Over a decade has passed since the publication of the First Edition; we have yet to understand the pathogenesis of rheumatoid arthritis and there is still no cure. A wide range of developments in techniques, materials, imaging and understanding of biological processes justify this rewrite. Above all, the experience of many colleagues worldwide with commitments to a particular anatomical or technical area has led to a more balanced and refined approach to the problems of rheumatoid surgery. It has also highlighted areas where we need radically new solutions. The two additional authors, apart from their relative youth, bring expertise in their respective fields: Steve Copeland, on the shoulder and Jo Edwards on the mechanisms of tissue damage and the rationale of medical measures. We are conscious of the importance of communication between orthopaedic colleagues worldwide and the need to use a language shared with rheumatologists and histopathologists. We consider it is for the surgeon to know more of this disease than the narrow confines of surgical technique. We are doctors with a special interest in rheumatoid arthritis and we aim to maintain the broader view rather than be confined by the limitations of over specialisation. Although joint replacement is at present a major weapon for the relief of suffering we appreciate that it is an unphysiological procedure and there are other directions for progress.
The Voice of Harriet Taylor Mill is a work about collaboration: Harriet's life with her lover, friends, and members of her family; Harriet's joint work with John Stuart Mill; and the author's interaction with the reader. Jo Ellen Jacobs explores and expands the concept of biography using Salman Rushdie's analogy of history as a process of "chutnification." She gives Harriet's life "shape and form -- that is to say, meaning" in a way that will "possess the authentic taste of truth." In the first chapter, the first 30 years of Harriet's life are presented in the format of a first-person diary -- one not actually written by HTM herself. The text is based on letters and historical context, but the style suggests the intimate experience of reading someone's journal. The second chapter continues the chronological account of HTM until her death in 1858. In an interlude between the first and second chapters, Jacobs pauses to explore Harriet's life with John Stuart Mill; and in the final chapter, she argues persuasively that Harriet and John collaborated extensively on many works, including On Liberty.

Covered wagons brought a wave of migration to northern Illinois in the mid-1830s. On April 1, 1834, the first permanent white settlers, Joseph McCarthy and two assistants, paddled up the Fox River. The vicinity was known as Waubonsie's Village at that time. They built a log cabin, a dam across the Fox, and eventually a sawmill. The village had about 400 Native Americans who bartered fish for bread and tobacco. For almost 175 years now, growth has been steady and sure, and the city of Aurora is the second-largest metropolitan area in Illinois. Aurora is home to honorable civic institutions, excellent education, and a multicultural and energetic population.
Enormous numbers of boys and youths served in the American Civil War. The first book to arrive at a careful estimate, Of Age argues that underage enlistees comprised roughly ten percent of the Union army and likely a similar proportion of Confederate forces. This book examines the current interest in recruiting and supporting more men in the early childhood education workforce. Drawing on extensive empirical data from case studies of an unusual English preschool setting and interviews with Swedish male preschool staff, the author explores the potential benefits to society linked to the increased presence of men in early childhood education, and how this might be achieved. Throughout the book, the author maintains that the potential of including more men in early childhood education can only be achieved by gender-sensitive practitioners, both women and men, who are willing to create a gender-flexible pedagogy. This books focus is on one particular corporate governance risk that arises from the similarities between traditional elements of a willful blindness cause of action and those of company directors bad faith. An essential aspect of any consideration of corporate governance is the role played by the directors of companies who might have been facilitating the wrongdoing, in part, by remaining willfully blind while falling foul of the good-faith obligation. This study investigates cross application of the doctrines of good faith and willful blindness in company law in Delaware, USA, in comparison with company law in England, UK, with relation to company directors conduct. Here is the argument that courts in both legal systems under a particular set of conditions and hearing cases of company directors falling foul of the good-faith obligation should consider whether the willful-blindness doctrine sheds light on the interpretation of company directors alleged misconduct. A positive consideration of this approach could expand the courts horizons to include the most apparent individuals, namely the company directors, to face liability concerns for corporate disasters.
This book offers a critical perspective of the dominant discourses within the field of psychological trauma. It provides a challenge to normative western constructs and unsettles assumptions about accepted notions of universality and the nature of trauma. Traditionally the concept of psychological trauma has been widely accepted within mental health professions. However, in a post-positivist era, the language of mental health is shifting and making room for alternative discourses that include wider contextual influences, such as the impact of sociological, cultural, and technological developments. These wider discourses are illuminated as the authors draw together some of these arguments into one accessible text. Rather than claim definitive answers to the issues raised, readers are invited to engage with the discussions presented in order to position themselves in relation to the range of trauma discourses available.

How did chemistry and physics acquire their separate identities, and are they on their way to losing them again? Mary Jo Nye has written a graceful account of the historical demarcation of chemistry from physics and subsequent reconvergences of the two, from Lavoisier and Dalton in the late eighteenth century to Robinson, Ingold, and Pauling in the mid-twentieth century. Using the notion of a disciplinary "identity" analogous to ethnic or national identity, Nye develops a theory of the nature of disciplinary structure and change. She discusses the distinctive character of chemical language and theories and the role of national styles and traditions in building a scientific discipline. Anyone interested in the history of scientific thought will enjoy pondering with her the question of whether chemists of the mid-twentieth century suspected chemical explanation had been reduced to physical laws, just as Newtonian mechanical philosophers had envisioned in the eighteenth century.
