone interested in string history.
