The first edition of this book was suggested by Tom Arie, Professor of Health Care of the Elderly at the University of Nottingham. The seven years since the first edition of this book have seen a revolution in the way that Health Services in the United Kingdom are organized. One of the original co-authors, Mike Church, has moved on through several NHS posts to independent practice in the area of management consultancy. He has been replaced by Carol Martin, a lecturer in the Psychology of Old Age in our department. In addition to the changes in the NHS there has been an explosion in the knowledge base of old age psychiatry, with several journals now devoted to the subject. A new diagnostic classificatory system (ICD10) has also been launched. In preparing this second edition, we have tried to remain true to the spirit of the first edition, whilst incorporating new knowledge and the changing context of our practice. Throughout the text we have used the terms 'patient' and 'client' interchangeably. The people we serve are the same whatever we call them! We have also, generally, but not always referred to patients in the female gender, since this reflects the fact that more women than men survive into old age. We have ordered the book so that the basic background information and skills in approaching the problems of old age are dealt with first.

The family, that most fundamentalof human groups, is currently perceived to be changing in response to social, biological, cultural and technological developments in our postmodernsociety. While the observed changes in families have been considered by some sociologists to be evidence of adaptation and, therefore, normal, the authors of this volume, consider them maladaptive. Viewing society from the point of view of clinical psychiatry, they point to greatly increased numbers of children born to single mothers, soaring rates of divorce, a statistically confirmed increase in mental disorders, increase in reported incest, high rates of depression in younger people and escalation of the amount of reported family violence as evidence that the family, as a social institution, is in crisis and can either move toward renewed vitality or continued deterioration. Perceiving a need to obtain information about family functioning that might lead to the increased stability and well-being of this critically important type of system, Dr. John Schwab and his associates designed and camed out a research program that began with a thorough review of relevant literature beginning with LePlay’s study of 300 families in the 1850’sand including important recent statistical studies. They found that although these studies represent advances in understanding the family system, some serious problems with the research remain, one of which is confounding variables such as family function and mental or substance abuse disorders so that if a family member has a problem, such as drug abuse, the family is classifiedasdysfunctional.

A conventional wisdom among creolists is that creole is a sociohistorical term only: that creole languages share a particular history entailing adults rapidly acquiring a language usually under conditions of subordination, but that structurally they are indistinguishable from other languages. The articles by John H. McWhorter collected in this volume demonstrate that this is in fact untrue. Creole languages, while complex and nuanced as all human languages are, are delineable from older languages as the result of their having come into existence only a few centuries ago. Then adults learn a language under untutored conditions, they abbreviate its structure, focusing upon features vital to communication and shaving away most of the features useless to communication that bedevil those acquiring the language non-natively. When they utilize their rendition of the language consistently enough to create a brand-new one, this new creation naturally evinces evidence of its youth: specifically, a much lower degree of the random accretions typical in older languages, which only develop over vast periods of time. The articles constitute a case for this thesis based on both broad, cross-creole ranges of data and focused expositions referring to single creole languages. The book presents a general case for a theory of language contact and creolization in which not only transfer from source languages but also structural reduction plays a central role, based on facts whose marginality of address in creole studies has arisen from issues sociopolitical as well as scientific. For several decades the very definition of the term creole has been elusive even among creole specialists. This book attempts to forge a path beyond the inter- and intra-disciplinary misunderstandings and stalemates that have resulted from this, and to demonstrate the place that creoles might occupy in other linguistic subfields, including typology, language contact, and syntactic theory.

The seemingly innocent observation that the activities of organisms bring about changes in environments is so obvious that it seems an unlikely focus for a new line of thinking about evolution. Yet niche construction--as this process of organism-driven environmental modification is known--has hidden complexities. By transforming biotic and abiotic sources of natural selection in external environments, niche construction generates feedback in evolution on a scale hitherto underestimated--and in a manner that transforms the evolutionary dynamic. It also plays a critical role in ecology, supporting ecosystem engineering and influencing the flow of energy and nutrients through ecosystems. Despite this, niche construction has been given short shrift in theoretical biology, in part because it cannot be fully understood within the framework of standard evolutionary theory. At the time of this writing, there is much uncertainty about the form of this country's future healthcare system and the role of psychiatry and other mental health disciplines in that system. Current experience with various managed healthcare programs is not encouraging. Most often patients with severe psychiatrie disturbances receive, at best, so me form of crisis intervention or brief treatment. Marital and family approaches to treatment receive even less support. This discouraging socioeconomic context makes the work of John Schwab and his colleagues even more important than it would be in more favorable times. Their message is clear: The family is crucial to an understanding of psychiatrie disorders and must often be the major focus in the treatment of these disorders. This book is unique in its direct reflection of the senior author's long-term professional interests-the family, epidemiology, and history. A careful reading provides family therapists and researchers with won derful opportunities to examine the ways in which history, socio economie and politieal contexts, and epidemiology can be used to in crease understanding of the family. This his tory of the family is unusually thorough; in particular, I found fascinating the information about early Egyptian families (3000 B.C.) and their accordance of high status of women.
