The Merger: M.D.s and D.O.s in California If you are interested in the recent history of the medical professions, this book is for you. If personal narratives of historical events speak to you as a second layer of documentation, this book is for you. If you are aware that in America there exist two separate yet equal, fully licensed physicians, M.D.s and D.O.s, you might be interested in learning about their unique relationship in California. If you know little about D.O.s, this book will give you a picture of their approach to patient care and to their M.D. colleagues. The osteopathic profession in California has a unique history, as it differs dramatically from the professions history in the rest of the nation. More than 100 years ago, a small pioneering group of osteopathic physicians established in Southern California the Pacific School of Osteopathy to graduate physicians and surgeons with the ability to acquire an unlimited license. Since then, the educational, research, and regulatory arenas of osteopathy have seen in California low points of near elimination and high points of recognition. Cultures are based on firm beliefs in the truth of their understanding of the world. Often they collide with those who respect different truths. Similarly, the medical culture in California went through collisions between osteopathic and allopathic medicine, often in response to competition and antagonism. Which values and beliefs about each others profession were held so fervently in California that prompted the unique event of absorbing the osteopathic profession into allopathic mainstream medicine? This project explores the events, unique to California but with repercussions nation-wide, of a merger between osteopathic and allopathic medicine. In 1962, the relatively small medical organization of fully licensed osteopathic physicians (the California Osteopathic Organization) merged with the much larger mainstream medical profession (the California Medical Association). What were the incentives for a fully licensed parallel healthcare profession to forfeit its identity and philosophy? What key players and leaders emerged? How did the individual practicing physician think and feel about the merger? While about two thousand osteopathic physicians changed to the M.D. degree, about two hundred California D.O.s did not merge but persevered in their battle to restore the licensing power of their profession in California. What social and personal motivational sources sustained this group for over a decade? How has osteopathys unique history affected medical education and professional relations, nation-wide and internationally? Answers to these questions have emerged in historical narratives by key persons figuring in the events. Most of them have not written about their lives and their social and political surroundings at the time of the merger and its repercussions. Many never learned the long-term outcomes of their endeavors. Our multidisciplinary research team transcribed in-depth interviews to capture the thoughts and feelings among individuals who played significant roles from the 1940s to the 70s. With the approval of the Institutional Review Board of the University of California, Irvine for the protection of the participants rights, we asked a diverse group, 35 in all, of physicians, administrators, lawyers and lobbyists, to provide their historical narratives and their suggestions for future directions. Our objective has been to give an unbiased account, listening equally to representatives of allopathy, osteopathy, and politics. Inspired by Dr. Gevitz cogent academic analysis of osteopathic medicine in America, this book presents personal perceptions of events, integrated with documented descriptions, stored in archives, to facilitate the readers understanding and analysis. MicroRNAs (miRNAs) are a member of the family of non-coding RNA molecules, and consist of small conserved sequences between 19–25 nucleotides in length that are responsible for regulating many cellular functions by affecting a wide range of messenger RNAs in a sequence specific manner. Fundamental biological processes like cell proliferation and growth, stress resistance, tumorigenesis, fat metabolism, and neural development have all been shown to be governed by miRNAs. miRNAs carry out the post-transcriptional silencing of gene expression via targeting the 30-untranslated region (UTR) of the complementary mRNA sequence. The dysregulation of the expression levels of various miRNAs is typical of tumor cells, and has been associated with tumor progression and poor prognosis. Many miRNAs are up-regulated in cancer, where they can silence tumor suppressor genes such as apoptosis and immune response associated genes. Therefore, it is possible to profile the expression levels of miRNAs as biomarkers, in order to diagnose cancer and noncancerous diseases. Moreover, cancer detection in the early stages is crucial in clinical situations. Characterization of miRNAs in serum, plasma, and other bodily fluids, and understanding their stability against RNase degradation, is important to assess their suitability as biomarkers and diagnostic tools. Exosomes play an important role in inter-cellular communications, and these nanosized particles have various functions in diverse physiological pathways, in normal as well as abnormal cells. Exosomes can carry diverse cargos such as mRNAs, miRNAs, and proteins that transfer information between donor and recipient cells. Furthermore, uptake of exosomes and their cargos may promote or suppress various molecular and cellular pathways, which alter the cellular behavior. Many reports have discussed the role of exosomes released from cancer cells on the progression of cancer at various stages. Exosomes and their cargos may affect the growth of the tumor, metastasis, drug resistance, immune system function, as well as angiogenesis. Therefore, exosomes have been explored as diagnostic biomarkers in many cancers. Moreover, exosomes can be used as biological vehicles to deliver different drugs and agents like doxorubicin (DOX), miRNAs, and siRNAs. The present book covers the role of exosomes and micro-RNAs in the pathogenesis and treatment of various diseases.

The marchers Poles, Slovaks, Hungarians, most of whom could not yet speak English were themselves armed only with an American flag and a timid, budding confidence in their new found rights as free men in their newly adopted country. The mine operators took another view of these rights and of the strange, alien men who claimed them. When the posse was done firing, nineteen of the demonstrators were dead and thirty-nine were seriously wounded. Some six months later a jury of their peers was to exonerate the deputies of any wrong-doing. This long-forgotten incident is here movingly retold by Michael Novak, himself the son of Slovak immigrants and one of our most gifted writers and social observers. In his hands, the so-called "Lattimer Massacre" becomes not only a powerful story in its own right (and an invaluable key to the history of the growth of the united mine Workers), but an allegory of that peculiarly American experience undergone over and over again throughout the land, and down to this very day; the experience of new immigrants, still miserable with poverty and bewilderment and suffering the trauma of culture shock, being confronted by the hostility and blind contempt of the "real" Americans. This chronicle primarily concerns the spiritual journey that led him to United Methodism and what it was like, not only to choose it, but also to inhabit it.
