In 2005 Medicare paid over $1.4 billion for anesthesia services (AS). These services are provided by anesthesia practitioners (AP), such as anesthesiologists and certified registered nurse anesthetists. A study found that Medicare payments for AS are lower than private payments. This difference may create regional discrepancies in the supply of AP. This report examines: the extent to which Medicare payments for AS were lower than private payments across Medicare payment localities; whether the supply of AP across Medicare payment localities was related to the differences between Medicare and private payments for AS or the concentration of Medicare beneficiaries; and compensation levels for AP in 2005 and trends in graduate training. The Wyoming Centennial Wagon Train ended in Cody in a dismal, torn-down drive-in movie theater. Before setting up the corral, we were forced to clear away shards of glass, bent nails, broken lumber. My prairie skirt and petticoats hung ragged and clay-caked, and under a droopy Stetson my frizzled hair appeared at once greased and starched beyond human recognition. A cloud, a sort of vaporousness, redolent with fresh acrid sweat on top of powerful stale sweat, hung thickly about me. Laced, as it was, with a woman's sweet musky secretions, and all gone past ripe, oddly it was a pungency I savored. Such goaty piquance, though, was cause to be shunned in any town setting. The look of my world had changed. Gone were the high-dollar designer clothes and the zipping around fabled Marin County in a candy-apple-red 1966 Mustang convertible. It was true that I unfailingly sought the ironies in life and, with a kind of dual personality, shifted easily through incongruencies such as town strolls in high heels and backcountry hiking in bare feet; the bucket seats of a classic automobile and the broken-down bench of a beater truck. It was only during the years that Iíd worn white overalls, taped drywall, and come home every night much like Charles Schulz's Pig Pen, flaking a cloud of dried white mud bits onto the rug, that I'd felt moved to keep my fingernails painted red. Now I was to slip farther than ever planned toward one end of my seesaw and then, incredibly, by conscious design, inch out even farther." --from Barefoot-Hearted With more than 1.5 million copies in print, Kathleen Meyer's groundbreaking international bestseller, How to Shit in the Woods: An Environmentally Sound Approach to a Lost Art, has been widely embraced by the outdoor community and has found its way into myriad places: national parks, outdoor leadership schools and scout-troop headquarters, the camp tents of those who have discovered that it is amusing out-loud reading, and the bathroom-literature baskets of households around the world. Now, from the Rocky Mountain West, Meyer brings us Barefoot-Hearted: A Wild Life Among Wildlife, a coming-into-the-country story told with the frank, dry humor and sharp research of her first book. The country, in this case, is Montana's tall, reaching landscape with its ever underfoot wild critters; the on-tenterhooks territory of a new romantic relationship; and the pressure cooker that is our precarious global imbalance. Meyer finds herself in midlife standing out under yawning skies, surrounded by sagebrush and cactus, having fallen for the Irish charm of itinerant farrier Patrick McCarron. As partners, they travel across three mountain states with draft horses and a covered wagon and then set up housekeeping in a seventy-five-year-old dairy barn. In this primitive structure, the author rapidly discovers she's living with troops of mice, a nursery colony of seventy-five bats, sexually fired-up skunks, and more flies than in a pig shed. She tells of a freakish season that or-phaned seventy-seven bear cubs, an unusual fly-fishing trip on a famed blue-ribbon trout stream, the visitations of moose, and the discovery of a den of wolves. Meyer's prose is original and inspired, playful yet provocative. She carries us vividly back to the settlers' old West while pondering modern-day dilemmas, those of fitting into this fast hurtling world, of determining amid the earth's rising extinctions of species, whose planet it is, and of managing to stay empowered residing with a man who "stands six feet six and beats steel on an anvil for a living." A personal chronicle of conscience and a love story of rare and quirky dimension, Barefoot-Hearted catapults readers into new realms of thought, deftly guided there by Meyer's sense of the ironic, the randy, and the humorous.

Smartsville and Timbuctoo (California State Landmarks Nos. 321 and 320) are essentially one place with two names. As worked-out claims and floods forced placer forty-niners up from the sandbars into the hills above the Yuba River, and as word spread around the world about gold in the California hills, towns and communities formed. The Smartsville and Timbuctoo area was once the most populated place in eastern Yuba County. Black Bart, Jim "the Timbuctoo Terror" Webster, and other desperadoes haunted the local roads. Eventually fires, worked-out diggings, and the Sawyer Decision succeeded in driving out all but the most dedicated (and in some cases eccentric) residents. Neither town, though, is ready yet for the dustbin of history: the population might once again explode-this time not with gold seekers but with long-distance commuters, turning the former boomtowns into future bedroom communities.
In the first decades after mass production, between 1913 and 1939, middle-class Americans not only bought cars but also enthusiastically redesigned them. By examining the ways Americans creatively adapted their automobiles, Tinkering takes a fresh look at automotive design from the bottom up, as a process that included manufacturers, engineers, advice experts, and consumers in various guises. Franz argues that automobile ownership opened new possibilities for ingenuity among consumers even as large corporations came to control innovation. Franz weaves together a variety of sources, from serial fiction to corporate documents, to explore tinkering as a form of authority in a culture that valued ingenuity. Women drivers represented one group of consumers who used tinkering to advance their claim to social autonomy. Some canny drivers moved beyond modifying their individual cars to become independent inventors, patenting and selling automotive accessories for the burgeoning national demand for aftermarket products. Earl S. Tupper was one such tinkerer who went on to invent Tupperware. These savvy tinkerers worked in a changing landscape of invention shaped increasingly by automotive giants. By the 1930s, Ford and General Motors worked to change the popular discourse of ingenuity and used the world's fairs of the Depression as a stage to promote a hierarchy of innovation. Franz not only demonstrates the entrepreneurial spirit of American consumers but she engages larger historical questions about gender, consumption and ingenuity while charting the impact corporate expansion on tinkering during the first half of the twentieth century.

Mining in Yuba County illustrates the story of how this geologically unique part of California drew multitudes of people from around the world during the early days of the Gold Rush. It depicts the region's evolution from anarchy to civilization. It gives faces to the individuals who were instrumental in creating society in Yuba. It elaborates on incidents in which Yuba influenced the nation, on matters as historically significant as California entering the Union as a free state and affecting the outcome of the Civil War. Mining in Yuba fostered technological advancement precipitated by the depletion of surface gold and the necessity to get to the obscured gold. Conflicts between miners and agriculturists over hydraulic mining were litigated, and landmark legal decisions regarding the regulation of hydraulic mining all but ended the practice and were the beginnings of environmental protectionism and water rights issues in California. Finally, it reveals that despite heavy regulations that exist today, there is still mining in Yuba County.
A magisterial history of Indigenous North America that places the power of Native nations at its center, telling their story from the rise of ancient cities more than a thousand years ago to fights for sovereignty that continue today “A feat of both scholarship and storytelling.”—Claudio Saunt, author of Unworthy Republic Long before the colonization of North America, Indigenous Americans built diverse civilizations and adapted to a changing world in ways that reverberated globally. And, as award-winning historian Kathleen DuVal vividly recounts, when Europeans did arrive, no civilization came to a halt because of a few wandering explorers, even when the strangers came well armed. A millennium ago, North American cities rivaled urban centers around the world in size. Then, following a period of climate change and instability, numerous smaller nations emerged, moving away from rather than toward urbanization. From this urban past, egalitarian government structures, diplomacy, and complex economies spread across North America. So, when Europeans showed up in the sixteenth century, they encountered societies they did not understand—those having developed differently from their own—and whose power they often underestimated. For centuries afterward, Indigenous people maintained an upper hand and used Europeans in pursuit of their own interests. In Native Nations, we see how Mohawks closely controlled trade with the Dutch—and influenced global markets—and how Quapaws manipulated French colonists. Power dynamics shifted after the American Revolution, but Indigenous people continued to command much of the continent’s land and resources. Shawnee brothers Tecumseh and Tenskwatawa forged new alliances and encouraged a controversial new definition of Native identity to attempt to wall off U.S. ambitions. The Cherokees created institutions to assert their sovereignty on the global stage, and the Kiowas used their power in the west to regulate the passage of white settlers across their territory. In this important addition to the growing tradition of North American history centered on Indigenous nations, Kathleen DuVal shows how the definitions of power and means of exerting it shifted over time, but the sovereignty and influence of Native peoples remained a constant—and will continue far into the future.
Most Americans eat genetically modified food on a daily basis, but few of us are aware we’re eating something that has been altered. Meanwhile, consumers abroad refuse to buy our engineered crops; their groceries are labeled so that everyone knows if the contents have been modified. What’s going on here? Why does the U.S. government treat engineered foods so differently from the rest of the world? Eating in the Dark tells the story of how these new foods quietly entered America’s food supply. Kathleen Hart explores biotechnology’s real potential to enhance nutrition and cut farmers’ expenses. She also reveals the process by which American government agencies decided not to label genetically modified food, and not to require biotech companies to perform even basic safety tests on their products. Combining a balanced perspective with a sense of urgency, Eating in the Dark is a captivating and important story account of the science and politics propelling the genetic alteration of our food.

In a comparison of communication in the U.S. presidential primaries of the twentieth century, Kendall examines the role of the candidates and the media during the period of primary elections. Drawing upon information from a broad array of sources, Kendall uncovers communication patterns that transcend time regarding political image, horse race coverage, and negative campaigning. She takes a strong communication perspective, arguing that the verbal context of the presidential primaries is an important factor overlooked in traditional studies. Topics covered include the effect of party rules on communication, the role of speeches and debates, the role of political advertising, and the media's construction of the primaries in the pre- television era and the age of television. Kendall examines the 1996 primaries in light of patterns discovered in earlier years, and she makes predictions and recommendations regarding the 2000 primaries. With its century-wide scope and the variety of research methods used, the book will be of considerable value to researchers, scholars, journalists and students involved with political communication and American presidential elections.

This volume presents the Late Classical through Roman pottery from the University of Chicago excavations at Isthmia (1952-1989). In a series of three chapters-on the Late Classical and Hellenistic pottery, the Roman pottery, and the pottery from the Palaimonion-a general discussion is followed by a catalog presenting datable contexts and then by a catalogue of other noteworthy pottery. Appendixes discuss the stratigraphy of the Palaimonion and observations on new and previously published lamps. Amphora stamps are the focus of a further appendix, followed by a catalogue of the Slavic and Byzantine pottery found in the sanctuary area. Although the pottery is sometimes fragmentary, the range of materials over this thousand-year period is typical of Corinthian sites. The finds presented here provide critical information about the history of the Panhellenic sanctuary of Poseidon and the ritual activities that took place there.

Comestibles is Burnham’s first full-length poetry collection, reviewing the ties that bind in life and literature, exploring the connections that food/consumption share with language, birth, sexuality, spirituality, death, and God. The work is a confessionary minefield of what it means to be “the other woman” and to navigate becoming a mother in the wake of a failed relationship. Burnham weaves themes of family, illness, loss, maternity, divinity, grief, existential angst, and hope into poems that are darkly humorous and—most of all—unapologetic and direct.
Since the dawn of the republic, faith in social equality, religious freedom, and the right to engage in civic activism have constituted our national creed. In this bracing history, Kathleen D. McCarthy traces the evolution of these ideals, exploring the impact of philanthropy and volunteerism on America from 1700 to 1865. What results is a vital reevaluation of public life during the pivotal decades leading up to the Civil War. The market revolution, participatory democracy, and voluntary associations have all been closely linked since the birth of the United States. American Creed explores the relationships among these three institutions, showing how charities and reform associations forged partnerships with government, provided important safety valves for popular discontent, and sparked much-needed economic development. McCarthy also demonstrates how the idea of philanthropy became crucially wedded to social activism during the Jacksonian era. She explores how acts of volunteerism and charity became involved with the abolitionist movement, educational patronage, the struggle against racism, and female social justice campaigns. What resulted, she contends, were heated political battles over the extent to which women and African Americans would occupy the public stage. Tracing, then, the evolution of civil society and the pivotal role of philanthropy in the search for and exercise of political and economic power, this book will prove essential to anyone interested in American history and government.
