“[S]heds light not only on the obstacles to making motherhood safer, but to improving the health of poor populations in general.”—Social Anthropology Since 1987, when the global community first recognized the high frequency of women in developing countries dying from pregnancy-related causes, little progress has been made to combat this problem. This study follows the global policies that have been implemented in Sololá, Guatemala in order to decrease high rates of maternal mortality among indigenous Mayan women. The author examines the diverse meanings and understandings of motherhood, pregnancy, birth and birth-related death among the biomedical personnel, village women, their families, and midwives. These incongruous perspectives, in conjunction with the implementation of such policies, threaten to disenfranchise clients from their own cultural understandings of self. The author investigates how these policies need to meld with the everyday lives of these women, and how the failure to do so will lead to a failure to decrease maternal deaths globally. From the Introduction: An unspoken effect of reducing maternal mortality to a medical problem is that life and death become the only outcomes by which pregnancy and birth are understood. The specter of death looms large and limits our full exploration of either our attempts to curb maternal mortality, or the phenomenon itself. Certainly women’s survival during childbirth is the ultimate measure of success of our efforts. Yet using pregnancy outcomes and biomedical attendance at birth as the primary feedback on global efforts to make pregnancy safer is misguided.
When the United Nations announced the Millennium Development Goals (MDGs) in 2000, approximately half a million women worldwide died each year from complications associated with pregnancy and childbirth. The fifth MDG aimed to reduce the maternal mortality rate by 75 per cent between 1990 and 2015, but by the target date, the goal had not been reached. In The Limits of Trust Lisa Nicole Mills investigates the reasons why Mexico in particular did not meet its objective. Focusing on the states of Guerrero, Chiapas, and Oaxaca, where maternal mortality rates are the highest in the country, Mills looks into how MDG 5 has been implemented in Mexico, how it has been experienced by individuals and groups, what obstacles have been encountered, and what factors have facilitated improvements in maternal health. Using data gathered from interviews with NGOs, government officials, and health care workers, the book argues that government and feminist NGO efforts to build trust in the health care system have fallen short because of systemic failures to protect women’s rights and enhance the quality of health care. In Mexico a woman’s risk of dying from a pregnancy-related complication is five times higher than in developed countries. The Limits of Trust explores the realities of implementing maternal health initiatives on the ground in rural, remote, and impoverished areas, and the steps that can be taken to successfully combat maternal mortality.
In recent years many nations have asked why not enough housing is being built or, when it is built, why it isn't of the highest quality or in the best, most sustainable, locations. Politics, Planning and Housing Supply in Australia, England and Hong Kong examines the politics and planning of new homes in three very different settings, but with shared political traditions: in Australia, in England and in Hong Kong. It investigates the power-relationships and politics that underpin the allocation of land for large-scale residential schemes and the processes and politics that lead to particular development outcomes. Using a comparative framework, it asks: how different systems of urban governance and planning mediate the supply of land for housing; whether and how these system differences influence the location, quantity and price of residential land and the implications for housing outcomes; what can be learned from these different systems for allocating land, building consensus between different stakeholders, and delivering a steady supply of high quality and well located homes accessible to, and appropriate for, diverse housing needs. This book frames each case study in a comprehensive examination of national and territorial frameworks before dissecting key local cases. These local cases – urban renewal and greenfield growth centres in Australia, new towns and strategic sites in England, and major development schemes in Hong Kong – explore how broader urban planning and housing policy goals play out at the local level. While the book highlights a number of potential strategies for improving planning and housing delivery processes, the real challenge is to give voice to a broader array of interests, reconstituting the political process surrounding planning and housing development to prioritise homes in well-planned places for the many, rather than simply facilitating investment opportunities for the few.
