Selective mutism is an anxiety disorder, and its currently one of the most misunderstood, under-diagnosed, and undertreated mental health conditions. When children with selective mutism feel expected or pressured to speak in social situations, they become terrified. Therefore, their level of anxiety significantly increases, but by remaining silent, their anxiety level slightly decreases, obtaining some relief for themselves. For these children, remaining silent is actually an ineffective coping mechanism, or a maladapted solution to create a sense of safety within themselves. Hence, their mouths freeze, and they are silent. This is temporary relief, but longitudinally, these children suffer in silence. Due to the fact that many children will display normative shy behavior or even socially anxious behavior at times, it can be challenging to discern if the childs behavior falls within normal limits or if the child actually demonstrates clinically significant behavior, signifying a selective mutism diagnosis. This can be confusing for parents, teachers, and even clinicians. This manual is meant to help clarify this distinction and will also assist you in figuring out how to seek help, ask the right questions, and what you can expect from early intervention services, school-based services, and holistic, therapeutic, and psychiatric services.

Our aims are to provide both the student and practicing profes sional with a collection of actual neuropsychological case studies that typify many of the cardinal disorders or syndromes frequently seen by practicing clini cal neuropsychologists. We have not provided an exhaustive survey of case studies representing many of the disorders commonly seen in neuropsychology; rather, we have focused on a few select cases that illustrate some of the more common disorders. This book includes detailed case reports complete with referral questions, historical information, relevant neuroradiological findings, actual neuropsycho logical test data, and clinical interpretations that take into account all available information on the patient. We had our first conversation about cognition, metacognition, and reading in September of 1976. Our particular concern was with reading and learning to read, and what, if anything, meta cognition might have to do with it all. We didn't really know much about metacognition then, of course, but then most other people were in the same predicament. Some people had been working with interesting approaches and results on metalanguage and reading, among them J. Downing, L. Ehri, L. Gleitman, 1. Mattingly, and E. Ryan, and it also was about that time that people were becoming aware of E. Markman's first studies of comprehension monitoring. Other than that perhaps the most influential item around was the perhaps already "classic" monograph by Kruetzer, Leonard, and Flavell on what children know about their own memory. Also in the air at that time were things like A. Brown's notions about "knowing, knowing about know ing, and knowing how to know," D. Meichenbaum's ideas about cognitive behavior modification, and the work by A. Brown and S. Smiley on the awareness of important units in text. Even though these developments were cited as new and innovative, it was not the case that psychologists had never before been of questions. They certainly interested in, or concerned with metacognitive sorts had, as clearly evidenced by the notion of "metaplans", in Miller, Galanter, and Pribram's Plans and the Structure of Behavior.
