In this book, public health ethicist Daniel S. Goldberg sets out to characterize the subjective experience of pain and its undertreatment within the US medical establishment, and puts forward public policy recommendations for ameliorating the undertreatment of pain. The book begins from the position that the overwhelming focus on opioid analgesics as a means for improving the undertreatment of pain is flawed, and argues instead that dominant Western models of biomedicine and objectivity delegitimize subjective knowledge of the body and pain in the US. This general intolerance for the subjectivity of pain is part of a specific American culture of pain in which a variety of actors take part, including not only physicians and health care providers, but also pain sufferers, caregivers, and policymakers. Concentrating primarily on bioethics, history, and public policy, the book brings a truly interdisciplinary approach to an urgent practical ethical problem. Taking up the practical challenge, the book culminates in a series of policy recommendations that provide pathways for moral agents to move beyond contests over drug policy to policy arenas that, based on the evidence, hold more promise in their capacity to address the devastating and inequitable undertreatment of pain in the US.
This progressive resource places concepts of social determinants of health in the larger contexts of contemporary health ethics and the evolution of social reform. It provides needed analysis of the larger causes behind the immediate causes of illness and epidemics, particularly injustice, systemic inequities, and the cumulative effect of compound disadvantages. This moral approach to collective and individual responsibilities—on the part of practitioners as well as the public—supports a sound blueprint for finding answers to longstanding global and local concerns. Readers are challenged to recognize the critical role of social determinants to their perception of health issues, controversies, and possibilities as the book: · Details the epidemiologic evidence regarding social determinants of health. · Key ethical implications of the evidence regarding social determinants of health. · Considers the role of risky health behaviors in determining population health outcomes. · Addresses ethical questions of priority-setting at the policy and practice levels. · Translates social determinants of health into health policy goals. Half textbook, half monograph, Public Health Ethics and the Social Determinants of Health Is geared toward students in MPH programs as well as public health professionals in diverse contexts such as local health departments and non-profit organizations. It informs public health scientists and scholars, and can also serve as an introductory text for students in public health ethics, or as part of a general applied ethics course.
A timely look at the ethical, legal, and policy issues surrounding brain injury and collision sports. American tackle football is an industry like any other. And like many industries, it sells a product that is dangerous to those who use it—or, in this case, those who play it. In Tackle Football and Traumatic Brain Injuries, Daniel S. Goldberg explores the connections among traumatic brain injury, collision sports, and the industry's continuing efforts to manufacture doubt. Focusing especially on youths and adolescents—the most vulnerable population that comprises over 99% of tackle football players in the US—Goldberg addresses the ethical and social implications of their participation in tackle football. Goldberg discusses the true scope of the danger and the costs to society and individuals of caring for injured participants. If these risks were to become widespread public knowledge, the profitability and perhaps even the viability of American football would be at risk. As the tackle football industry has consistently worked to mask the health hazards involved in playing football, it has used a particular tool that has proved highly effective in achieving this subterfuge: the manufacture of doubt. Goldberg advocates for using public health laws as a tool for countering these efforts at obfuscation, and he outlines specific policy proposals intended to address the population health and ethical problems presented by tackle football. The book draws on public health ethics, public health law, and the histories of occupational and public health to assess the limits of parental choice to expose their children to risks of injury. Should kids play tackle football at all—and who decides if they should? Goldberg offers practical answers to these critical legal, ethical, and social questions. Chris Nowinski, former Harvard football player and WWE wrestler, provides a timely and insider's perspective on these critical issues in the foreword.
The Death of the Income Tax explains how the current income tax is needlessly complex, contains perverse incentives against saving and investment, fails to use modern technology to ease compliance and collection burdens, and is subject to micromanaging and mismanaging by Congress. Daniel Goldberg proposes that the solution to the problems of the current income tax is completely replacing it with a progressive consumption tax collected electronically at the point of sale.
We examine the stability and strength of the relationship between exchange rates and trade over time using three alternative approaches, mitigating the endogeneity of the relation. We find that both exchange rate pass-through and the price elasticity of trade volumes are largely stable over time. Economic slack and financial conditions affect the relationship, but there is limited evidence that participation in global value chains has significantly changed the exchange rate–trade relationship over time.

This book defines and examines the counterpart of Schur functions and Schur analysis in the slice hyperholomorphic setting. It is organized into three parts: the first introduces readers to classical Schur analysis, while the second offers background material on quaternions, slice hyperholomorphic functions, and quaternionic functional analysis. The third part represents the core of the book and explores quaternionic Schur analysis and its various applications. The book includes previously unpublished results and provides the basis for new directions of research.

Social skills training (SST) continues to be a widely accepted and recommended intervention for improving the psychosocial functioning of persons with schizophrenia and other serious mental illnesses. The book begins by providing useful background information, including the nature and importance of social skills (Chapter 1), updated evidence supporting the effectiveness of SST (Chapter 2), and assessment and goal-setting for SST (Chapter 3). The next group of chapters addresses the practicalities of providing SST groups and are also similar to the second edition, including methods for teaching social skills (Chapter 4), starting an SST group (Chapter 5), choosing curricula for an SST group (Chapter 6), tailoring SST to meet individual participant needs (Chapter 7), and solutions to common challenges encountered when providing SST (Chapter 8). The third edition of this book differs from the second edition in the variety of special topics related to SST that are addressed. While the second edition had only one such chapter on providing SST to individuals with comorbid substance use problems, in addition to retaining (and updating) this chapter (Chapter 9), the third edition also has six additional chapters on special topics, including younger individuals who are either at risk for psychosis or recovering from a first episode of psychosis (Chapter 10), technology-based communication skills (Chapter 11), SST with older individuals (Chapter 12), providing SST in residential or inpatient settings (Chapter 13), cultural factors when providing SST (Chapter 14), and gender and sexual identity issues and sexual harassment (Chapter 15)"--
This book integrates insights from dialogic theory and systemic functional linguistics (SFL) to extend our understandings of engagement in medical research articles, going beyond notions of the role of verbal dialogue to encompass mathematical and visual semiotics and consider text not just as language but as multisemiosis. The volume begins by outlining the engagement framework and offering a brief overview of historical developments in medical research discourse. This discussion culminates in the introduction of the corpus used for analysis, drawing on original research articles from key medical journals to explore verbal, mathematical, and visual engagement in turn. A subsequent chapter brings these perspectives together to demonstrate intersemiotic engagement across different stages and phases of the medical research article and how such resources work together to construe and maintain the authoritative position commonly associated with medical discourse. The book looks ahead to engagement in other related disciplinary fields and future directions for work on multisemiosis and medical research discourse more generally. This book will be of particular interest to graduate students and researchers in multimodality, critical discourse analysis, applied linguistics, SFL, and science education.
This Volume contains the papers presented by twenty-eight invited speakers at the symposium entitled, "Genetic Manipulation of Woody Plants," held at Michigan State University, East Lansing, Michigan, from June 21-25, 1987. Also included are abstracts of contributed poster papers presented during the meeting. That the molecular biology of woody plants is a rapidly expanding field is attested to by the large attendance and high level of enthusiasm generated at the conference. Leading scientists from throughout the world discussed challenging problems and presented new insights into the devel opment of in vitro culture systems, techniques for DNA analysis and manipulation, gene vector systems, and experimental systems that will lead to a clearer understanding of gene expression and regulation for woody plant species. The presence at the conference of both invited speakers and other scientists who work with nonwoody plant species also added depth to the discussions and applicability of the information presented at the conference. The editors want to commend the speakers for their well-organized and informative talks, and feel particularly indebted to the late Dr. Alexander Hollaender and others on the planning committee who assist ed in the selection of the invited speakers. The committee consisted of David Burger (University of California, Davis), Don J. Durzan (University of California, Davis) , Bruce Haissig (U. S. Department of Agriculture Forest Service), Stanley Krugman (U. S. Department of Agriculture Forest Service) , Ralph Mott (North Carolina State University), Otto Schwarz (Univer.sity of Tennessee, Knoxville), and Roger Timmis (Weyerhaeuser Company).

This book describes the major achievements and discoveries relevant to bacterial protein toxins since the turn of the new century illustrated by the discovery of more than fifty novel toxins (many of them identified through genome screening). The establishment of the three-dimensional crystal structure of more than 20 toxins during the same period offers deeper knowledge of structure-activity relationships and provides a framework to understand how toxins recognize receptors, penetrate membranes and interact with and modify intracellular substrates. Edited by two of the most highly regarded experts in the field from the Institut Pasteur, France 14 brand new chapters dedicated to coverage of historical and general aspects of toxinology Includes the major toxins of both basic and clinical interest are described in depth Details applied aspects of toxins such as therapy, vaccinology, and toolkits in cell biology Evolutionary and functional aspects of bacterial toxins evaluated and summarized Toxin applications in cell biology presented Therapy (cancer therapy, dystonias) discussed Vaccines (native and genetically engineered vaccines) featured Toxins discussed as biological weapons, comprising chapters on anthrax, diphtheria, ricin etc.
This second edition presents a collection of exercises on the theory of analytic functions, including completed and detailed solutions. It introduces students to various applications and aspects of the theory of analytic functions not always touched on in a first course, while also addressing topics of interest to electrical engineering students (e.g., the realization of rational functions and its connections to the theory of linear systems and state space representations of such systems). It provides examples of important Hilbert spaces of analytic functions (in particular the Hardy space and the Fock space), and also includes a section reviewing essential aspects of topology, functional analysis and Lebesgue integration. Benefits of the 2nd edition Rational functions are now covered in a separate chapter. Further, the section on conformal mappings has been expanded.
Born out of the excitement of a convergence of ideas and passions, this book provides a synthesis of the work of researchers, clinicians, and theoreticians who are leaders in the field of trauma, attachment, and psychotherapy. As we move into the third millennium, the field of mental health is in an exciting position to bring together diverse ideas from a range of disciplines that illuminate our understanding of human experience: neurobiology, developmental psychology, traumatology, and systems theory. The contributors emphasize the ways in which the social environment, including relationships of childhood, adulthood, and the treatment milieu change aspects of the structure of the brain and ultimately alter the mind.

It's 2012 and the unprecedented has happened. Fifty-two year old Ron Goldberg, a liberal U.S. Senator from Massachusetts, has been elected the first Jewish President of the United States. Proclaimed "the Jewish Kennedy", his kingdom "the Kosher Camelot", he still is not universally embraced by America. Or the world. Blackmail, dirty tricks and charges of nepotism quickly threaten to sabotage his presidency. Anti-Semitism resurges. Death threats abound. Old flames are fanned. Bacon is eschewed. Camels are bombed. Furniture is rearranged. A dead father speaks. All the while, Goldberg soldiers on, determined to make his mark by unveiling the centerpiece of his agenda--an ambitious world disarmament plan that could either work or end all life on Earth in a nanosecond. The stormy odyssey of an American Jew through the shark-infested waters of national and international politics, Roll Over, Hitler! is a comic novel with a razor sharp edge.
