The United States’ health care system stands out for its strict division of policies dealing with public health and individual medicine. Seeking to explain how this division came to be, what alternative paths might have been taken, and how this shapes the contemporary landscape, Daniel Sledge offers nothing less than a reinterpretation of the making of modern American health policy in Health Divided. Where previous scholars have focused on failed attempts to adopt national health insurance, Sledge demonstrates that the development of health policy cannot be properly understood without considering the connections between public health policy and policies dealing with individual medicine. His work shows how the distinct politics of the formative years of health policy—and the presence of debilitating diseases in the American South—led to outcomes that have fundamentally shaped modern policies and disputes. Until the end of the nineteenth century, health care in the United States was seen as a local issue, with the sole exception being the government’s role in providing care to seamen and immigrants. Then, as Health Divided reveals, the health problems that plagued the American South in the early twentieth century, from malaria to hookworm and pellagra, along with the political power of the southern Democrats during the New Deal, fueled the emergence of national intervention in public health work. At the same time, divisions among policymakers, as well as the resistance of the American Medical Association, led to federal inaction in the realm of individual medical services—setting the stage for the growth of employer-sponsored health insurance. The vision of those who built the institutions that became the Centers for Disease Control and Prevention was, we see here, far more expansive and innovative than has previously been realized—and it came surprisingly close to succeeding. Exploring the history behind its failure, and tracing the inextricable links between public health and national health policy, this book provides a valuable new perspective on the origins of America’s disjointed health care system.

In 1900 the manufacture of rubber products in the United States was concentrated in several hundred small plants around New York and Boston that employed low-paid immigrant workers with no intervention from unions. By the mid-1930s, thanks to the automobile and the Depression, production was concentrated in Ohio, the labor force was largely native born and highly paid, and labor organizations had a decisive influence on the industry. Daniel Nelson tells the story of these changes as a case study of union growth against a background of critical developments in twentieth-century economic life. The author emphasizes the years after 1910, when a crucial distinction arose between big, mass-production rubber producers and those that were smaller and more labor intensive. In the 1930s mass-production workers took the lead in organizing the labor movement, and they dominated the international union, the United Rubber Workers, until the end of the decade. Professor Nelson discusses not only labor's triumph over adversity but also the problems that occurred with union victories: the flight of the industry to low-wage communities in the South and Midwest, internal tensions in the union, and rivalry with the American Federation of Labor. The experiences of the URW in the late 1930s foreshadowed the longer-term challenges that the labor movement has faced in recent decades. Originally published in 1988. The digital age we now live in is fundamentally changing how we relate to our perceptions and images. Daniel O'Shiel provides the first comprehensive phenomenology of virtual technology in order to show how the previously well-established experiential lines and structures between three basic categories of phenomenal experience – our everyday perceptions of reality; our everyday fantasies of irreality; and our everyday engagements with external images, not least digital ones – are becoming blurred, inverted or are even collapsing in a new era where a specific type of virtuality is coming to the fore. O'Shiel examines in depth just what this means for the phenomenology behind it, as well as the concrete practical consequences going forward. The work is divided into two main parts. In the first O'Shiel fully investigates the phenomenological natures of perception and imagination through close textual analyses of the relevant works by Edmund Husserl, Eugen Fink and Jean-Paul Sartre. In each phenomenologist perception and imagination are ultimately seen as different in kind, although the dividing line differs, especially with reference to a middle category of 'image-consciousness' (Bildbewusstsein). This first part argues for basic phenomenological differences between perceptions; physical and external images; and more mental imagery, while also allowing for a more general gradation between them. The second part then applies these theoretical findings to some of the most influential 'virtual technologies' today – social media; online gaming; and some virtual, augmented and mixed reality technologies – in order to show how previously clear categories of real and irreal, present and absent, genuine and fake, and even true and false, are becoming less so.
