Liberation from stigma is the first step in freeing from addiction. Historical and clinical scientific considerations of alcohol, cocaine and stimulants are reviewed. Substance use disorder is a legitimate medical disorder with its locus in the pleasure centers of the brain. People who have addictions frequently also have medical and psychiatric comorbidities that complicate their addictions. With perseverance, all of these challenging disorders can be prevented and treated. Prevention can be classified as primary, secondary, and tertiary. The sine qua non of treatment and prevention is the biopsychosocial model. Classification is crucial in science. As an example, the DSM-5 publication has been an important scientific achievement. In it, Substance- Related and Addictive Disorders can be diagnosed using the four Cs: craving, control (loss of), compulsion, and use despite negative consequences. Thanks to pharmacogenetics and epigenetics in the future, the high risk for different addictions can be clarified. Tragic cases of celebrities can be changed, and their triumphs celebrated instead of their deaths mourned from substance use disorders. Examples of tragic cases in the past are Ernest Hemingway, whose family also suffered five suicides (including Hemingway himself), and Eugene ONeill with three suicides in his family. On the other hand, Bob and Bill W., Betty Ford, and Robert Downey Jr. and Drew Barrymore all succeeded in their fights against addiction. As far as I know, Hemingway and ONeil were never told they had an alcohol use disorder and were never referred to AA. Although it is speculation, if both authors and their families were treated with lithium (used for mania first in Australia in 1949 and approved for the treatment of mood disorders in the United States in 1972) or, in the future, ketamine, their suicides could have been prevented. Bob Smith and Bill Watson, who experienced a spiritual transformation experience, were able to abstain from alcohol for the rest of their lives. Evidence-based studies can be used along with FDA-approved addiction medication as part of the biopsychosocial model. For alcohol use disorders, three medications are recommended: disulfiram, acamprosate, and naltrexone. The key is determining which medication is indicated for a specific patient. For cocaine and stimulant use disorders, FDA-approved medications for the treatment of ADHD are an alternative for patients trying to self-medicate with cocaine. For opiate use disorders, three other medications are also FDA-approved: naltrexone, methadone and buprenorphine. I favor buprenorphine due to the fact that is a partial mu blocker (the mu receptor is the most important analgesic opiate receptor)the risk of respiratory depression in cases of overdosing is low, and the success rate is high. Buprenorphine can be paired with naloxone, which is an opiate blocker, to prevent the illegal distribution. Kits with naloxone, Evzio (brand name of naloxone 0.4-milligram auto-injector), are being distributed to patients and family members, making it a life-saving medication similar to EpiPen for the treatment of anaphylactic shock. In order to diminish the risk of diversion, many deterrent techniques are being developed by manufactures of opiates. All these precautions will be almost superfluous if an implantable version of buprenorphine becomes available. Effective and updated medical education is the best antidote against stigma. Above all, all patients with substance use disorders should be treated with respect and humane care.
It was mid-December 1610 in Mexico City. The Church was in its preparatory season of Advent, leading up to the celebration of Christ's birth at Christmas. The nuns of the Encarnacion convent had just celebrated the feast of the Immaculate Conception of the Virgin Mary, on 8 December. But now, in this time usually filled with joy, some of the nuns were nervous. Their choirbooks were missing. Without them, the nuns would not be able to celebrate the anniversary of Christ's birth adequately. A musician priest of the metropolitan cathedral, located just three blocks from the convent, had caused the nuns' alarm: Antonio Rodríguez Mata (d. 1643) had all five of the missing books. He had borrowed them from Sister Flor de Santa Clara, the convent "vicaria de coro" (choir vicar) but had failed to return them despite the convent's repeated requests. The diocesan vicar general and the attorney general were summoned. The nuns of the Encarnación demanded that Mata be imprisoned if he failed to return the books immediately following the denunciation. The threat of jail time was serious, but so too was the alleged offense: Mata was impeding the nuns from performing their liturgical music for Christmas"--

Ana Cristina Cesar (1952-1983) has posthumously become one of Brazil’s best known avant-garde poets. After her suicide in 1983, her innovative, mythic, and dreamlike poetry has greatly influenced subsequent generations of writers. At Your Feet was originally published as a poetic sequence and later became part of a longer hybrid work— sometimes prose, sometimes verse—documenting the life and mind of a forcefully active literary woman. Cesar, who also worked internationally as a journalist and translator, often found inspiration in the writings of other poets, among them Emily Dickinson, Armando Freitas Filho, and Gertrude Stein. Her innovative writing has been featured in Sun and Moon’s classic anthology Nothing the Sun Could Not Explain—20 Contemporary Brazilian Poets (2000). Poet Brenda Hillman and her mother Helen Hillman (a native speaker of Portuguese) worked with Brazilian poet Sebastião Edson Macedo and translator/editor Katrina Dodson to render as faithfully as possible the intricately layered poems of this legendary writer. The mediastinum is a hollow space within the thoracic cavity that may give rise to a wide spectrum of tumoral conditions from the benign to the intermediate to the highly malignant neoplasms. In addition, other inflammatory and infectious conditions may also arise in this space, which renders the practice of mediastinal pathology, almost a general surgical pathology practice. However, there are numerous mediastinal conditions that are important not only for diagnosis but also for treatment and clinical outcome. Due to the unusual location and because of the complexity of the tumors occurring in the mediastinum, there are only a few experts in this particular anatomic area. In fact, there are no more than 3-4 individuals worldwide with enough experience to properly diagnose, classify, and make treatment suggestions when these tumors arise in the mediastinum. More recent developments in tumor diagnosis and classification have created a vacuum in the general pathologist who often is confronted with this pathology and becomes uneasy about the specifics. For instance, recent developments in tumor pathology have led to the classification and staging of mediastinal tumors in more innovative ways that are in need of proper highlighting. Needless to say, even though there are a few publications on mediastinal pathology, the books are either obsolete or incomplete, in addition of providing superficial knowledge. Unfortunately and with all due respect to the authors of those publications, neither of those books is written by experts in the area. Rather the books were written out of the lack of information in mediastinal pathology. Even the World Health Organization (WHO) publication, which is truly more on lung and pleural neoplasms, has inserted in its contents a portion of mediastinal tumors, such portion not only is superficial and incomplete but also at least highly controversial if not flat out incorrect. . This text’s goal is to present comprehensive and up to date information of the diverse pathology that can be encountered in the mediastinal compartment. The book will be divided in 11 different chapters based on the lineage of the diverse tumors that occur in the mediastinum, i.e. epithelial, neuroendocrine, lymphoid, etc (see table of contents). In addition, two separate chapters will be included -one on the radiological aspects and the others on the surgical approach to the different mediastinal tumors., bringing a total of 13 chapters. In the pathology section, the book will include current staging and classification systems of the different tumors. In addition, it will address the most current criteria for diagnosis and the differential diagnosis. Although the emphasis of the book is on diagnostic surgical pathology, every chapter will include the use of immunohistochemical techniques, electron microscopy, and molecular biology when necessary. The book will be extensively illustrated so that the reader will benefit of the different histomorphology of mediastinal tumors. Because of the nature of this specific anatomic area and the need to develop a standard text, Mediastinal Pathology should be of high interest to general surgical pathologists, thoracic pathologists in particular as well as thoracic surgeons, oncologists, and radiologists. Needless to say, this book will be of interest to pathology residents and fellows as well as to those seeking more in depth knowledge of thoracic – mediastinal pathology.
