How had my mother and father became ensnared in a storm and sun relationship, a "jackals wedding," with my sister and me dragged along unwittingly? How had the exigencies of wartime prevented them from dealing with their own blow-ups? What was the big taboo within our family? Why so many secrets locked away, as my mothers heart had seemed locked to me? I was hungry to know the truth, so I began to dig down to the beginning through my first memories that are entwined with the unrest of the times. With a stroke of serendipity, my husband convinced me I must return to India, the land of my birth, and my childhood home in Dehra Dun. During this trip together, time spans were erased. People stepped forward to help. Images and voices and feelings came flooding back, and I was ready to examine them as Id examined the belongings that had traveled half the world in battered leather cases. Brought to light, the joys of my childhood flashed vivid and fragile as glass bangles. Fears that had lurked large as nightmare lions and scary as snakes dissolved like thunderheads shrinking and fading into a quiet sky. In Jackals Wedding, the stories of the child I was and the woman Ive become are braided with my mothers story and the stories she told. Many times during the writing, it seemed she was back, whispering in my ear what she wasnt able to tell, in life. I am still searching for my father.

Migraine is an extremely common condition affecting 28 million Americans. Fully 1 in every 6 adult women suffers from this malady. Patients with migraine are frequently seen in the Emergency Room and Urgent Care Clinic for acute treatment of their headaches. Headache generally accounts for nearly 3 % of all ER visits, resulting in almost 3.5 million visits annually. There currently are no widely used guidelines for the treatment of patients with headache in the ER or UCC, and reducing the number of CT scans remains a key goal for administrators in many ER or urgent care settings. In one study, 35 drugs, alone or in combination, were used to treat migraine. The most common medication class used was narcotics, which were used in 25% of the visits. The next two most commonly used classes were antiemetics and NSAIDs. Only 5% of patients seen for headache received headache-specific medications during their visit. ER and Urgent Care physicians generally have little or no formal training in treating these patients. Consequently, they often feel less comfortable ruling out secondary causes of headaches and using appropriate headache treatment medications and procedures. Since many patients have received narcotics in previous visits, providers may view them as drug seekers. Patients seen in ERs and UCCs for acute treatment of their headaches often report high levels of dissatisfaction. They often end up in the ER or UCC because they don’t have a formal treatment strategy for their severe headaches. Health plans and insurance agencies are often frustrated by the high cost of fragmented care for patients with headaches. Because of the environment that most ER and UCC providers operate in, many unnecessary scans and tests are done because of uncertainty of the diagnosis and unfamiliarity of the patient. Primary care physicians are also frustrated by patients with headaches presenting to them after being seen in the ER or UCC, wanting a refill of their narcotics that they were given. Many patients don’t follow up with their PCPs and simply keep returning to the ER/UCC for their treatment. In short, all involved in the care for patients with headache are frustrated by the current system. The care for these patients is clearly not optimal. It doesn’t have to be that way. There are a wide variety of effective treatment options available, but are underutilized in the acute setting. With these treatment options, patients no longer have to be treated only with parenteral narcotics, only to perpetuate the cycle of suboptimal care described above. This concise handbook covers all aspects of acute headache care, including care of the child and adolescent with acute headaches, treatment of pregnant and breastfeeding women with acute headaches and appropriate evaluation of secondary headaches. In addition, two other novel chapters are included: one on caring for older patients with acute headaches as well as a chapter describing how to ensure a seamless transition of the patient back to a headache interested provider. Representing an important milestone in the care of patients with headache, this is the first concise handbook available to exclusively address the issue of headache treatment in the acute care setting. Including protocols and strategies that can be used right away, Management of Headaches in Emergency Room and Urgent Care Settings: Diagnosis and Management provides information about lesser known, but effective strategies such as greater occipital nerve blocks that can be easily learned and incorporated in the acute care setting. It emphasizes the continuity of care that is so vital to keep headache patients from returning to the ER and UCC for acute treatment.

Then she faces the failures of the Australian Defence Force and the Department of Veterans' Affairs. To the Australian people these two systems appear to be functioning at an optimum for the serving members of the ADF, veterans' and their families respectively, but it only appears that they are. Lorna also gives accounts of how the leadership within the Christian community believes it is above the law of the land, and how the judicial system aides the church's betrayal of trust. In the end, Lorna's resilience and strength wins through to overcome the impossible, leading her to write this inspiring book.

Synopsis: Gary Smalley, a widower and inventor with two young daughters is worried about Ginger, his 6-year-old.She’s had a make-believe friend for a while and bugs Gary to get a dog because Buttercup, the cat, isn’t any fun. Clem, Ginger’s older sister, is too wrapped up in sports to pay much attention to Ginger. Diane, Gary’s best friend and secret admirer, has a government contract to train dogs for espionage. A special blue formula makes the dogs invisible. The Chief sends special agents Mr. Water and Mr. Blue, to the training facility for an update. The government guys are all agog over the Star-Warsey vault that contains the training course and the HAL 2000 Dog Monitor computer – Gary’s creations. While performing a live test downtown, Seventy-Six, a Jack Russell Terrier and Diane’s star pupil, becomes partially visible. Everyone panics: glitches are traceable. Even with the flaw, Seventy-Six completes the mission undetected. Mr. Blue tells Diane the mission is a success and for her to hand over Seventy-Six; he’s needed right away in the government’s war against terrorists. Diane is gobsmacked by this bit of news. She trained the dogs for espionage. HAL instructs Seventy-Six to run. As Gary leaves the patent office, Seventy-Six jumps in the back of his SUV and discovers Ginger’s missing doll. Seventy-Six, invisible, jumps on Ginger. She screams up a storm as she's pushed down and licked. When she discovers that he’s a real invisible dog, she’s thrilled. She names him Ralphie. Gary and Clem thinks Ginger’s new dog is make believe. Mr. Blue and Mr. Water search for Ralphie while Diane is distraught at losing her beloved dog and possibly her government contract. Gary and Clem finally realize that Ralphie is a real dog, and one of Gary’s inventions allows them to see him, if only briefly. Diane asks Gary to help her devise a plan to keep him safe from the government agents. Mr. Water and Mr. try to capture Ralphie. They break into the Smalley home at night, but he keeps alluding them. Ralphie sneaks out one night to scare the agents away; Ginger sneaks out after him. At the crack of dawn, Gary, Diane and Clem discover Ginger and Ralphie missing -- possibly kidnapped and dognapped. They set out to find and rescue them with the aid of Gary’s invention. It’s a happy ending for all. The Chief retires Ralphie and Ginger gets to keep him. Diane is awarded the full contract for her espionage program. Mr. Water shows up with a government contract for two of Gary’s inventions.

Why does political decentralization seem reduce intrastate conflict more in some countries than in others? This question constitutes the central focus of Peace by Design. Brancati argues that the ability of decentralization to reduce intrastate conflict hinges on the electoral strength of regional parties. According to Brancati, regional parties tend to promote intrastate conflict by creating regional identities, advocating legislation harmful to other regions and regional minorities, and mobilizing groups to engage in conflict or supporting extremist organizations that do. Brancati also highlights a number of conditions under which regional parties are more likely to promote conflict, such as democratic transitions. Brancati further argues that decentralization increases the strength of regional parties depending on particular features of decentralization (i.e., the proportion of legislative seats a region possesses, the number of regional legislatures in a country, the upper house election procedures, the sequencing of national and regional elections). These features of decentralization vary across countries and are fundamental to explaining why decentralization is not effective in reducing conflict in all countries. Brancati's ultimate conclusion is that decentralization can be effectively designed to promote peace, as long as it is designed to encourage statewide parties to incorporate regional parties into their agendas and to limit the strength of regional parties. The author provides compelling evidence for her argument through three detailed cases studies (e.g., Czechoslovakia, Spain, and India) and a rigorous quantitative analysis in which she introduces a new dataset on constituency-level elections that will prove an invaluable resource for many future studies.
