This volume contains the papers of the 19th International Congress of Computer Assisted Radiology and Surgery (CARS 2005) held in Berlin, Germany between 22 and 25 June 2005. For 20 years, CARS has developed a culture of innovation with its focus on interdisciplinary and international cooperation. In approximately 20,000 pages of proceedings written by several thousand authors from more than 50 countries, many innovative developments have been reported which now assist the daily practice of physicians in their care of patients. Examples are PACS, a concept on which CARS was founded, and computer assisted surgical tools and systems, which were initially reported in CAR 85 and have now become mainstream developments. Some of these innovations are incremental, making noticeable improvements in daily practice, but others like PACS or minimally invasive surgery are transformational innovations in a sense that they have fundamentally changed the way "things" are done. CARS has established itself as the major event for the presentation of R & D work of high actuality. In addition to the traditional scientific/medical sessions, some of the outstanding topics presented and which are included in the CARS 2005 conference proceedings include: Interventional Radiology; Colon and Liver CAD; Intra-Operative Imaging; Minimal Invasive Spine Surgery; PACS Beyond Radiology (in conjunction with EuroPACS); Surgical PACS and the Digital Operating Room (in conjunction with SPIE Integrating the Health Care Enterprise (in conjunction with EuroPACS). The process of innovation in these fields is a continuum with many examples of other new developments being presented at CARS 2005, which marks the 20th anniversary of the congress.

Dorotheus of Gaza and Ascetic Education approaches fundamental questions about the role and function of education in late antiquity through a detailed study of the thought of Dorotheus of Gaza, a sixth-century Palestinian monk. It illumines the thought of a significant figure in Palestinian monasticism, clarifies relationships between ascetic and classical education, and contributes to debates about how different educational projects related to late-antique cultural change. Dorotheus appropriates and reconfigures classical discourses of rhetoric, philosophy, and medicine and builds on earlier ascetic traditions. Education is a powerful site for the reconfiguration and reproduction of culture, and Dorotheus' educational programme can be read as a microcosm of the wider culture he aims to construct partly through his adaptation and representation of classical and ascetic discourses. Key features of his educational programme include the role of the notion of godlikeness, the governing role of humility as an epistemic virtue intended to organize affective and ethical development, and his notion of education as life-long habituation. For Dorotheus, education is irreducibly affective and transformative rather than merely informative at the individual and communal scales. His epistemology and ethics are set within an account of the divine plan of salvation which is intended to provide a narrative framework through which his students come to understand the world and their place in it. His account of ways of knowing and ordering knowledge, ethics and moral development, emotions of education, and relationships between affect, cognition, and ethical action aims towards transformation of his students and their communities.

The key contribution of the approach to x-ray mammographic image analysis developed in this monograph is a representation of the non-fatty compressed breast tissue that we show can be derived from a single mammogram. The importance of the representation, called hint, is that it removes all those changes in the image that are due only to the particular imaging conditions (for example, the film speed or exposure time), leaving just the non-fatty 'interesting' tissue. Normalising images in this way enables them to be enhanced and matched, and regions in them to be classified more reliably, because unnecessary, distracting variations have been eliminated. Part I of the monograph develops a model-based approach to x-ray mammography, Part II shows how it can be put to work successfully on a range of clinically-important tasks, while Part III develops a model and exploits it for contrast-enhanced MRI mammography. The final chapter points the way forward in a number of promising areas of research.
How great is the evolutionary distance between humans and apes, and what is it that creates that gulf? Philosophers and scientists have debated the question for centuries, but Michael Corballis finds the mystery revealed in our right hands. For humans are the only primates who are predominantly right handed, a sign of the specialization of the left hemisphere of the brain for language. And that specialization, he tells us, makes a massive distance indeed, as he describes what exactly it means to be the lopsided ape. In The Lopsided Ape, Corballis takes us on a fascinating tour of the origins and implications of the specialization of the two halves of the brain--known as laterality--in human evolution. He begins by surveying current views of evolution, ranging from the molecular level--the role of viruses, for instance, in transporting genes between species--to the tremendous implications of such physical changes as walking on two feet. Walking upright freed our ancestors' arms for such things as tool-making and gesturing (a critical part of early language). Corballis argues that the evolution of the brain--and language--was intimately tied up with these changes: The proliferation of objects made by early hominids, in an increasingly artificial environment marked by social cooperation, demanded greater flexibility in communication and even in thinking itself. These evolutionary pressures spurred the development of laterality in the brain. He goes on to look at the structure of language, following the work of Noam Chomsky and others, showing how grammar allows us to create an infinite variety of messages. In examining communication between animals and attempts to teach apes and dolphins language, he demonstrates that only humans have this unlimited ability for expression--an ability that he traces back through hominid evolution. After this engrossing account of what we know about evolution, language, and the human brain, Corballis suggests that the left hemisphere has evolved a Generative Assembling Device, a biological mechanism that allows us to manipulate open-ended forms of representation and provides the basis for mathematics, reasoning, music, art, and play as well as language and manufacture. Activity within neural circuits shapes the synaptic properties of component neurons in a manner that maintains stable excitatory drive, a process referred to as homeostatic plasticity. These potent and adaptive mechanisms have been demonstrated to modulate activity at the level of an individual neuron, synapse, circuit, or entire network, and dysregulation at some or all of these levels may contribute to neuropsychiatric disorders, intellectual disability, and epilepsy. Greater mechanistic understanding of homeostatic plasticity will provide key insights into the etiology of these disorders, which may result from network instability and synaptic dysfunction. Over the past 15 years, the molecular mechanisms of this form of plasticity have been intensely studied in various model organisms, including invertebrates and vertebrates. Though once thought to have a predominantly postsynaptic basis, emerging evidence suggests that homeostatic mechanisms act on both sides of the synapse through mechanisms such as retrograde signaling, to orchestrate compensatory adaptations that maintain stable network function. These trans-synaptic signaling systems ultimately alter neurotransmitter release probability by a variety of mechanisms including changes in vesicle pool size and calcium influx. These adaptations are not expected to occur homogenously at all terminals of a pre-synaptic neuron, as they might synapse with neurons in non-overlapping circuits. However, the factors that govern the homeostatic control of synapse-specific plasticity are only beginning to be understood. In addition to our limited molecular understanding of pre-synaptic homeostatic plasticity, very little is known about its prevalence in vivo or its physiological and disease relevance.
