On the same day the Japanese surrender ended World War II, Vietnamese nationalists declared independence from France. Within weeks, France sought to reestablish colonial rule. In 1906, Cord Meyer Development Company purchased 600 acres in Whitepot and renamed it Forest Hills after its high elevation of rolling hills and proximity to Forest Park. After the Russell Sage Foundation acquired 142 acres and Grosvenor Atterbury and Frederick Law Olmsted Jr. partnered, the Forest Hills Gardens, founded in 1909, became America's earliest planned garden community. When Henry Schloh and Charles Hausmann of the Rego Construction Company came upon farmland in Forest Hills West, they renamed it Rego Park in 1923 after their slogan, "REal GOod Homes." Between the Tudor and Colonial landmarks, one can sense the footsteps of a few hundred notables who granted soul to the community and society. At the Forest Hills Tennis Stadium, imagine the Beatles landing in a helicopter in front of screaming fans in 1964, or when Althea Gibson became the first African American to win a US national tennis title in 1957. Forest Hills High School was a cornerstone for notable alumni, such as composer Burt Bacharach; musical duo Simon & Garfunkel; Bob Keeshan, who portrayed Captain Kangaroo; and the first space tourist, Dennis Tito.

The Kakoli of the Western Highlands of Papua New Guinea (PNG), the focus of this study, did not traditionally have a concept of mental illness. They classified madness according to social behaviour, not mental pathology. Moreover, their conception of the person did not recognise the same physical and mental categories that inform Western medical science, and psychiatry in particular was not officially introduced to PNG until the late 1950s. Its practitioners claimed that it could adequately accommodate the cultural variation among Melanesian societies. This book compares the intent and practice of transcultural psychiatry with Kakoli interpretations of, and responses to, madness, showing the reasons for their occasional recourse to psychiatric services. Episodes involving madness, as defined by the Kakoli themselves, are described in order to offer a context for the historical lifeworld and praxis of the community and raise fundamental questions about whether a culturally sensitive psychiatry is possible in the Melanesian context.

During the last 20 years the development of enzyme histochemical techniques has contributed greatly to knowledge of muscle pathology. However, these and other new methods, such as electron microscopy and immunocytochemistry, have only relatively recently become gener ally available for routine use in histopathology. Muscle biopsy is a long-established technique in clinical practice, having been introduced by Duchenne in 1868 (Arch. Gen. Med. , 11, 5-179). However, the needle method used by Duchenne was not generally adopted, although Shank and Hoagland described a similar technique in 1943 (Science, 98, 592). During this time muscle biopsies required a surgical procedure and this was a considerable disincentive to their use. It was not until Bergstrom (1962; Scand. J. Clin. Lab. Invest. , 14, Suppl. 68) and Edwards (1971; Lancet, ii, 593--6) developed a simple biopsy needle suitable for muscle work in connection with exercise physiology that the advantages of needle muscle biopsies came to be appreciated. Since then, muscle biopsies have become a relatively minor procedure. This has led to the increasing use of muscle biopsy in clinical practice, both for diagnosis and for assessing progress in repeated biopsies during the course of a disorder and its treatment. The full range of enzyme histochemical and ultrastructural histological techniques can be applied to these small biopsies and many of the older histological staining methods can also be used. This book is intended to serve as a practical guide in muscle pathology, particularly for histopathologists, and for those in training.

This book is intended for clinicians as well as basic scientists in biochemistry, chemistry, pharmacology, and cell biology.
