This book traces the social and environmental determinants of human infectious diseases from the Neolithic to the present day. Despite recent high profile discoveries of new pathogens, the major determinants of these emerging infections are ancient and recurring. These include changing modes of subsistence, shifting populations, environmental disruptions, and social inequalities. The recent labeling of the term "re-emerging infections" reflects a re-emergence, not so much of the diseases themselves, but rather a re-emerging awareness in affluent societies of long-standing problems that were previously ignored. An Unnatural History of Emerging Infections illustrates these recurring problems and determinants through an examination of three major epidemiological transitions. The First Transition occurred with the Agricultural Revolution beginning 10,000 years ago, bringing a rise in acute infections as the main cause of human mortality. The Second Transition first began with the Industrial Revolution; it saw a decline in infectious disease mortality and an increase in chronic diseases among wealthier nations, but less so in poorer societies. These culminated in today's "worst of both worlds syndrome" in which globalization has combined with the challenges of the First and Second Transitions to produce a Third Transition, characterized by a confluence of acute and chronic disease patterns within a single global disease ecology. This accessible text is suitable for advanced undergraduate and graduate level students and researchers in the fields of epidemiology, disease ecology, anthropology, health sciences, and the history of medicine. The majority of the offending toxicants to be reviewed in this volume were devel oped to help mankind, and it is only with prolonged or widespread application that their adverse effects have been recognized. Conversely, in the case of pre scrip tion drugs, there has been an attempt to identify the adverse effects in advance and incorporate these risks into the decision of approval for human consumption. Unfortunately, for those drugs in which recognized injury occurs only after prolonged use, such appraisals are made in retrospect. Despite this, most renal injury induced by drugs or toxicants can be either prevented by excluding drugs with unacceptable side effects or interrupted by eliminating the offending agent once damage is manifested. The fact that prevention, reversibility, or arrest of renal injury is possible provided a major impetus for this publication. Since no international registry for nephrotoxic injury exists, estimates of incidence must rely on less than ideal sources. Recently I, together with Dr. William Bennett, summarized a survey of the frequency of various categories of nephrologic disease (Porter and Bennett, 1981). Based on this survey, we projected that in nearly one of ten patients seeking nephrologic consultation a nephrotoxic etiology may be involved. Of cases of end-stage renal disease, between 3 and 4% are due to drug nephrotoxicity, according to recent published results(European Dialysis and Transplant Association, 1979). For acute renal failure, antibiotics and contrast agents persist as major offending agents, while for chronic renal failure, analgesics remain a worldwide problem.

Within this text, white women philosophers critique the field of philosophy for its complicity with whiteness as a structure of power, as normative, and as hegemonic. In this way, the authority of whiteness to define what is philosophically worthy is seen as reinforcing forms of philosophical narcissism and hegemony. Challenging the whiteness of philosophy in terms of its hubristic tendencies, white women philosophers within this text assert their alliance with people of color who have been both marginalized within the field of philosophy and have had their philosophical and intellectual concerns and traditions dismissed as particularistic. Aware that feminist praxis does not necessarily lead to anti-racist praxis, the white women philosophers within this text refuse to telescope as a site of critical inquiry one site of hegemony (sexism) over another (racism). As such, the white women philosophers within this text are conscious of the ways in which they are implicated in perpetuating whiteness as a site of power within the domain of philosophy. Framed within a philosophical space that values the multiplicity of philosophical voices, and driven by a feminist framework that valorizes de-centering locations of hegemony, interdisciplinary dialogue, and transformative praxis, The Center Must Not Hold refuses to allow the white center of philosophy to masquerade as universal and given. The first six chapters present modern mathematical theory of boundary integral equations that arise in fundamental problems in continuum mechanics and electromagnetics based on the approach of variational formulations of the equations. The second six chapters present an introduction to basic classical theory of the pseudo-differential operators. The aforementioned corresponding boundary integral operators can now be recast as pseudo-differential operators. These serve as concrete examples that illustrate the basic ideas of how one may apply the theory of pseudo-differential operators and their calculus to obtain additional properties for the corresponding boundary integral operators. These two different approaches are complementary to each other. Both serve as the mathematical foundation of the boundary element methods, which have become extremely popular and efficient computational tools for boundary problems in applications. This monograph narrates the decade-long struggle of workers, unions, and management in transforming one of the largest ailing family-owned jute businesses in India, into a sustainable worker-owned and governed cooperative. It focuses on the variation in the three groups’ involvement in the transformation. It begins with the employees’ struggles in taking over the business, deserted by its owners, to save their jobs. The study analyzes the tensions between the three groups in creating and maintaining democratic governance that would sustain the initial leap in employee participation in the transformation. The analysis reveals contradictions at multiple levels, starting with the unexpected outcome of information sharing with workers: increased information sharing by management resulting in decreased employee involvement. The study explains this paradox by showing that for workers, information has a symbolic nature and information sharing is a signal of their trustworthiness in the assessment of those who are privy to the information. This means involvement is contingent upon the feeling that the information that workers consider crucial is being shared with them. However, what workers consider crucial, and thus a symbol of trust, changes over time as the nature and breadth of their involvement evolves. Thus, worker expectation as well as management and union expectation of information sharing evolves. However, the evolution has the potential to create a mismatch between the two expectations that might lead to contradictions in employee involvement. While for management, information sharing is an instrument in eliciting involvement, and thus management’s expectation of information sharing goes through an instrumental loop, for employees, information sharing is a matter of trust, and thus their expectation of information sharing goes through an institutional trust-based loop. To sustain high employee involvement, the organization should ideally institutionalize the trust-based loop and avoid engaging with the instrumental loop. The author proposes a collaborative approach to organizational transformation that will help deal with the contradictions more effectively, sustaining employee involvement in the transformation. The author also discusses the implications of these propositions for academic scholarship and organizational practices and situates them in the ongoing attempts to reform Industrial Disputes Act in India.

Overall, the book studies the processes by which physical symptoms like fever are treated as epidemics to be controlled, and are therefore brought within a biomedical system, thereby opening up options for commercialization of care.
