Cost-effectiveness analysis is becoming an increasingly important tool for decision making in the health systems. Cost-Effectiveness of Medical Treatments formulates the cost-effectiveness analysis as a statistical decision problem, identifies the sources of uncertainty of the problem, and gives an overview of the frequentist and Bayesian statistical approaches for decision making. Basic notions on decision theory such as space of decisions, space of nature, utility function of a decision and optimal decisions, are explained in detail using easy to read mathematics. Features Focuses on cost-effectiveness analysis as a statistical decision problem and applies the well-established optimal statistical decision methodology. Discusses utility functions for cost-effectiveness analysis. Enlarges the class of models typically used in cost-effectiveness analysis with the incorporation of linear models to account for covariates of the patients. This permits the formulation of the group (or subgroup) theory. Provides Bayesian procedures to account for model uncertainty in variable selection for linear models and in clustering for models for heterogeneous data. Model uncertainty in cost-effectiveness analysis has not been considered in the literature. Illustrates examples with real data. In order to facilitate the practical implementation of real datasets, provides the codes in Mathematica for the proposed methodology. The motivation for the book is to make the achievements in cost-effectiveness analysis accessible to health providers, who need to make optimal decisions, to the practitioners and to the students of health sciences. Elías Moreno is Professor of Statistics and Operational Research at the University of Granada, Spain, Corresponding Member of the Royal Academy of Sciences of Spain, and elect member of ISI. Francisco José Vázquez-Polo is Professor of Mathematics and Bayesian Methods at the University of Las Palmas de Gran Canaria, and Head of the Department of Quantitative Methods. Miguel Ángel Negrín is Senior Lecturer in the Department of Quantitative Methods at the ULPGC. His main research topics are Bayesian methods applied to Health Economics, economic evaluation and cost-effectiveness analysis, meta-analysis and equity in the provision of healthcare services.

In the last several years, this disastrous escalation has placed the archipelago more centrally on the radar of residents and politicians in the United States, as the US Congress established an oversight board with emergency powers to ensure Puerto Rico's economic survival—and its ability to repay its debt. These events should not be understood as a random string of compounding misfortune. Rather, as demonstrated by Jose Atiles in Crisis by Design, they result from the social, legal, and political structure of colonialism. Moreover, Atiles shows how administrations, through emergency powers and laws paired with the dynamics of wealth extraction, have served to sustain and exacerbate crises. He explores the role of the local government, corporations, and grassroots mobilizations. More broadly, the Puerto Rican case provides insight into the role of law and emergency powers in other global south, Caribbean, and racialized and colonized countries. In these settings, Atiles contends, colonialism is the ongoing catastrophe.

Out of equilibrium, any system is assumed to depend locally on the same set of variables as when it is in eqUilibrium. This leads to a formal thermody namic structure identical to that of eqUilibrium: intensive parameters such as temperature, pressure and chemical potentials are well-defined quantities keeping their usual meaning, thermodynamic potentials are derived as Leg endre transformations and all equilibrium thermodynamic relations retain their validity. The theory based on this hypothesis has turned out to be very useful and has achieved a number of successes in many practical situations. of interest in going However, the recent decade has witnessed a surge beyond the classical formulation. There are several reasons for this. One of them is the development of experimental methods able to deal with the response of systems to high-frequency and short-wavelength perturbations, such as ultrasound propagation and light and neutron scattering. The ob served results have led to generalizations of the classical hydrodynamical theories, by including memory functions or generalized transport coefficients depending on the frequency and the wavevector. This field has generated impressive progress in non-equilibrium statistical mechanics, but for the moment it has not brought about a parallel development in non-equilibrium thermodynamics. An extension of thermodynamics compatible with gener alized hydrodynamics therefore appears to be a natural subject of research.

Instead, it focuses on a specific individual: Juan Rena (1480–1539). This Venetian clergyman spent 40 years serving the king in several capacities while travelling from the Maghreb to northern Spain, from the Pyrenees to the western fringes of the Ottoman Empire. By focusing on his activities, the book offers an account of the Spanish Empire’s frontiers as a vibrant political space where a multiplicity of figures interacted to shape power relations from below. Furthermore, it describes how merchants, military officers, nobles, local elites and royal agents forged a specific political culture in the empire’s liminal spaces. Through their negotiations and cooperation, but also through their competition and clashes, they created practices and norms in areas like cross-cultural diplomacy, the making of the social fabric, the definition of new jurisdictions, and the mobilization of resources for war.

We have come to know that our ability to survive and grow as a nation to a very large degree depends upon our scientific progress. Moreover, it is not enough simply to keep 1 abreast of the rest of the world in scientific matters. We must maintain our leadership. President Harry Truman spoke those words in 1950, in the aftermath of World War II and in the midst of the Cold War. Indeed, the scientific and engineering leadership of the United States and its allies in the twentieth century played key roles in the successful outcomes of both World War II and the Cold War, sparing the world the twin horrors of fascism and totalitarian communism, and fueling the economic prosperity that followed. Today, as the United States and its allies once again find themselves at war, President Truman’s words ring as true as they did a half-century ago. The goal set out in the Truman Administration of maintaining leadership in science has remained the policy of the U. S. Government to this day: Dr. John Marburger, the Director of the Office of Science and Technology (OSTP) in the Executive Office of the President, made remarks to that effect during his 2 confirmation hearings in October 2001. The United States needs metrics for measuring its success in meeting this goal of maintaining leadership in science and technology. That is one of the reasons that the National Science Foundation (NSF) and many other agencies of the U. S.

The Sublime South: Andalusia, Orientalism, and the Making of Modern Spain is the first systematic study on cultural images of Andalusia as Spain’s “Orient” and the impact they have had on nation-building and modernization since the late nineteenth century. While a wealth of studies have examined how northern Europeans from the Romantic period viewed Spain and Andalusia as Europe’s Orient, little attention has been paid to how contemporary Spanish artists and intellectuals assimilated Romantic legacies to engage in an internal form of orientalism. José Luis Venegas deftly explores Spain’s shifting engagements with oriental identity and otherness by looking, not just beyond national, ethnic, and racial borders, but at a territory that is institutionally embedded in the nation-state while symbolically placed between inclusion and abjection. The Sublime South shifts the focus and scale of Edward Said’s notion of orientalism by examining how it evolves and manifests transnationally, as the result of European colonialism in Africa and Asia, and intra-nationally, in a European yet orientalized country. Finally, Venegas challenges ethnocentric notions of Iberian cultures and fosters an understanding of the encounters between Western and Muslim cultures beyond opposing, and often mutually negating, essentialisms.

Race, Class, and the Dominican Experience in Providence

The descendents of twentieth-century southern and central European immigrants successfully assimilated into mainstream American culture and generally achieved economic parity with other Americans within several generations. So far, that is not the case with recent immigrants from Latin America and the Caribbean. A compelling case study of first- and second-generation Dominicans in Providence, Rhode Island, Encountering American Faultlines suggests that even as immigrants and their children increasingly participate in American life and culture, racialization and social polarization remain key obstacles to further progress. Encountering American Faultlines uses occupational and socioeconomic data and in-depth interviews to address key questions about the challenges Dominicans encounter in American society. What is their position in the American socioeconomic structure? What occupations do first- and second-generation Dominicans hold as they enter the workforce? How do Dominican families fare economically? How do Dominicans identify themselves in the American racial and ethnic landscape? The first generation works largely in what is left of Providence's declining manufacturing industry. Second-generation Dominicans do better than their parents economically, but even as some are able to enter middle-class occupations, the majority remains in the service-sector working class. José Itzigsohn suggests that the third generation will likely continue this pattern of stratification, and he worries that the chances for further economic advancement in the next generation may be seriously in doubt. While transnational involvement is important to first-generation Dominicans, the second generation concentrates more on life in the United States and empowering their local communities. Itzigsohn ties this to the second generation's tendency to embrace panethnic identities. Panethnic identity provides Dominicans with choices that defy strict American racial categories and enables them to build political coalitions across multiple ethnicities. This intimate study of the Dominican immigrant experience proposes an innovative theoretical approach to look at the contemporary forms and meanings of becoming American. José Itzigsohn acknowledges the social exclusion and racialization encountered by the Dominican population, but he observes that, by developing their own group identities and engaging in collective action and institution building at the local level, Dominicans can distinguish themselves and make inroads into American society. But Encountering American Faultlines also finds that hard work and hope have less to do with their social mobility than the existing economic and racial structures of U.S. society.

On December 22, 1997, forty-five unarmed members of the indigenous organization Las Abejas (The Bees) were massacred during a prayer meeting in the village of Acteal, Mexico. The members of Las Abejas, who are pacifists, pledged their support to the Zapatista Army of National Liberation, a primarily indigenous group that has declared war on the state of Mexico. The massacre has been attributed to a paramilitary group composed of ordinary citizens acting on their own, although eyewitnesses claim the attack was planned ahead of time and that the Mexican government was complicit.In Without History, Jose Rabasa contrasts indigenous accounts of the Acteal massacre and other events with state attempts to frame the past, control subaltern populations, and legitimatize its own authority. Rabasa offers new interpretations of the meaning of history from indigenous perspectives and develops the concept of a communal temporality that is not limited by time, but rather exists within the individual, community, and culture as a living knowledge that links both past and present. Due to a disconnection between indigenous and state accounts as well as the lack of archival materials (many of which were destroyed by missionaries), the indigenous remain outside of, or without, history, according to most of Western discourse. The continued practice of redefining native history perpetuates the subalternization of that history, and maintains the specter of fabrication over reality.Rabasa recalls the works of Marx, Lenin, and Gramsci, as well as contemporary south Asian subalternists Ranajit Guha and Dipesh Chakrabarty, among others. He incorporates their conceptions of communality, insurgency, resistance to hegemonic governments, and the creation of autonomous spaces as strategies employed by indigenous groups around the globe, but goes further in defining these strategies as millennial and deeply rooted in Mesoamerican antiquity. For Rabasa, these methods and the continuum of ancient indigenous consciousness are evidenced in present day events such as the Zapatista insurrection.

The solution for this problem, in the case of Puerto Rico, is full admission into the union as a state. Puerto Rico has already voted twice in favor of becoming a state, in 2012 and 2017. It's time for Congress to act and grant the US citizens the political equality they have voted for.
