In an incubated petri dish in a Boston laboratory, a cluster of eight living cells holds the key to a daring new era of medical science. These cells come from a human embryo, containing genetic material from my wife, Paulina, and me, obtained through in vitro fertilization (IVF). Research using these cells holds enormous potential to cure a fatal disease, saving the lives of countless children and relieving their families of unspeakable pain. We lived that pain ourselves. When our son was born we didn't know what a tortuous road stretched ahead of us- how our baby would suffer, and how we'd suffer with him. We didn't know he'd spend nearly 1,000 days in seven different hospitals, see more than 300 doctors and swerve close do death many times. We didn't know we'd have to take matters into our own hands to find the medical miracle that would save him. Nine years ago, Paulina and I embarked on a desperate quest. We needed to create a baby with the right genetic profile, whose umbilical cord blood could save our 2-year-old son, Andy, dying from a rare genetic disease. The baby would also have to be free of the inherited disease killing our son. After three years and five IVF cycles, and with the critical intervention of a novel medical technology called pre-implantation genetic diagnosis (PGD), our daughter, Sofia, was born. Her cord blood stem cells, transplanted into her brother via blood transfusion, replaced his faulty immune system and saved his life. Today we are blessed that Andy, 11, and Sofia, 6, are both healthy, beautiful children. In gratitude, Paulina and I donated the remaining frozen IVF embryos-the ones that could never be used because they carried the flawed gene causing the disease-to the Stem Cell Research Program at Children's Hospital Boston, where Andy's disease was diagnosed and cured. Scientists there were able to develop two new stem cell lines whose unique genetic properties will help researchers learn how Andy's disease, and many others, develop. From that knowledge will come treatments and cures. My wife and I know our story and its difficult moral choices is highly controversial. We've chosen to share it to give the issue a human face and voice-and to give hope to other families facing similar dilemmas.

The violence that he witnessed led him to support Castro's revolution, and his brother Mario introduced him to Castro's 26th of July Movement (M267). Minor league baseball brought him to the United States, where he hoped to pursue a career in the majors, and left Villamia uniquely placed to aid Castro's revolution from abroad. From Tampa, New York City, Bridgeport, Union City, Miami, and Key West, the Villamias, Angel Perez-Vidal, Howard K. Davis and others supported Castro through fundraising, collecting supplies for the revolutionaries, propaganda campaigns, and arms smuggling. Raul rubbed elbows with Castro and his top men and with American gangsters who did business in Cuba. He was hounded by the FBI, and his brother Mario is mentioned in the Warren Commission Report. This memoir recalls Villamia's experience as an advocate for Castro in the United States and tells the story of those in America whose efforts helped to oust Batista.

The incorporation of new diagnostic and therapeutic interventions in clinical hepatology as well as liver transplantation has exponentially grown and specialty fields have emerged within the liver disease arena. Interventional procedures play a critical role in the management of patients with hepatobiliary diseases as they offer diagnostic and definite treatment options for an array of conditions. Interventions include those that directly measure portal pressure, perform transjugular liver biopsy and place transjugular intrahepatic portsosystemic shunts as therapy for complications of portal hypertension. Other commonly performed intervention is digestive and hepatobiliary endoscopy which offers not only diagnostic but also therapeutic choices in various areas such as portal hypertension and hepatobiliary disease. Endoscopy plays a fundamental role in the management of patients with almost all types of liver disease. Additionally, both the fields of hepatology and endoscopy have become very specialized and thus a thorough knowledge of the indications, findings, therapeutic possibilities and complications that arise from endoscopic interventions is a must for the practicing clinician. The rising incidence of liver cancer has also placed radiofrequency ablation and chemoembolization of liver lesions at the forefront of interventional therapies for hepatocellular carcinoma. Finally the management of acute liver failure has certainly evolved with new therapeutic options such as liver assist devices that can help manage these patients in the intensive care unit. This issue of Clinics in Liver Disease devoted to Interventional Hepatology is a timely and unique one. The Guest Editors have assembled an outstanding group of experts in several fields that can provide practitioners with state of the art and evidence based articles that help manage patients with hepatobiliary disease.

Latin America and the Caribbean continues to face adverse global headwinds: high interest rates, modest G-7 growth, soft commodity prices and uncertain prospects in China will all depress growth. Well-grounded policy responses have led to largely recovering employment and income losses from the pandemic and falling rates of inflation. However, the region faces the mutually reinforcing triple challenges of low growth, limited fiscal space, and citizen dissatisfaction. Expanding digital connectivity offers a possibility to make progress on all three fronts. To maximize the social benefits of connectivity as well as to ensure that it does not exacerbate spatial, educational, gender or racial inequalities, three challenges are important to address: first, expanding coverage to the remaining unconnected areas as well as improving the quality of service; second, increasing the productive use of existing infrastructure, and; third, as with any other infrastructure "hardware," investments in "software" - such as digital and traditional skills, managerial capabilities, supportive regulatory frameworks, and deeper financial markets are critical.

Fluctuations in commodity prices are an important driver of business cycles in small emerging market economies (EMEs). We document how these fluctuations correlate strongly with the business cycle in EMEs. We then embed a commodity sector into a multi-country EMEs’ business cycle model where exogenous fluctuations in commodity prices follow a common dynamic factor structure and coexist with other driving forces. The estimated model assigns to commodity shocks 42 percent of the variance in income, of which a considerable part is linked to the common factor. A further amplification mechanism is a ”spillover” effect from commodity prices to risk premia.

From late 1960 until the October 1962 missile crisis, 14,048 unaccompanied Cuban children left their homeland, the small island suddenly at the center of the Cold War struggle. Their parents, unable to obtain visas to leave Cuba, believed a short separation would be preferable to subjecting their offspring to Castro's totalitarian Marxist state. For the children, the exodus began a prolonged and tragic ordeal--some didn’t see their parents again for years; a few never did. Until now, this chapter of the Cuban Revolution has been relatively obscure. Initially the result of an effort by James Baker, headmaster of an American school in Cuba who worked closely with the anti-Castro underground, Pedro Pan quickly came to involve the Catholic Church in Miami and, in particular, Father Bryan Walsh, who established the Cuban Children's Program, the nationwide organization that cared for those children without relatives or friends in the United States--almost half of them. The latter program, in effect until 1981, was the first to allot federal money to private agencies for child care, an action with far-reaching repercussions for U.S. social policy. Victor Andres Triay traces this story from its political and social origins in Cuba, setting it in the context of the Cold War and describing the roles of the organizations involved in Cuba and in the United States. Reyes Córdova is a young boy tired of being poor and feeling hopeless. Reyes learns from teachers that mastering English can help him become more American and that will give him an opportunity for a good job. He becomes obsessed with learning the language, a task made difficult by his handicaps—illegitimate, a mother who speaks only Spanish, subsisting on public welfare—in addition to being part of a culture that promotes conformity, immediate gratification, close family relationships and xenophobic rejection of Anglophone society. Reyes’ story is told in a poignant and picaresque series of journal-like portraits that trace his emergence from the mystical realm of Cibola that is a blend of an ancient Pueblo culture, an archaic Spanish heritage, and an encroaching American dominion in the age of Eisenhower and its cataclysmic events—the hydrogen bomb, the Communist Menace, Sputnik, accelerated farm-to-urban migration, and momentous protests for minority and women’s rights.
