The aim of the book is to analyse why poor nursing care is still occurring within care homes and hospitals in the UK, the USA, and Australia, despite numerous recommendations over more than 30 years. It became evident through the course of research that provision of care depended on the dominant political ethos, sometimes termed free market ideology or neoliberalism – terms describing an economic theory which claims the market should be the sole determinant of people’s lifestyle choices, and that the market should be free from government interference, since it is self-regulating. The Commodity of Care documents the pervasive influence of some neoliberal reforms of healthcare. The emphasis will be on what negates good nursing care delivered with a compassionate attitude. Any reform, from whatever political perspective, that promotes good, compassionate care should be supported by all nurses, who should equally oppose measures that lead to poor and indifferent care.

Equally, large multi-national companies like Wal-Mart, Nestle, or Coca-Cola are inherently complex and issues and relationships cut across functions, levels, geographies, and companies. Cross-Enterprise Leadership goes beyond a functional perspective to understanding the complexity of business issues from all angles and how they can be integrated, how leaders can rely almost entirely on influence when they may be operating without power or authority, and how they can develop the capacity to make decisions and implement them in an environment filled with uncertainty and complexity. Most managers operate like the traditional orchestra-waiting to do their written part. But there is no tidy score for business today. CEL enables today's leaders to be more like a jazz band, improvising and building off of one another, creating music in real time and in relationship to one another.

Care in care homes is a highly skilled area and recognition of its importance is sadly lacking. The author has identified a number of dilemmas of care, based upon her considerable experience within the care home sector, supported where possible with reference to research. Staff need guidance in resolving these dilemmas. There is often a conflict between residents needs and wishes, staff's personal and professional aims, regulations and the requirements of inspectors. This book services to remind us who we are caring for and, hopefully, it will promote recognition of the skills needed to provide care in care homes and as well as aiding the decisions made by the staff who work in these homes.
