Various applications in the field of pulmonary image analysis require a registration of CT images of the lung. For example, a registration-based estimation of the breathing motion is employed to increase the accuracy of dose distribution in radiotherapy. Alexander Schmidt-Richberg develops methods to explicitly model morphological and physiological knowledge about respiration in algorithms for the registration of thoracic CT images. The author focusses on two lung-specific issues: on the one hand, the alignment of the interlobular fissures and on the other hand, the estimation of sliding motion at the lung boundaries. He shows that by explicitly considering these aspects based on a segmentation of the respective structure, registration accuracy can be significantly improved.

Noninvasive brain stimulation, particularly transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), are emerging as realistic tools for seizure control. Numerous open-label trials and a few recent randomized controlled trials suggest the capacity of both techniques to suppress seizures. Additionally, specialized TMS protocols aimed to map cortical function and to measure cortical excitability may have realistic roles as diagnostic tools in epilepsy. As the prevalence of drug-resistant epilepsy has not changed in recent years, TMS and tDCS offer noninvasive and nonpharmacological options to improve control of intractable seizures.

Deep brain stimulation (DBS) is a neurosurgical intervention whose efficacy, safety, and utility have been shown in the treatment of movement disorders. For the treatment of chronic pain refractory to medical therapies, many prospective case series have been reported, but few have published findings from patients treated during the past decade using current standards of neuroimaging and stimulator technology. We summarize the history, science, selection, assessment, surgery, and personal clinical experience of DBS of the ventral posterior thalamus, periventricular/periaqueductal gray matter, and, latterly, the rostral anterior cingulate cortex (Cg24) in 100 patients treated now at two centers (John Radcliffe Hospital, Oxford, UK, and Hospital de São João, Porto, Portugal) over 12 years. Several experienced centers continue DBS for chronic pain with success in selected patients, in particular those with pain after amputation, brachial plexus injury, stroke, and cephalalgias including anesthesia dolorosa. Other successes include pain after multiple sclerosis and spine injury. Somatotopic coverage during awake surgery is important in our technique, with cingulate DBS considered for whole-body pain or after unsuccessful DBS of other targets. Findings discussed from neuroimaging modalities, invasive neurophysiological insights from local field potential recording, and autonomic assessments may translate into improved patient selection and enhanced efficacy, encouraging larger clinical trials.
Vasculitis is a primary phenomenon in autoimmune diseases such as polyarteritis nodosa, Wegener’s granulomatosis, Churg–Strauss syndrome, microscopic polyangiitis, and essential mixed cryoglobulinemia. As a secondary feature vasculitis may complicate, for example, connective tissue diseases, infections, malignancies, and diabetes. Vasculitic neuropathy is a consequence of destruction of the vessel wall and occlusion of the vessel lumen of small epineurial arteries. Sometimes patients present with nonsystemic vasculitic neuropathy, i.e., vasculitis limited to peripheral nerves and muscles with no evidence of further systemic involvement. Treatment with corticosteroids, sometimes in combination with other immunosuppressants, is required to control the inflammatory process and prevent further ischemic nerve damage.

One of his first missions is dealing with a barrage of media questions following the brutal murder of six Royal Military Police by a crazed mob in Majar al-Kabir. Abbott recalls the adrenalin-filled atmosphere when the British garrison at Basra Palace is surrounded by a crowd unleashing hundreds of rounds from their AK-47s. It’s only after a tense stand-to that the nervous troops discover that they are not under attack: the crowd is celebrating the demise of Saddam Hussein’s sons. There are plenty of lighter moments, too, as Abbott tells stories that fortunately didn’t make the news at the time. The author admits how criminal thoughts might just have briefly crossed his mind over the tens of millions of dollars flown in by the US government each week and stored feet from his camp-bed. Above all this is a vivid account of a controversial operation that cost many lives and severely tarnished the reputation of British Prime Minister Tony Blair and US leadership. Unconvinced of the merits of military action before arriving in theater, Abbott ends his tour in a positive mindset despite the failure to locate WMD. His diary is more SOS than SAS, with little of the derring-do of an Andy McNab adventure. Yet it’s just as un-put- downable. More in the tradition of Leslie Thomas’ Virgin Soldiers, it’s an honest, authentic and often funny read which has the potential to appeal beyond a niche audience. There has been no account of the British in Iraq quite like this.
