The riddle of melancholia has stumped generations of doctors. It is a serious depressive illness that often leads to suicide and premature death. The disease's link to biology has been intensively studied. Unlike almost any other psychiatric disorder, melancholia sufferers have abnormal endocrine functions. Tests capable of separating melancholia from other mood disorders were useful discoveries, but these tests fell into disuse as psychiatrists lost interest in biology and medicine. In the nineteenth century, theories about the role of endocrine organs encouraged endocrine treatments that loomed prominently in practice. This interest faded in the 1930s but was revived by the discovery of the adrenal hormone cortisol and descriptions of its abnormal functioning in melancholic and psychotic depressed patients. New endocrine tests were devised to plumb the secrets of mood disorders. Two colorful individuals, Bernard Carroll and Edward Sachar, led this revival and for a time in the 1960s and 1970s intensive research interest established connections between hormone dysfunctions and behavior. In the 1980s, psychiatrists lost interest in hormonal approaches largely because they did not correlate with the arbitrary classification of mood disorders. Today the relation between endocrines and behavior have been disregarded. This history traces the enthusiasm of biological efforts to solve the mystery of melancholia and their fall. Using vibrant language accessible to family care practitioners, psychiatrists and interested lay readers, the authors propose that a useful, a potentially live-saving connection between medicine and psychiatry, has been lost.

Since the publication of the first edition of The Diabetic Pancreas in 1977, much progress has been made in various areas of diabetes research. While only a rela tively short while ago diabetes was considered a single disease, in more recent years it has become apparent that it is a heterogeneous group of disorders, all of which are characterized by a decreased tolerance of carbohydrates and most of which have a genetic basis, although the genetic types vary. In more recent years, an International Work Group sponsored by the National Diabetes Data Group of the NIH proposed a now generally accepted classification, according to which the insulin-dependent ketosis-prone diabetes, formerly and inappropriately called the juvenile type, is considered a subclass of diabetes, type 1. Because it can occur at any age, it was recommended that the diagnosis based on age be eliminated. The non-insulin-dependent, non-keto sis-prone type of diabetes, which is not secondary to other diseases or conditions, and which was formerly called matu rity-onset diabetes, was considered a second subclass, type II, because although this form usually develops after age 40, it also occurs in young persons, who do not require insulin or are not ketotic. Although this classification is not entirely agreed upon by all diabetologists, for practical purposes it has been generally accepted and has been utilized by the contributors to this volume.

The Sino-Vietnamese War was the third Indochina war, and contemporary Southeast Asia cannot be properly understood unless we acknowledge that the Vietnamese fought three, not two, wars to establish their current role in the region. The war was not about the Sino-Vietnamese border, as frequently claimed, but about China’s support for its Cambodian ally, the Khmer Rouge, and the book addresses US and ASEAN involvement in the effort to support the regime. Although the Chinese completed their troop withdrawal in March 1979, they retained their strategic goal of driving Vietnam out of Cambodia at least until 1988, but it was evident by 1984-85 that the PLA, held back by the drag of its ‘Maoist’ organization, doctrine, equipment, and personnel, was not an effective instrument of coercion. Although not the only volume in this series in which lipids are discussed, the present volume is devoted entirely to methods for the study of membrane lipids. Even now, when membrane proteins are properly receiving so much attention, this emphasis on membrane lipids is appropriate. Essentially all of the phospholipids and sterols of cells are in membranes. Moreover, although membrane proteins are certainly of utmost importance, the more we learn about the functional properties of membrane proteins, the more we appreciate the unique features of phospholipids, without which biological membranes would be impossible. The hydrophobic-hydrophilic duality of phospholipids allows, indeed requires, their association, in an aqueous environment, into an essentially two-dimensional membrane-only molec ularly thick in one dimension but relatively infinite in the other two; a structure composed of small molecules, not covalently linked, and therefore, infinitely mobile and variable, but yet a structure with great stability and one largely impermeable to most biomolecules. These membrane-forming properties are shared by many amphipathic polar lipids-phospholipids, glycolipids, and sphingolipids-that differ significantly from each other in the nature of their polar head groups and their fatty acids. These variations in structure allow a range of specific interactions among membrane lipids and between lipids and proteins and also provide for membranes of variable, but controlled, fluidity. In this way, phospholipids provide an appropriate milieu for functional membrane proteins and also significantly modulate their catalytic activities.
