The book has been developed in conjunction with NERS 462, a course offered every year to seniors and graduate students in the University of Michigan NERS program. The first half of the book covers the principles of risk analysis, the techniques used to develop and update a reliability data base, the reliability of multi-component systems, Markov methods used to analyze the unavailability of systems with repairs, fault trees and event trees used in probabilistic risk assessments (PRAs), and failure modes of systems. All of this material is general enough that it could be used in non-nuclear applications, although there is an emphasis placed on the analysis of nuclear systems. The second half of the book covers the safety analysis of nuclear energy systems, an analysis of major accidents and incidents that occurred in commercial nuclear plants, applications of PRA techniques to the safety analysis of nuclear power plants (focusing on a major PRA study for five nuclear power plants), practical PRA examples, and emerging techniques in the structure of dynamic event trees and fault trees that can provide a more realistic representation of complex sequences of events. The book concludes with a discussion on passive safety features of advanced nuclear energy systems under development and approaches taken for risk-informed regulations for nuclear plants.

In the post-1945 era, the aircraft carrier has remained a valued weapon despite the development of nuclear weapons, cruise and ballistic missiles, and highly capable submarines. At times, as in the early days of the Korean and Vietnam Wars and in the Falklands conflict, carriers alone could deploy high-performance aircraft to the battlefield. In other operations, such as enforcing the no-fly zones and the U.S. invasions of Afghanistan and Iraq, only carriers could provide the bases needed for sustained combat and support operations. This second volume of Norman Polmar's landmark study details the role of carriers in the unification of the U.S. armed forces and strategic deterrence, fiscally constrained Great Britain, the development of British Commonwealth and ex-colonial navies, and the efforts of France and the Netherlands to rebuild their fleets. The role of the modern carrier-nine nations currently possess them-is discussed, as are the issues confronting nations that might acquire them. Chapters on the Soviet Union's effort to produce carriers are included for the first time. The development of both carrier planes and the many "oddball" aircraft that have flown from carriers-such as the U-2 spy plane-are also examined. Appendixes include comprehensive data on all carriers built and converted through 2006. This is the sixth monograph in the series The U.S. Navy and the Vietnam War. It covers aircraft carrier activity during Operation Rolling Thunder in the war. Operation Rolling Thunder was one of the longest sustained aerial bombing campaigns in history. And it would be a failure. The U.S. Navy proved essential to the conduct of Rolling Thunder. Exploiting the inherent flexibility and mobility of naval forces, the Seventh Fleet operated with impunity for three years off the coast of North Vietnam. The success with which the Navy executed the later Operation Linebacker campaign against North Vietnam in 1972 revealed how much the service had learned from and exploited the Rolling Thunder experience of 1965–1968. The book includes several photographs with backgrounds of key aircraft used as part of Operation Rolliing Thunder during the Vietnam War. In early August 1974, despite incredible risks and after six years of secret preparations, the CIA attempted to salvage the sunken Soviet ballistic missile submarine K-129 from the depths of the North Pacific Ocean. The audacious effort was undertaken with the cover of an undersea mining operation sponsored by eccentric billionaire Howard Hughes. “Azorian”—incorrectly identified as Project Jennifer by the press—was the most ambitious ocean engineering endeavor attempted by man. Following the accidental sinking of a Soviet missile submarine in March 1968, U.S. intelligence agencies were able to determine the precise location and to develop a means of raising the submarine from a depth of 16,400 feet. The remarkable salvage effort of the K-129, which contained nuclear-armed torpedoes and one nuclear tipped missile as well as crypto equipment, was conducted with Soviet naval ships a few hundred yards from the lift ship, the Hughes Glomar Explorer. The book is based, in part, on the research for Michael White’ documentary film Azorian: The Raising of the K-129, released in late 2009. The research for the book and the documentary forced the CIA to issue a brief report on Project Azorian in early 2010, with one-third of the document redacted.
