This is the tale of two plastic surgeons, and how they changed the face of plastic surgery. Their contributions will have a standing impact on future generations. About the Author S. Anthony Wolfe, M.D. was the Emeritus Chief of Plastic Surgery at Nicklaus Children’s Hospital in Miami, Florida. He was also a clinical professor of surgery at the University of Miami, the University of Florida, and Florida International University. He was a founding member and Past-President of the International Society of Craniofacial Surgery. He served for 12 years on the Medical Advisory Board for SmileTrain and was trained in general surgery at the Peter Bent Brigham Hospital in Boston, Massachusetts under Francis D. Moore. He served in plastic surgery at the University of Miami under D. Ralph Millard. He became Dr. Millard’s associate in 1975 and remained so for 25 years. In 1974, he served as assistant to Dr. Paul Tessier in Paris and remained a close collaborator until his death in 2008. Dr. Wolfe also authored the biography of Dr. Tessier, “A Man from Héric.” Erin M. Wolfe, M.D. is a Plastic Surgery Resident at The University of Southern California in Los Angeles.

This study measures the impact of changing economic conditions in OECD countries on tourist arrivals to countries/destinations in Latin America and the Caribbean. A model of utility maximization across labor, consumption of goods and services at home, and consumption of tourism services across monopolistically competitive destinations abroad is presented. The model yields estimable equations arrivals as a function of OECD economic conditions and the elasticity of substitution across tourist destinations. Estimates suggest median tourism arrivals decline by at least three to five percent in response to a one percent increase in OECD unemployment, even after controlling for declines in OECD consumption and output gaps. Arrivals to individual destination are driven by differing exposure to OECD country groups sharing similar business cycle characteristics. Estimates of the elasticity of substitution suggest that tourism demand is highly price sensitive, and that a variety of costs to delivering tourism services drive market share losses in uncompetitive destinations. One recent cost change, the 2009 easing of restrictions on U.S. travel to Cuba, supported a small (countercyclical) boost to Cuba’s arrivals of U.S. non-family travel, as well as a pre-existing surge in family travel (of Cuban origin). Despite the US becoming Cuba’s second highest arrival source, Cuban policymakers have significant scope for lowering the relatively high costs of family travel from the United States.

The shortcomings result from the focus on the general government, the exclusion of non-cash operations, and divergences between the timing of cash outlays and the economic impact of assistance operations. Complementing the standard measures of the fiscal balance with an “augmented” balance would provide a definition that is transparent, comprehensive, and reasonably comparable across countries. The augmented balance would explicitly incorporate the major quantifiable fiscal costs of bank assistance operations that are not already included in current definitions of the overall balance.
