The Doyle, Carbone, and Roth families immigrate to the United States from Europe for a variety of reasons: the 1850 potato famine in Ireland, religious freedom, and persecution of the mafia in Sicily by Mussolini in 1925. It is the appearance of Oscar Klein, an orphan from Philadelphia, which brings them all together in the post-depression, pre-World War II era of the late 1930s to 1940s. Oscar Klein is a kid of the streets without formal education, but he becomes the driving force and innovator. His ideas shape, mold, and secure the future of his colleagues, Patrick Doyle and Anthony Carbone. Together they create KCD Incorporated, purchase taxi medallions, and own and operate parking garages, private clubs, and restaurants. Oscar marries Mildred Stein—a wedding not to be missed at the Plaza Hotel attended by the New York City elite, including Anthony’s father, Luca Carbone, the Mafia Don and head of the Cosa Nostra in New York. Together they build success and give back to the community. However, with accomplishments come unavoidable heartbreak and misfortune, illness, drug addiction, and death. Follow the saga of four families that combines historical events with the personal dramas of its characters.

Babies Hospital of New York, The

The Babies Hospital, now known as Morgan Stanley Children's Hospital, was founded in 1887 by Drs. Sarah and Julia McNutt in a brownstone on Fifty-Fifth Street and Lexington Avenue. The hospital is the first freestanding children's hospital in New York City and the fourth oldest in the United States. However, the hospital traces its roots to the establishment of the Vagelos College of Physicians and Surgeons of Columbia University, then King's College, more than 250 years ago. In 1929, the hospital relocated to a new 204-bed facility as part of the Columbia-Presbyterian Medical Center. The New York Times referred to the new Babies Hospital as "the last word in hospital design and equipment." Under the leadership of Rustin Mcintosh from 1931 to 1960, the hospital became a beacon for discovery and innovation, assembled a department of noted subspecialists, and was one of the first children's hospitals to develop programs in neonatology, surgery, radiology, neurology, hematology-oncology, and psychiatry. Michael Weiner, MD, Hettinger professor of pediatrics at Columbia University Irving Medical Center, is a vice chair of pediatrics, former director of the Division of Pediatric Oncology, and an author of Living Cancer: Stories of an Oncologist, Father, Survivor and a Philanthropist. Stephen E. Novak has been the head of Archives & Special Collections at Columbia's Augustus C. Long Health Sciences Library since 1997. The images in the book come from the rich photographic resources of the library.

To date (2013), we have witnessed over 100 years of research on Alzheimer’s disease (AD). In the past century, this gave new and deep insights into its pathophysiology. However, no new treatment with an innovative mode of action has found its way to the patients since the first targeted therapies with cholinergic drugs like Tacrin. So far A-beta and related targets have shown negative results in pivotal Phase III studies, after showing promising trends in Phase II. Thus the question arises as to whether A-beta is the wrong target for disease-modifying treatments, or whether other substantial changes to clinical development plans need to be made, e.g. including patients at risk of developing AD instead of those already experiencing symptoms. Alternative targets and study design options like enrichment strategies, however, do not evolve as an easy alternative solution.

Alzheimer’s disease (AD) confers an additional risk of death. The magnitude of this impact is related to the person’s underlying mortality and current AD stage. When planning clinical trials, estimating the number of deaths prior to study initiation, and periodically during the study as participants are enrolled, assists in providing an informative context, particularly if there are few life events. Additionally, deaths have the potential to introduce a bias, thus, equal follow-up needs to ensure for participants assigned to either active or placebo, particularly if the period of observation for safety events extends beyond the end of the study. During AD clinical trials, patients discontinue for reasons other than death and at substantially high rates, which has the potential to introduce significant bias. Planning and incorporating strategies to improve retention and identify successful techniques should be an integral part of the study process.

The literature on inference from the resulting incomplete data is now huge, and continues to grow both as methods are developed for large and complex data structures, and as increasing computer power and suitable software enable researchers to apply these methods. This book focuses on a particular statistical method for analysing and drawing inferences from incomplete data, called Multiple Imputation (MI). MI is attractive because it is both practical and widely applicable. The authors aim is to clarify the issues raised by missing data, describing the rationale for MI, the relationship between the various imputation models and associated algorithms and its application to increasingly complex data structures. Multiple Imputation and its Application: Discusses the issues raised by the analysis of partially observed data, and the assumptions on which analyses rest. Presents a practical guide to the issues to consider when analysing incomplete data from both observational studies and randomized trials. Provides a detailed discussion of the practical use of MI with real-world examples drawn from medical and social statistics.
