This book takes an in-depth look at the emerging technologies that are transforming the way clinicians manage patients, while at the same time emphasizing that the best practitioners use both artificial and human intelligence to make decisions. AI and machine learning are explored at length, with plain clinical English explanations of convolutional neural networks, back propagation, and digital image analysis. Real-world examples of how these tools are being employed are also discussed, including their value in diagnosing diabetic retinopathy, melanoma, breast cancer, cancer metastasis, and colorectal cancer, as well as in managing severe sepsis. With all the enthusiasm about AI and machine learning, it was also necessary to outline some of criticisms, obstacles, and limitations of these new tools. Among the criticisms discussed: the relative lack of hard scientific evidence supporting some of the latest algorithms and the so-called black box problem. A chapter on data analytics takes a deep dive into new ways to conduct subgroup analysis and how it’s forcing healthcare executives to rethink the way they apply the results of large clinical trials to everyday medical practice. This re-evaluation is slowly affecting the way diabetes, heart disease, hypertension, and cancer are treated. The research discussed also suggests that data analytics will impact emergency medicine, medication management, and healthcare costs. An examination of the diagnostic reasoning process itself looks at how diagnostic errors are measured, what technological and cognitive errors are to blame, and what solutions are most likely to improve the process. It explores Type 1 and Type 2 reasoning methods; cognitive mistakes like availability bias, affective bias, and anchoring; and potential solutions such as the Human Diagnosis Project. Finally, the book explores the role of systems biology and precision medicine in clinical decision support and provides several case studies of how next generation AI is transforming patient care.

The dominant model of democratic politics emphasizes reason at the expense of the passions. Passions have been treated as dangerous, the opposite of reason and the enemy of virtue. Paul Ginsborg and Sergio Labate challenge this model and put forward a very different view, developing an account of modern democratic politics in which both passions and reason play a crucial role. To do justice to the role of passions in politics, we must pay close attention to the way in which they circulate among us; then we must develop a suitable language to describe them – an ‘alphabet of the passions’ that enables us to understand how they combine with one another and connect with certain states of mind in order to shape political outcomes. Adopting this approach enables the authors to shed new light on one of the major phenomena of our time – the triumph of neoliberalism on a world scale. Neoliberalism has worked so well because it has incorporated its own romantic and individualist version of the passions into its worldview, seducing both individuals and families with the allure of consumption. By developing a new model of democratic politics based on the interplay of passions and reason, Ginsborg and Labate provide a much needed framework for understanding the crucial role that passions play in the unfolding of political life. At a time when populist leaders are on the ascendancy and political processes are shaped as much by anger, resentment and fear as they are by reason and argument, this refocusing of political analysis on the role of the passions could not be more timely.

The colony called Santo Domingo, which became the Dominican Republic, was the violent crucible in which the ingredients of the New World, drawn from America, Europe and Africa, were fused together for the first time: humans, religions, technologies, animals, plants and learned behaviors. The history of the Dominican Republic diverged from the patterns established by the rest of Latin America, as it ultimately gained independence not from Spain, but from Haiti, and Spain later recolonized the country during a watershed period in the 1860s. In the 20th century, the United States occupied the Dominican Republic on two formative occasions, from 1916 to 1924 and again in 1965-1966, interventions detailed in this volume. At every turn, the backdrop to this pattern of shaky sovereignty has been the extreme instability of Dominican politics, which has been punctuated by incessant civil wars, coups, and periods of dictatorship, until the last few decades. This book, Bacteriophages in health and disease, is an effort to provide an introduction to the breadth of roles that phages play or can play in our everyday lives. To capture this variety of phage roles in human conditions, both natural and applied, the book is divided into three parts. A brief introduction to various concepts and terminology associated with phages is provided in chapter 1. Part I (chapters 2-6) considers the role of phages in the natural state. That is, where phages are, how they contribute directly to disease, the underlying mechanism by which phages do this, and how they can contribute indirectly to disease, that is, to pathogen evolution. Part II (chapters 7-11) considers various phage-based technologies other than the use of whole phages to combat bacterial infections (i.e. besides phage therapy). This includes in particular the use of both modified and 'disembodied' phage parts. Phages thus can serve as carriers and delivery vehicles of DNA and also of other chemicals, including serving as vectors for either gene therapy or DNA vaccines. Part III (chapters 12-17) covers phage-based antibacterial strategies. It includes chapters on: phage translocation, safety and immunomodulation; phage therapy of wounds and related purulent infections; phage therapy of non-wound infections; phage-based enzybiotics; and phage-based control of bacterial pathogens in food. The final chapter of this book is targeted to would-be phage therapy experimentalists, one that considers, in light of phage properties, how phage therapy protocols may be developed in terms of the use of animal models of bacterial disease.
