Family Change and Decline in Modern Societies

Disturbing the Nest assesses the future of the family as an institution through an historical and comparative analysis of the nature, causes, and social implications of family change in advanced western societies such as the United States, New Zealand, and Switzerland by focusing on the one society in which family decline is found to be the greatest, Sweden. The founding of the modern Swedish welfare state was based in large part on the belief that it was necessary for the state to intervene in society in order to improve the situation of the family. Of great concern was the low birthrate, which was seen as a threat to the very survival of Swedes as a national population group. The Social Democrats pioneered welfare measures that aimed to strengthen the family, to alleviate its worst trials and tribulations, and to make possible harmonious living. With the Social Democrats remaining in power continuously until 1976, a period of almost forty-five years, Sweden went on to implement governmental "family policies" that are among the most comprehensive (and expensive) in the world. In view of this major policy goal of family improvement, the actual situation of the Swedish family today presents a genuine irony; some have claimed that Swedish welfare state policies have had consequences that are the opposite of those originally intended. Comparing contemporary Swedish family patterns with those of other advanced nations, one finds a very high family dissolution rate, probably the highest in the Western world, and a high percentage of single-parent, female headed families. Even marriage seems to have fallen increasingly out of favor, with Sweden having the lowest marriage rate and latest age of first marriage, and the highest rate of children born out-of-wedlock. The early pronatalist aspirations of the Swedish government have been spectacularly unsuccessful, as Sweden continues to have one of the world's lowest birthrates and smallest average family sizes.

Our ability to treat common bacterial infections with antibiotics goes back only 65 years. However, the authors of this report make it clear that sustaining a supply of effective and affordable antibiotics cannot be without changes to the incentives facing patients, physicians, hospitals, insurers, and pharmaceutical manufacturers. In fact, increasing resistance to these drugs is already exacting a terrible price. Every day in the United States, approximately 172 men, women, and children die from infections caused by antibiotic-resistant bacteria in hospitals alone. Beyond those deaths, antibiotic resistance is costing billions of dollars through prolonged hospital stays and the need for doctors to resort to ever more costly drugs to use as substitute treatments. Extending the Cure presents the problem of antibiotic resistance as a conflict between individual decision makers and their short-term interest and the interest of society as a whole, in both present and future: The effort that doctors make to please each patient by prescribing a drug when it might not be properly indicated, poor monitoring of discharged patients to ensure that they do not transmit drug-resistant pathogens to other persons, excesses in the marketing of new antibiotics, and the broad overuse of antibiotics all contribute to the development and spread of antibiotic-resistant bacteria. The book explores a range of policy options that would encourage patients, health care providers, and managed care organizations to serve as more responsible stewards of existing antibiotics as well as proposals that would give pharmaceutical firms greater incentives to develop new antibiotics and avoid overselling. If the problem continues unaddressed, antibiotic resistance has the potential to derail the health care system and return us to a world where people of all ages routinely die from simple infections. As a basis for future research and a spur to a critically important dialogue, Extending the Cure is a fundamental first step in addressing this public health crisis. In the wake of the black civil rights movement, other disadvantaged groups of Americans began to make headway--Latinos, women, Asian Americans, and the disabled found themselves the beneficiaries of new laws and policies--and by the early 1970s a minority rights revolution was well underway. In the first book to take a broad perspective on this wide-ranging and far-reaching phenomenon, John D. Skrentny exposes the connections between the diverse actions and circumstances that contributed to this revolution--and that forever changed the face of American politics. Though protest and lobbying played a role in bringing about new laws and regulations--touching everything from wheelchair access to women's athletics to bilingual education--what Skrentny describes was not primarily a bottom-up story of radical confrontation. Rather, elites often led the way, and some of the most prominent advocates for expanding civil rights were the conservative Republicans who later emerged as these policies' most vociferous opponents. This book traces the minority rights revolution back to its roots not only in the black civil rights movement but in the aftermath of World War II, in which a world consensus on equal rights emerged from the Allies' triumph over the oppressive regimes of Nazi Germany and Imperial Japan, and then the Soviet Union. It also contrasts failed minority rights development for white ethnics and gays/lesbians with groups the government successfully categorized with African Americans. Investigating these links, Skrentny is able to present the world as America's leaders saw it; and so, to show how and why familiar figures--such as Lyndon Johnson, Richard Nixon, and, remarkably enough, conservatives like Senator Barry Goldwater and Robert Bork--created and advanced policies that have made the country more egalitarian but left it perhaps as divided as ever.

This book tells the story of Armenian migration to North America in the late Ottoman period, and Istanbul's efforts to prevent it. It shows how, just as in the present, migrants in the late 19th and early 20th centuries were forced to travel through clandestine smuggling networks, frustrating the enforcement of the ban on migration. Further, migrants who attempted to return home from sojourns in North America risked debarment at the border and deportation, while the return of migrants who had naturalized as US citizens generated friction between the United States and Ottoman governments. The author sheds light on the relationship between the imperial state and its Armenian populations in the decades leading up to the Armenian genocide. He also places the Ottoman Empire squarely in the middle of global debates on migration, border control and restriction in this period, adding to our understanding of the global historical origins of contemporary immigration politics and other issues of relevance today in the Middle East region, such borders and frontiers, migrants and refugees, and ethno-religious minorities.
