Hans Pols proposes a new perspective on the history of colonial medicine from the viewpoint of indigenous physicians. The Indonesian medical profession in the Dutch East Indies actively participated in political affairs by joining and leading nationalist associations, by publishing in newspapers and magazines, and by becoming members of city councils and the colonial parliament. Indonesian physicians were motivated by their medical training, their experiences as physicians, and their subordinate position within the colonial health care system to organise, lead, and join social, cultural, and political associations. Opening with the founding of Indonesia's first political association in 1908 and continuing with the initiatives of the Association of Indonesian Physicians, Pols describes how the Rockefeller Foundation's projects inspired the formulation of a nationalist health programme. Tracing the story through the Japanese annexation, the war of independence, and independent Indonesia, Pols reveals the relationship between medicine and decolonisation, and the role of physicians in Asian history.
The book is divided into two parts: Part I deals with the relevant physics and planning algorithms of protons (H Breuer) and Part II with the radiobiology, radiopathology and clinical outcomes of proton therapy and a comparison of proton therapy versus photon therapy (BJ Smit). Protons can be used for radiosurgery and general radio therapy. Since proton therapy was first proposed in 1946 by Wilson, about sixteen facilities have been built globally. Only a very few of these have isocentric beam delivery systems so that proton therapy is really only now in a position to be compared directly by means of randomised clinical trials, with modern photon radiotherapy therapy sys tems, both for radiosurgery and for general fractionated radiotherapy. Three-dimensional proton planning computer systems with image fusion (image of computerised tomography (CT), magnetic resonance registration) capabilities imaging (MRI), stereotactic angiograms and perhaps positron emission tomography (PET) are essential for accurate proton therapy planning. New planning systems for spot scanning are under development. Many of the older comparisons of the advantageous dose distributions for protons were made with parallel opposing or multiple co-planar field arrangements, which are now largely obsolete. New comparative plans are necessary once more because of the very rapid progress in 3-D conformal planning with photons. New cost-benefit analy ses may be needed. Low energy (about 70 MeV) proton therapy is eminently suitable for the treatment of eye tumours and has firmly established itself as very useful in this regard.

Whether Christians or Jews, believers in God or atheists, the discussion today runs right across old denominations and new ideologies—but the longing for certainty is unquenched. Does God exist? We are putting all our cards on the table here. The answer will be "Yes, God exists," As human beings in the twentieth century, we certainly can reasonably believe in God—even more so in the Christian God—and perhaps even more easily today than a few decades or centuries ago. Wheat has a long history of serving as an important food crop to mankind. Especially in the Northern Hemisphere, it has been appreciated as a major source of energy through its carbohydrates, and in more recent times for its supply of valuable proteins. This combination of carbohydrates and proteins gives wheat its unique properties for making breads of different kinds of tastes. During the course of history, the quality of wheat has improved stead ily, undoubtedly for a long time by accident, and for reasons little under stood. Over the last 150 years our knowledge has increased on farming and crop husbandry, on bringing about improvements through goal-oriented plant breeding, and on milling and baking technology, leading to the standards that we enjoy today. This process will certainly continue as our knowledge of the genetic reservoir of wheat species increases. The European Cereal Atlas Foundation (ECAF) maintains the aim of in creasing and disseminating knowledge about cereal crops. Within that scope ECAF has decided to publish a book on the history of bread wheat in Europe, the development of associated bread-making technology, and the breeding of bread wheats during the twentieth century. As ECAF is a Dutch foundation, its Board is particularly pleased to have found three Dutch scientists willing to contribute to this volume. Two of them have served wheat science in the Netherlands for their entire scientific careers, spanning a period starting around 1955 and lasting for several decades of very productive wheat science development.
