The story begins with the lively musical activities of a frontier town, the antecedents of the symphony orchestra that took shape at the turn of the century. From the early years of the organization under the batons of Emil Oberhoffer and Henri Verbrugghen, the chronicle rises to the period of the great contemporaries, Ormandy, Mitropoulos, and Antal Dorati. There is a wealth of detail on the career of Mitropoulos, the renowned New York Philharmonic conductor who reached his present stature during his leadership of the Minneapolis orchestra.The extensive concert tours that have earned for the Minneapolis symphony the nickname of "orchestra on wheels" are recalled in anecdotes that will evoke many a chuckle and plenty of amazement. Accounts of early recording sessions offer fascinating sidelights on this aspect of musical history. A complete list of the works performed by the orchestra during the past fifty years provides a significant record of changing trends in musical tastes. Since the dawn of civilization, mankind has been engaged in the conception and manufacture of discrete products to serve the functional needs of local customers and the tools (technology) needed by other craftsmen. In fact, much of the progress in civilization can be attributed to progress in discrete product manufacture. The functionality of a discrete object depends on two entities: form, and material composition. For instance, the aesthetic appearance of a sculpture depends upon its form whereas its durability depends upon the material composition. An ideal manufacturing process is one that is able to automatically generate any form (freeform) in any material. However, unfortunately, most traditional manufacturing processes are severely constrained on all these counts. There are three basic ways of creating form: conservative, subtractive, and additive. In the first approach, we take a material and apply the needed forces to deform it to the required shape, without either adding or removing material, i. e. , we conserve material. Many industrial processes such as forging, casting, sheet metal forming and extrusion emulate this approach. A problem with many of these approaches is that they focus on form generation without explicitly providing any means for controlling material composition. In fact, even form is not created directly. They merely duplicate the external form embedded in external tooling such as dies and molds and the internal form embedded in cores, etc. Till recently, we have had to resort to the 'subtractive' approach to create the form of the tooling.

Throughout history, humanity has been plagued by a myriad of humanitarian crises that seemingly take the form of perpetual human suffering. Today, approximately 125,000,000 people require humanitarian assistance as the result of famine, war, geopolitical conflict, and natural disasters. A core component of this suffering is afflictions related to human health, where disturbances strain or overwhelm the existing healthcare infrastructure to create the conditions for an increase in morbidities and co-morbidities. One of the more startling elements is the loss of life to preventable medical conditions that were not properly treated or even diagnosed in the field, and is often due to the limited interventional capacity that medical teams and humanitarian practitioners have in these scenarios. These individuals are often hindered by medical equipment deficiencies or devices not meant to function in austere conditions. The development of highly versatile, feasible, and cost-effective medical devices and technologies that can be deployed in the field is essential to enhancing medical care in unconventional settings. In this book we examine the nature of the creative problem-solving paradigm, and dissect the intersection of frugal, disruptive, open, and reverse innovation processes in advancing humanitarian medicine. Specifically, we examine the feasible deployment of these devices and technologies in unconventional environments not only by humanitarian aid and disaster relief agencies, but also by crisis-affected communities themselves. The challenge is complex, but the financial support and technical development of innovative solutions for the delivery of humanitarian aid is a process in which everyone is a stakeholder.
