Using real-life cases describing patients with neuro-ophthalmic disorders, this book is a case-based teaching tool that bridges the gap between textbook information and everyday clinical practice. Each case illustrates a particular area of frequent diagnostic confusion, and highlights the specific clinical features that should point to the correct diagnosis. Focusing on errors in this way serves as motivation to the clinician to master the material so that 'pitfalls' can be avoided. The level of the case discussions assumes that the reader has some familiarity with basic neuroanatomy, physiology and disease process but each case discussion furnishes a brief review of such information, always with an emphasis on those features that are clinically relevant. The case-histories are succinct and amply illustrated, including motility and fundus photographs, visual fields and radiographic studies. The narrative is moved along with questions to the reader, making it easy to follow the logic of the cases.
What does it take to survive? This is the question posed by the extraordinary Finnish novella that has taken the Nordic literary scene by storm. 1867: a year of devastating famine in Finland. Marja, a farmer's wife from the north, sets off on foot through the snow with her two young children. Their goal: St Petersburg, where people say there is bread. Others are also heading south, just as desperate to survive. Ruuni, a boy she meets, seems trustworthy. But can anyone really help? Why Peirene chose to publish this book: 'Like Cormac McCarthy's The Road, this apocalyptic story deals with the human will to survive. And let me be honest: There will come a point in this book where you can take no more of the snow-covered desolation. But then the first rays of spring sun appear and our belief in the human spirit revives. A stunning tale.' Meike Ziervogel ' White Hungeris Aki Ollikainen's debut work, but it is written with the control of someone who has mastered the form.' Nicholas Lezard, Guardian 'Such a powerful, honest and thought-provoking story deserves an audience far beyond the shores of Scandinavia.' Pam Norfolk, Lancashire Evening Post 'Impossible not to respond to its raw, unsparing drama.' Elizabeth Bucan, Daily Mail 'A tale of epic substance compacted into a mere seven-score pages.' Ben Paynter, Los Angeles Review of Books

Untimely Sacrifices questions why individuals may give their time and energy to the collective against their own self-interest. Turning to Finland where public health officials named occupational burnout as a "new hazard" of the new economy, Daena Funahashi asks: What moves people to work to the point of pathological stress? Contrary to health experts who highlight the importance of self-management and energetic conservation, Funahashi questions the very economic premise of cognitive psychology that one could "economize" one's energy and thus save oneself. By pitting anthropological takes on sacrifice next to the clinical discourses on pressure, work, and coping, Funahashi offers ways to rethink what drives stress. Untimely Sacrifices also provides a compelling critique of state welfare and political economy, contesting the tendency to treat the gift economy as something separate from the force that makes redistributive mechanisms of state welfare work. It is a book essential to those interested in how forces unassimilable to conventional economy come to matter in issues of labor, stress, and welfare.

A beautifully evocative account of one man’s odyssey to discover authentic and unbroken magical traditions in the East and reawaken them in the West • Details the author’s encounters with the Naga Babas, his initiation into their tradition, and his experience at the Kumbh Mela, the largest spiritual gathering on Earth • Shares the similarities he discovered between the teachings of the Indian tradition and the Western traditions of magic, alchemy, and pagan pantheons • Introduces a wide cast of characters, including Goa Gil, the world-renowned guru of the Goa techno-trance scene, and Mahant Amar Bharti Ji, a “raised-arm Baba,” who for more than 40 years has held up one arm in devotion to Shiva Beautifully detailing his spiritual pilgrimage from West to East and back again, in the age of strife known as the Kali Yuga, Aki Cederberg shares the authentic and unbroken magical traditions he experienced in India and Nepal and how his search for a spiritual homeland ultimately led him back to his native Europe. Cederberg explains how his odyssey began as a search for spiritual roots, something missing in the spiritually disconnected life of the Western world, where the indigenous traditions were long ago severed by the spread of Christianity. Traveling to India, he encounters the ancient esoteric order of mystic, wild, naked holy men known as the Naga Babas, the living source of the Hindu traditions of magic and yoga. Immersing himself in the teachings of the tradition, he receives an initiation and partakes in the Kumbh Mela, the largest spiritual gathering on Earth. With his evocative descriptions, Cederberg shows how traveling in India can be an overwhelming, even psychedelic experience. Everything in this ancient land is multiplied and manifold: people and things, sights and sounds, joy and suffering. Yet beyond the apparent confusion and chaos, a strange, subtle order begins to reveal itself. He starts to glimpse resemblances and analogies between the teachings of the Indian tradition and the Western traditions of magic, alchemy, and pagan pantheons. He meets a wide cast of characters, from mystical hucksters in Rishikesh and the veritable army of naked, chillum-smoking mystics of Maya Devi to Goa Gil, the world-renowned guru of the Goa techno-trance scene, and Mahant Amar Bharti Ji, an urdhvabahu or “raised-arm Baba,” who for more than 40 years has held up one arm in devotion to Shiva. After extensive traveling and immersing himself in the extraordinary world of India, Cederberg returns to his native soil of Europe. Traveling to holy places where old pagan divinities still linger in the shadows of the modern world, he dreams of forgotten gods and contemplates how they might be awakened yet again, reconnecting the West with its own pre-Christian spiritual traditions, sacred landscapes, and soul.
Embrace the joy of Paris wherever you live with American expat Ajiri Aki, founder of the French lifestyle brand Madame de la Maison. “More than being a terrific guide to the city, it’s a thoroughly comprehensive guide to better living.”—Chioma Nnadi, editor of Vogue online The French are known for their joie de vivre—celebrating the simple things—a philosophy that tastemaker Ajiri Aki embraced all of her American life. As a child, she frequently tried to convince her Nigerian-Jamaican mother to pull out the fine china for everyday meals or when hosting friends. Her mother always said she was waiting for a special occasion, which sadly never came before she passed away when Ajiri was only twelve. Ajiri promised herself she would never hesitate to use her treasured pieces. When she moved to Paris, France, as an adult, she learned how central that idea is to French life, and she also began to absorb other essential lessons from her new friends: treat yourself to fresh flowers just because, take time to source the best baguette, and perhaps most importantly, enjoy être—just being. In this beautifully photographed volume of everything French, Ajiri shares what she’s learned about living in Paris—from hosting the perfect apéro (happy hour) to lingering around town like a flâneur (loafer) to thrifting for antiques at the market. While exploring the prettiest cafes and shops, you’ll be inspired to reclaim your right to leisure as the French have, so you, too, can savor the spontaneous, joyful moments that happen every day.

There are not enough resources in health care systems around the world to fund all technically feasible and potentially beneficial health care interventions. Difficult choices have to be made, and economic evaluation offers a systematic and transparent process for informing such choices. A key component of economic evaluation is how to value the benefits of health care in a way that permits comparison between health care interventions, such as through costs per quality-adjusted life years (QALY). Measuring and Valuing Health Benefits for Economic Evaluation examines the measurement and valuation of health benefits, reviews the explosion of theoretical and empirical work in the field, and explores an area of research that continues to be a major source of debate. It addresses the key questions in the field including: the definition of health, the techniques of valuation, who should provide the values, techniques for modelling health state values, the appropriateness of tools in children and vulnerable groups, cross cultural issues, and the problem of choosing the right instrument. This new edition contains updated empirical examples and practical applications, which help to clarify the readers understanding of real world contexts. It features a glossary containing the common terms used by practitioners, and has been updated to cover new measures of health and wellbeing, such as ICECAP, ASCOT and AQOL. It takes into account new research into the social weighting of a QALY, the rising use of ordinal valuation techniques, use of the internet to collect data, and the use of health state utility values in cost effectiveness models. This is an ideal resource for anyone wishing to gain a specialised understanding of health benefit measurement in economic evaluation, especially those working in the fields of health economics, public sector economics, pharmacoeconomics, health services research, public health, and quality of life research.

Al Qaeda did not stop after 9/11. Its reign of terror continued with bombings and mayhem across Europe, Africa, Asia, and the Middle East. But its frustration grew as the group failed to fundamentally undermine America and its allies. Five years later the time was ripe for another spectacular mega-plot. Fresh from masterminding the London Underground carnage, one veteran operative set in motion a new operation to destroy passenger aircraft over the Atlantic Ocean—and kill thousands of people in the process. Disruption tells the story of that conspiracy and the heroic efforts by the intelligence services of the United States, Great Britain, and Pakistan to uncover and crush it. From the streets of London to the training camps of Pakistan to the corridors of power in Washington DC, the story unfolds with murders, double-crosses, probes, jailbreaks, and explosions. Former counterterrorism analyst Aki J. Peritz brings the story to life with vivid imagery, interviews with top intelligence officials, and never-before-seen declassified documents. Disruption is the not-to-be-missed account of the race to stop a terrorist conspiracy that would have remade our world—forever.

This book focuses on all-pairwise multiple comparisons of means in multi-sample models, introducing closed testing procedures based on maximum absolute values of some two-sample t-test statistics and on F-test statistics in homoscedastic multi-sample models. It shows that (1) the multi-step procedures are more powerful than single-step procedures and the Ryan/Einot–Gabriel/Welsh tests, and (2) the confidence regions induced by the multi-step procedures are equivalent to simultaneous confidence intervals. Next, it describes the multi-step test procedure in heteroscedastic multi-sample models, which is superior to the single-step Games–Howell procedure. In the context of simple ordered restrictions of means, the authors also discuss closed testing procedures based on maximum values of two-sample one-sided t-test statistics and based on Bartholomew's statistics. Furthermore, the book presents distribution-free procedures and describes simulation studies performed under the null hypothesis and some alternative hypotheses. Although single-step multiple comparison procedures are generally used, the closed testing procedures described are more powerful than the single-step procedures. In order to execute the multiple comparison procedures, the upper 100α percentiles of the complicated distributions are required. Classical integral formulas such as Simpson's rule and the Gaussian rule have been used for the calculation of the integral transform that appears in statistical calculations. However, these formulas are not effective for the complicated distribution. As such, the authors introduce the sinc method, which is optimal in terms of accuracy and computational cost.

Ruri convinces Jade to let her go to the battlefield to see for herself, and though the draconian soldiers are mostly just batting the humans around like cats playing with mice, Asahi has the means to turn this mock battle into a true tragedy...
This book focuses on multiple comparisons of proportions in multi-sample models with Bernoulli responses. First, the author explains the one-sample and two-sample methods that form the basis of multiple comparisons. Then, regularity conditions are stated in detail. Simultaneous inference for all proportions based on exact confidence limits and based on asymptotic theory is discussed. Closed testing procedures based on some one-sample statistics are introduced. For all-pairwise multiple comparisons of proportions, the author uses arcsine square root transformation of sample means. Closed testing procedures based on maximum absolute values of some two-sample test statistics and based on chi-square test statistics are introduced. It is shown that the multi-step procedures are more powerful than single-step procedures and the Ryan–Einot–Gabriel–Welsch (REGW)-type tests. Furthermore, the author discusses multiple comparisons with a control. Under simple ordered restrictions of proportions, the author also discusses closed testing procedures based on maximum values of two-sample test statistics and based on Bartholomew's statistics. Last, serial gatekeeping procedures based on the above-mentioned closed testing procedures are proposed although Bonferroni inequalities are used in serial gatekeeping procedures of many.
