In 'Critical Care Ethics', David F. Kelly describes and analyzes some of the major ethical issues facing American hospitals today and proposes practical approaches by which hospitals may ethically and legally resolve them. The author includes both theory and application in a way which insures accessibility to those without a background in moral philosophy or moral theology. The book's major emphasis is on the resolution within the American hospital of the practical question of forgoing treatment. Its central thesis is that a consensus has arisen in American medical ethics concerning the ethical and legal rightness of forgoing medical treatment in some circumstances. The present consensus is based on three pillars. The first two of these are ethical principles and distinctions arising from Catholic medical ethics, which have been generally accepted in American hospital ethics and American law. This open access book presents a methodology for the assessment of structural building details, taking into account the contemporary guidelines for earthquake-resistant and energy-efficient buildings. A review of structural details for energy-efficient buildings revealed that in some cases the structural system is interrupted, leading to solutions which are not suitable for earthquake-prone regions. Such typical examples would be the use of thermal insulation under the building foundation and reduction of the load-bearing elements’ dimensions – also at the potential locations of plastic hinges which are crucial for the dissipation of seismic energy. The proposed methodology of assessment favours a collaboration of architects, engineers, contractors and investors in the early stage of building design. By this the methodology enables efficient decision-making and contributes to a selection of optimal building structural details. This book covers the entire field of research in the area of minor tranquillizers and its application to current clinical practice in the treatment of anxiety and insomnia. These drugs are principally the benzodiazepines and related drugs with a similar mechanism of action, such as zolpidem and zopiclone. The molecular mechanism of action of benzodiazepines is described, focussing on the interaction of these drugs with the different isoform of the GABAA^O >receptor, and the consequences of this for brain function. Recent advances in this knowledge have provided a framework for defining the physiochemical nature of the interaction between such drugs and their receptor protein, and thus pave the way for the design of new anxiolytic and hypnotic drugs. The animal models available for evaluating the potential of such new therapeutic agents in the treatment of anxiety and insomnia are discussed. Furthermore, understanding of the physiological regulation of the GABAA receptor may provide insights into the aetiopathology of these diseases. The clinical use of benzodiazepines and related drugs in the treatment of anxiety, insomnia, epilepsy and as anaesthetics are explored. The advantages and limitations of such treatments are discussed, and the impact of drugs evaluated. A detailed history of the Confederate retreat after the Battle of Gettysburg and the Union effort to destroy the enemy during the American Civil War. The three-day Battle of Gettysburg left 50,000 casualties in its wake, a battered Southern army far from its base of supplies, and a rich historiographic legacy. Thousands of books and articles cover nearly every aspect of the battle, but One Continuous Fight is the first detailed military history of Lee’s retreat and the Union effort to destroy the wounded Army of Northern Virginia. Against steep odds and encumbered with thousands of casualties, Confederate commander Robert E. Lee’s post-battle task was to successfully withdraw his army across the Potomac River. Union commander George G. Meade’s equally difficult assignment was to intercept the effort and destroy his enemy. The responsibility for defending the exposed Southern columns belonged to cavalry chieftain James Ewell Brown (Jeb) Stuart. If Stuart fumbled his famous ride north to Gettysburg, his generalship during the retreat more than redeemed his flagging reputation. The long retreat triggered nearly two dozen skirmishes and major engagements, including fighting at Granite Hill, Monterey Pass, Hagerstown, Williamsport, Funkstown, Boonsboro, and Falling Waters. President Abraham Lincoln was thankful for the early July battlefield victory, but disappointed that General Meade was unable to surround and crush the Confederates before they found safety on the far side of the Potomac. Exactly what Meade did to try to intercept the fleeing Confederates, and how the Southerners managed to defend their army and ponderous 17-mile long wagon train of wounded until crossing into western Virginia on the early morning of July 14, is the subject of this study.
