In this era of proliferation of synthetic growth hormone in the marketplace, there is a parallel and accentuated interest in growth hormone in the scientific arena. Because many more people can be treated with available growth hormone, clinicians must be prepared to answer hard questions regarding appropriate therapeutic usage and their decisions should be based on substantiated research in growth hormone. In June 1987, an international group of basic and clinical inves tigators gathered in Tampa, Florida, to address these issues and to further explore the very nature of growth hormone. The presentations contained within this book bring together their most current and vital research related to growth hormone. Section I deals with an examination of the molecular and biochemical events which define the growth hormone process. In Section II the neuroregulation of growth hormone secretion is highlighted from contrasting perspectives. The third section emphasizes and defines methods of diagnosis of growth hormone deficiency states. Section IV reviews the physiology, biochemistry and molecular actions of growth hormone and somatomedin. Section V represents an assessment of growth hormone treatment for various disorders, and the sixth section expands current uses of growth hormone therapy as it evolves into the next decade. The symposium upon which this book is based proved to be a dynamic blending of scholarly interaction between basic and clinical scientists. I am indebted to the participants whose worthy contributions are reflected in these pages.
Most histories of the U.S. Army in World War II view the Mediterranean Theater of Operations primarily as a deadly training ground for very green forces, where lessons learned on the beaches of Oran, in the hills of the Kasserine Pass area, and at the collapse of the Tunis bridgehead all contributed to later success in Western Europe. Steven Barry, however, contends that victory in the MTO would not have materialized without the leadership of battalion-level commanders. They operated at a high level, despite the lack of combat experience for themselves and their troops, ineffective leadership at higher levels, and deficiencies in equipment, organization, and mobilization. Barry portrays these officers as highly trained, adaptable, and courageous in their first combat experiences in North Africa and Sicily. Their leadership, he argues, brought discipline, maturity, experience, and the ability to translate common operational guidance into tactical reality, and thus contributed significantly to battlefield success in North Africa and Sicily in 1942-1943. To explain how this happened, he examines their prewar experiences, including professional military education and unit training exercises; personal factors such as calmness and physical resilience under fire; and the ability to draw upon doctrine, creatively apply the resources at their disposal, and clearly define and communicate mission goals and means. He also reveals how battalion leaders incorporated technological innovations into combined arms maneuvers by employing tank capabilities and close air support doctrine. As Barry's assessment shows, these battalion commanders were not the sole reason for the Allied triumph in North Africa and Sicily, but victory would not have been possible without the special brand of military leadership they exhibited throughout those campaigns. Under their leadership, even inexperienced units were able to deliver credible combat performance, and without the regular army battalion leaders, U.S. units could not have functioned tactically early in the war. One of the few studies to focus on tactical adaptation at the battalion level in conventional warfare, Barry's book attests to the pivotal value of professional military education-and makes an important contribution to today's "organizational learning" debate-while providing an in-depth view of adaptation of U.S. infantry and armored forces in 1942-1943.

Changes in the allocation of healthcare resources have raised issues related to the efficacy and outcomes of medical therapy and how such factors may be measured. The questions associated with the quality of life and functional capability of patients with chronic health conditions have been of special interest. Endocrine disorders have the potential for disrupting the general health and well-being of affected individuals and their families; thus they warrant serious attention. This symposium was convened in November 1997 at Palm Beach Gardens, Florida, to bring together medical, behavioral, and social scientists. The meeting fostered the presentation and discussion of the most current clinical research on the effects of various therapies on a wide range of endocrine disorders from diabetes to adrenal insufficiency and growth hormone deficiency. The participants, all noted national and international experts in their fields, focused their attention on both the biomedical value and effectiveness of treatment, as well as on the impact such treatments have on psychological states such as mood and cognition. Many presentations specifically emphasized the quality of life (QOL) indicators that now regu larly appear in many research protocols and reports. The pioneering work of two clinical researchers was a major highlight of the meeting and an Award of Recognition was presented to Robert Blizzard, M. D. , and John Money, Ph. D. , for their innovative and insightful work in pediatric endocrinology and psychosexual development.
Bear Traps examines Russia’s longer term economic growth prospects. It argues that Russia’s growth challenges are conventionally misdiagnosed and examines the reasons why: a spatial misallocation that imposes excess costs on production and investment; distortions to human capital; an excessively high relative price of investment that serves as a tax on physical capital accumulation; and an economic mechanism that inhibits adjustments that would correct the misallocation. Bear Traps explains why Soviet legacies still constrain economic growth and outlines a feasible policy path that could remove these obstacles. The most popular proposals for Russian economic reform today — diversification, innovation, modernization — are misguided. They are based on a faulty diagnosis of the country’s ills, because they ignore a simple reality: Russia’s capital, both physical and human, is systematically overvalued, owing to a failure to account for the handicap imposed by geography and location. Part of the handicap is an unavoidable consequence of Russia’s size and cold climate. But another part is self-inflicted. Soviet policies placed far too much economic activity in cold, remote locations. Specific institutions in today’s Russia, notably its federalist structure, help preserve the Soviet spatial legacy. As a result, capital remains handicapped. Investments made to compensate for the handicaps of cold and distance should properly be treated as costs. Instead, they are considered net additions to capital. When returns to what appear to be large quantities of physical and human capital fail to satisfy expectations, the blame naturally goes to poor institutions, corruption, backward technology, and so on. Policy proceeds along the wrong path, with costly programs that can end up doing more damage than good. The authors insist that the goal should be to seek to remove the handicaps rather than to spend to compensate for them. They discuss how Russia could develop a modernization program that would let the nation finally focus on its economic advantages, not its handicaps.
