Though the Civil War ended in April 1865, the conflict between Unionists and Confederates continued. The bitterness and rancor resulting from the collapse of the Confederacy spurred an ongoing cycle of hostility and bloodshed that made the Reconstruction period a violent era of transition. The violence was so pervasive that the federal government deployed units of the U.S. Army in North Carolina and other southern states to maintain law and order and protect blacks and Unionists. Bluecoats and Tar Heels: Soldiers and Civilians in Reconstruction North Carolina tells the story of the army's twelve-year occupation of North Carolina, a time of political instability and social unrest. Author Mark Bradley details the complex interaction between the federal soldiers and the North Carolina civilians during this tumultuous period. The federal troops attempted an impossible juggling act: protecting the social and political rights of the newly freed black North Carolinians while conciliating their former enemies, the ex-Confederates. The officers sought to minimize violence and unrest during the lengthy transition from war to peace, but they ultimately proved far more successful in promoting sectional reconciliation than in protecting the freedpeople. Bradley's exhaustive study examines the military efforts to stabilize the region in the face of opposition from both ordinary citizens and dangerous outlaws such as the Regulators and the Ku Klux Klan. By 1872, the widespread, organized violence that had plagued North Carolina since the close of the war had ceased, enabling the bluecoats and the ex-Confederates to participate in public rituals and social events that served as symbols of sectional reconciliation. This rapprochement has been largely forgotten, lost amidst the postbellum barrage of Lost Cause rhetoric, causing many historians to believe that the process of national reunion did not begin until after Reconstruction. Rectifying this misconception, Bluecoats and Tar Heels illuminates the U.S. Army's significant role in an understudied aspect of Civil War reconciliation.

Patient-centered care for chronic illness is founded upon the informed and activated patient, but we are not clear what this means. We must understand patients as subjects who know things and as agents who do things. Bioethics has urged us to respect patient autonomy, but it has understood this autonomy narrowly in terms of informed consent for treatment choice. In chronic illness care, the ethical and clinical challenge is to not just respect, but to promote patient autonomy, understood broadly as the patients' overall agency or capacity for action. The primary barrier to patient action in chronic illness is not clinicians dictating treatment choice, but clinicians dictating the nature of the clinical problem. The patient's perspective on clinical problems is now often added to the objective-disease perspective of clinicians as health-related quality of life (HRQL). But HRQL is merely a hybrid transitional concept between disease-focused and health-focused goals for clinical care. Truly patient-centered care requires a sense of patient-centered health that is perceived by the patient and defined in terms of the patient's vital goals. Patient action is an essential means to this patient-centered health, as well as an essential component of this health. This action is not extrinsically motivated adherence, but intrinsically motivated striving for vital goals. Modern pathophysiological medicine has trouble understanding both patient action and health. The self-moving and self-healing capacities of patients can be understood only if we understand their roots in the biological autonomy of organisms. Taking the patient as the primary perceiver and producer of health has the following policy implications: 1] Care will become patient-centered only when the patient is the primary customer of care. 2] Professional health services are not the principal source of population health, and may lead to clinical, social and cultural iatrogenic injury. 3] Social justice demands equity in health capability more than equal access to health services.

In this original and illuminating new study, Mark Knights reveals how the political culture of the eighteenth century grew out of earlier trends and innovations. Arguing that the period from 1675 needs to be seen as the second stage of a seventeenth-century revolution that ran on until c.1720, Representation and Misrepresentation in Later Stuart Britain charts the growth of a national political culture and traces the development of the public as an arbiter of politics. In doing so, it uncovers a crisis of public discourse and credibility, and finds a political enlightenment rooted in local and national partisan conflict. The later Stuart period was characterized by frequent elections, the lapse of pre-publication licensing, the emergence of party politics, the creation of a public debt, and ideological conflict over popular sovereignty. These factors combined to enhance the status of the 'public', not least in requiring it to make numerous acts of judgement. Contemporaries from across the political spectrum feared that the public might be misled by the misrepresentations pedalled by their rivals. Each side, and those ostensibly of no side, discerned a culture of passion, slander, libel, lies, hypocrisy, dissimulation, conspiracy, private languages, and fictions. 'Truth' appeared an ambiguous, political matter. Yet the reaction to partisanship was also creative, for it helped to construct an ideal form of political discourse. This was one based on reason rather than passion, on moderation rather than partisan zeal, on critical reading rather than credulity; and an increasing realization that these virtues arose from infrequent rather than frequent elections. Finding synergies between social, political, religious, scientific, literary, cultural, and intellectual history, Representation and Misrepresentation in Later Stuart Britain reinvigorates the debate about the emergence of 'the public sphere' in the later Stuart period.
