Part of the Oxford Neurology Library, this pocketbook highlights the importance of stroke, its diagnosis, the problems of misdiagnosis, and current thoughts on its pathogenesis, acute treatment and rehabilitation as well as prevention.
Neurologists learn from their patients, and this selection of 60 stroke cases will inform and challenge clinicians at all stages in their careers. Including both common and unusual cases, the aim is to reinforce diagnostic skills through careful analysis of individual presenting patterns, and to guide treatment decisions. Each case consists of a clinical history, examination findings and special investigations, usually involving imaging before a diagnosis is given. There then follows for each case a discussion of the clinical issues raised by the case, in which the main teaching points are emphasized. Selected references, frequently including the first description, are provided at the conclusion of each case. Drawing on the expertise of leading teachers and practitioners, and liberally illustrated, these case studies and the discussions that accompany them are an essential guide to learning the complexity of stroke diagnosis.
Neuroimaging techniques are crucial in the management of stroke patients. This book is an important resource in the quest to better understand stroke and its heterogeneity. After a first chapter on the classification of stroke, it outlines that neuroimaging techniques are not only useful to diagnose stroke, its mechanisms, and its causes, but are also an important tool to improve our knowledge on the pathophysiology of stroke and of its recovery. This book has involved prestigious contributors who have a great knowledge on this topic, and are skilled at describing the current state of knowledge, and also at projecting developments that are likely to occur in the future. This book is useful for all those who have to manage stroke patients at the acute stage, or later, and for those who are in search of a focused, authoritative review on this subject. It will assume a prominent place as a reference.

This book provides a carefully selected compilation of challenging cases representative of the situations and pathologies likely to be encountered when performing cardiovascular imaging using CT. The conditions covered include coronary artery disease, anomalous coronary arteries, congenital heart disease, coronary artery bypass grafts, infectious diseases, structural heart disease, tumors, and aortic pathology. The book also provides insights into various scanning techniques geared towards the pre-procedural use of cardiac CTA in patients undergoing atrial fibrillation ablation, thoracic endovascular aortic repair, or transcatheter aortic valve replacement. In addition, scanning techniques for some of the more difficult cases that a cardiac imager may encounter in practice are reviewed. All images are high-resolution reproductions, and subsequent cardiac catheterization images are included for cases in which obstructive coronary artery disease was revealed. Clinical Pearls in Diagnostic Cardiac Computed Tomographic Angiography is specifically designed to meet the needs of residents, fellows, and physicians who have an interest in cardiovascular CT.

Modern adult brain surgery is a very new discipline. While many would consider the fathers of modern neurosurgery to be people like Victor Horsley, Harvey Cushing, and Walter Dandy, they all worked in the early 20th century, in an era before the creation of equipment now considered to be “game-changers” in the field of neurosurgery. Only towards the end of the 20th century did we see such critical advances as the operating microscope, the wide availability of CT and MRI imaging, neuro-endoscopy, stereotactic neuro-navigation, stereotactic radiosurgery, interventional neuro-endovascular techniques, and intra-operative neuromonitoring. It is not just that these advances occurred only recently, but it is even more recently that they have become accessible to many neurosurgeons. Furthermore, the scientific evidence for adult brain surgery in this new era is itself extremely new and a work in progress. There is certainly both an “art” and a “science” to the practice of medicine and also to the practice of adult brain surgery. Furthermore, there is also a wide range of acceptable practices in regards to adult brain surgery, ranging from the most conservative/minimally invasive options to the most aggressive approaches. The author of Put Down the Knife, neurosurgeon Dr. Michael Brisman, is of the belief that the pendulum in medicine has swung way too far to the “art” side and away from the “science.” Furthermore, given the very high risks associated with adult brain surgery, the default choice of treatment should be the more conservative/minimally invasive options when possible. This book explores adult brain surgery from a more conservative vantage point, highlighting potential errors in thought related to decision-making and rationales for brain surgery as well as interpretation of the surgical literature. Focused chapters then dive into considerations of less invasive and even non-invasive approaches for various conditions of the brain, including tumors, cysts, hematomas, pain and movement disorders, skull base disorders, and much more.
This text blends theoretical and scientific aspects with practical and directly applicable diving physiology and medical information. Despite a worldwide reduction in its incidence, stroke remains one of the most common diseases generally and the most important cause of premature and persistent disability in the industrialized countries. The most frequent cause of stroke is a localized disturbance of cerebral circulation, i.e., cerebral ischemia. Less common are spon taneous intracerebral and subarachnoid hemorrhages and sinus ve nous thromboses. The introduction of new diagnostic procedures such as cranial computed tomography, magnetic resonance imaging, digi tal subtraction radiologic techniques, and various ultrasound tech niques has led to impressive advances in the diagnosis of stroke. Through the planned application of these techniques, it is even possible to identify the pathogenetic mechanisms underlying focal cerebral ischemia in humans. However, these diagnostic advances have made the gap between diagnostic accuracy and therapeutic implications even greater than before. This fact can be easily explained. In the past, therapeutic studies had to be based on the symptoms and temporal aspects of stroke; it was impossible for early investigations to consider the various pathogeneses of cerebral ischemia. Inevitably, stroke patients were treated as suffering from a uniform disease.
