New problems of resistance and untoward effects arise; reduction of host toxicity without loss of antitumour activity by new substances occupies wide attention. The improved results with cancer chemotherapy, es pecially in leukaemias, are leading to a greater prevalence of severe infection in patients so treated, pharmacokinetics of drugs in normal and diseased subjects is receiving increasing attention along with related problems of bioavailability and interactions between drugs. Meanwhile the attack on some of the major bacterial infections, such as gonorrhoea and tubercu losis, which were among the first infections to feel the impact of chemotherapy, still continue to be major world problems and are now under attack with new agents and new methods. From this wide field and the 1,000 papers read at the Congress we have produced Proceedings which reflect the variety and vigour of research in this important field of medicine. Interesting and important ethical questions confront researchers, regulators, institutional review boards, support personnel, and research participants committed to the ethical conduct of human subjects research at all stages of research. Questions encompass - but are not limited to - study design, enrolling participants, balancing the clinical needs of participants against the research agenda, ending trials, discharging post-trial obligations, and resolving conflicts. Straightforward solutions to these types of questions are often not found in regulations, ethics codes, or the bioethics literature. Ethics consultation, which has been offered in clinical care settings with regularity since the 1980s, has since the turn of the century increasingly been sought in the clinical research context. Because there has only lately been recognition that ethics consultants can play a valuable role helping the research community conduct research in the most ethically informed way, there are many open questions in the field of research ethics consultation including the appropriate role of consultants and the best methods of consultation. The Clinical Center Bioethics Consultation Service has been serving the NIH community of researchers, administrators, healthcare providers, and research participants for more than a decade, conducting nearly 1,000 consultations in that time. In this book, members of the Bioethics Consultation Service reflect on this long track-record and unparalleled range of research ethics consultations to share a collection of their most interesting and informative research ethics consultations and to start a dialogue on remaining open questions. Although the NIH experience is unique, this book focuses on cases - and associated lessons - that are generalizable and valuable for the entire clinical research community. Volume XV "Urology in Childhood" was written in 1956/57 and was the first in the series of the Encyclopedia of Urology to appear. This present volume has been constructed as a supplement and the original intention was to deal only with those subjects in which there have been significant advances during the intervening sixteen years. As the work has proceeded, however, it has become evident that there is no aspect of paediatric urology which has not been developed, and no topic which has not been illuminated by many contributions to the literature. Indeed, there has been such a copious flow of publications devoted to children's urinary tract disease that a full review is no longer possible within the compass of a volume of this size: the decision as to what should be left out has therefore been a matter of the greatest difficulty. The choice has been inevit ably arbitrary and many omissions are regretted. The attempt has been made, however, to report the most notable developments of the subject, and perhaps the greatest change in the practice of paediatric urology has been the full integra tion with paediatric nephrology. The team approach to infant disease in particular has led to greatly improved results. I have been fortunate to have Dr. T. M.

After all, the rich and famous have everything—why would they need to murder? But the rich and famous kill for the very same reasons others do: love, power, money, jealousy, greed, revenge, and rage. Here, Scott takes us on a tour of murders committed by the rich and famous during the last century, looking at the motives, the responses of the community and local law enforcement, the media, and the outcomes. She argues that the rich and famous may kill for the same reasons as others, but they receive vastly different treatment and are often able to get away with murder. Homicide by the rich and famous is not new in this country, nor is fascination with the crimes committed by our most revered citizens. But being among the upper echelon of society does afford such suspects with a greater ability to escape punishment. They have greater access to better respresentation, they have the means to flee the country, they have influential friends in high places willing to put themselves on the line, and they are generally treated better by law enforcement and the criminal justice system. This book profiles the many ways in which homicides committed by the rich and famous are similar to other murders in their motives, but differ from those committed by everyday citizens in their outcomes. Scott provides readers with a showcase of crimes that will infuriate and fascinate readers.
