This title was first published in 2003: This book provides an evaluation of the Gateshead Community Care Scheme which was devised as an alternative to residential and hospital care for frail elderly people. An important feature of the scheme was the decentralization of control of resources to individual social workers acting as care managers, with defined caseloads and expenditure limits to ensure accountability. The initial social social care scheme was subsequently extended to provide both health and social care to clients from a large general practice based in a health centre. The social care team was enlarged to include a nurse care manager and part-time doctor and physiotherapist. The study examines the operation of care management in both settings, the use of devolved budgets and services developed, the outcomes for clients and carers and the costs of care. Admissions to residential care were reduced and the elderly people who received the scheme’s support experienced a better quality of care and greater well-being when compared with elderly people receiving the usual range of services. This was achieved at no greater cost. The characteristics of those for whom the scheme was most appropriate are described. In addition, the pattern of development of the scheme as it was incorporated into the mainstream of the Social Services and after the implementation of the NHS and Community Care Act are examined. Final, the implications for the development of care management are considered.

Their long tradition has been honoured in many of the place names of Canada, especially in the West. In this collection, over 250 of the NWMP, RNWMP and RCMP members who died while on duty, or who enjoyed long or extraordinary careers, are remembered. Other place names are connected to a Mountie-related event or were named by a pioneering Mountie in honour of some significant occurrence. Authors William "Bill" Hulgaard and John "Jack" White, both retired Mounties, extended their research across Canada to compile the information for Honoured in Places.

The competition for limited health care resources is intensifying. We urgently need an acceptable method for deciding how they should be allocated. But the goods that health care produces are of very different kinds. Health care can extend the lives of children and of older people. It can make it possible for a person to walk, when without health care that person would be permanently bedridden; and it can reduce the pain and distress of people who are terminally ill. How can we possibly decide which of these - and many more - diverse achievements of health care are more deserving than others? We need a common unit by which we might be able to measure these very different goods. The Quality-Adjusted Life Year, or QALY, is the most developed proposal for such a unit of measure. In this book a distinguished team of ethicists and economists defend the core of the QALY proposal: that health care resources should be used so as to produce more years of life, of the highest possible quality. This leads to a discussion of such fundamental questions as whether all lives are of equal value, whether health care should be allocated on the basis of need and whether the QALY approach incorporates an adequate account of fairness or justice. The result is the most thorough account yet of the ethical issues raised by the use of the QALY as a basis for allocating health care resources.
